03R6017 ?GENECHIP SCANNER This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Federal Acquisition Regulation Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation. Proposals are being requested and a written solicitation will not be issued. T he Request for Quotation (RFQ)Number is F33600-03-R-6017. This solicitation and incorporated clauses /provisions are those in effect through Federal Acquisition Circular 2001-08 and DFARS Change Notice 19990416 The solicitation incorporates by reference the following clauses: FAR 52.212-1, 52.212-3, 52.212-4 and 52.212-5. NAIC Code: 443120. This is a sole source requirement IAW FAR 6.302-1 to: AFFYMETRIX, INC. 3380 CENTRAL EXPWY SANTA CLARA, CA 95051-0704 (408) 731-5667 Air Force Research Laboratory requires a Gene Chip Scanner System and LIMS database server. This RFQ has the following line items: Line Item 0001 ? Upgrade from GA2500 Scanner to GCS3000 Scanner Line Item 0002 ? Autoloader Upgrade with EBR and Holder Line Item 0003 ? Affymetrix LIMS Server System 1 Lite Line Item 0004 ? 2nd Yr. Service Contract, Yrly Inst + Bionf Software The Period of Performance is 30 DARO and 24 months for the service contract. FOB Destination.
