- NA This is a combined synopsis/solicitation for commercial item(s) prepared in accordance with FAR12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation. Quotes are being requested and a written solicitation will not be issued. DADA15-03-T-0132 is issued as a Request for Quotation (RFQ). The Solicitation document and incorporated Provisions and Clauses are those in effect through Federal Acquisition Circular (FAC) 2001-14. This Procur ement is unrestricted, the SIC Code is 2835; the North American Industry Classification System (NAICS) code is 325413; the FSC 6550. The Medical Command Contracting Center-North Atlantic (MEDCOM MCC-NA) intends to issue a firm fixed price Indefinite Deli very Indefinite Quantity contract. based on FAR13 for laboratory testing of Kennedyâs Disease (SEMA) DNA test, Spinal Muscular Atrophy DNA test, Autosomal Dominant Ataxia Evaluation, Complete Ataxia Evaluation, Dystonia (DYT1) DNA Test, Friedreichâ s Ataxia DNA Test, Huntingtonâs Disease DNA Test, Complete Myotonic Dystrophy Evaluation, Glycogen Storage Myopathy âAâ Profile, Glycogen Storage Myopathy âBâ Profile, LHON mtDNA Profile, Lipid Storage Myopathy Profile, MELAS mtD NA Profile, MELAS DNA Test, MERRF mtDNA Profile, mtDNA Encephomyopathy Profile, Myoglobinuria Test Panel, LEMS Autoantibody Test, Autosomal Dominant CMT Evaluation, Complete HNPP Evaluation, GALOP Autoantibody Test, Partial CMT Evaluation Type 1 only, Part ial CMT Evaluation Type 2 only, Small Fiber Painful Axonal Neuropathy Profile, Transthyretin DNA Sequencing for base and four option years. Base year begins 1 July 2003 through 30 June 2004. First option year begins 1 July 2004 through 30 June 2005. Second option year begins 1 July 2005 through 30 June 2006. Third option year begins 1 July 2006 through 30 June 2007. Fourth option year begins 1 July 2007 through 30 June 2008. Delivery terms shall be FOB Destination, Walter Reed Army Medical Center, Client S ervice/Directorate of Pathology, Building 2, Room 2B15, 6900 Georgia Avenue NW, Washington, DC 20307. The following Provisions and Clauses are incorporated and shall remain in full force in any resulting purchase order: FAR Part 52.212-1 Instruction to O fferors - Commercial Items, FAR 52.212-2 EvaluationâCommercial Items, FAR 52.212-4 Contract Terms and Conditions - Commercial Items. Award will be made based on technical capability, past performance, and price. Proposals must include technical capabi lity proof of laboratory licensure and accreditation (CLIA AND CAP). Appropriate certifications must be maintained for the life of the contract. Contractor may be asked to provide proof at any time throughout contract. Scope of work is as follows: Laborato ry/Contractor will perform all the requested laboratory tests in compliance with Centers for Medicare and Medicaid Clinical Laboratory Improvement Amendments http://www.cms.hhs.gov/clia/). Laboratory shall provide all equipments, shipping containers, reag ents and labor in order to perform tests on supplied specimen. Laboratory/Contractor shall fax test results as soon as available (within approx. 4 to 6 weeks depending of the type of test) to Client Services. Subsequent hard copy results will be mailed. Laboratory/Contractor shall notify Client Services of any assay problems and critical results telephonically and by fax. Laboratory/Contractor will provide monthly invoice of test services rendered during the Period. For past performance information, prop osals must include contract number, name, address, point of contact, and telephone number of federal contracting office where prior work has been performed. Three references are required. The following Provisions and Clauses are also incorporated and shall remain in full force in any resulting purchase order: FAR 52.212-3 Officers Representations and Certification-Commercial Items, FAR 52.212-5 Contract Terms a nd Conditions Required to Implement Statues or Executive Orders Applicable to Defense Acquisitions of Commercial Items, FAR 52.216-18 Ordering, Any supplies and services to be furnished under this contract shall be ordered by issuance of delivery orders or task orders by the individuals or activities designated in the Schedule. Such orders may be issued from 1 October 2002 through 30 September 2003. FAR 52.216-19 Order Limitations, (a) Minimum Order. When the Government requires supplies or services covered by this contract in an amount of less than $25.00, the Government is not obligated to purchase, nor is the Contractor obligated to furnish those supplies or services under this contract. (b) Maximum order. The Contractor is not obligated to honor: (1) Any order for a single item in excess of $100,000.00, (2) Any order for a combination of items in excess of $200,000.00; or (3) A series of orders from the same ordering office within two days that together call for quantities exceeding the limitation on su bparagraphs (1) or (2). If this is a requirements contract (i.e. Includes the Requirements clause at subsection 52.216-21 of the Federal Acquisition Regualtion (FAR), the Government is not required to order a part of any one requirement from the Contractor if that requirement exceeds the maximum-order limitationns in paragraph (b) of this section. (d) Notwithstanding paragraph (b) and (c) of this section, the Contractor shall honor any order exceeding the maximum order limitations in paragraph (b), unless t hat order is returned to the ordering office within 3 days after issuance, with written notice stating Contractorâs intent not to ship the item called for and the reasons. Upon receiving this notice, the Government may acquire the supplies or services from another source. FAR 52.216-21 Requirements, FAR 52.217-5 Evaluations of Options, FAR 52.217-8 Option to Extend Services, The Government may require continued performance of any services within the limits and at the rates specified in the contract. Th ese rates may be adjusted only as a result of revisions to prevailing labor rates provided the Secretary of Labor. The option provision may be exercised more than once, but the total extension of performance hereunder shall not exceed six months. The Contr acting Officer may exercise the option by written notice to the Contractor within 30 days. FAR 52.217-9 Option to Extend The Term of the Contract, The Government may extend the term of this contract by written notice to the Contractor within 30 days; prov ided that the Government gives the Contractor a preliminary written notice of its intent to extend at least 60 days before contract expires. The preliminary notice does not commit the Government to an extension. If the Government exercises this option the extended contract shall be considered to include this option clause. The total duration of this contract including the exercise of any options under this clause, shall not exceed five years. Shipping shall be incorporated into test fee schedule for Walter Reed Army Medical Center. The Contractor shall fax copy of test results as available to Walter Reed Army Medical CenterâClient Services at 202-782-8241. Hard copy results shall be mailed to Client Service/ Directorate of Pathology, Walter Reed Army Medical Center, Building 2, Room 2B15, 6900 Georgia Avenue NW, Washington, DC 20307 . Payment will be made monthly by Government IMPAC Credit Card. Contractor shall provide monthly invoice for test services conducted during each month. The government reserves the right to make award on the initial quote received without discussions. The Go vernment will consider all quotes received by 20 June 2003.
