- Description
- Furnish all necessary supplies, equipment and supervision to collect specimens and provide various specialized laboratory tests on a cost-per-test basis to the Department of Veterans Affairs Medical Centers within VISN 7: Atlanta, GA; Augusta, GA; Dublin, GA; Birmingham, AL; Montgomery, AL; Tuscaloosa, AL; CAVHCS-Tuskegee, AL; Charleston, SC; and Columbia, SC. Contract period to be October 1, 2003 through September 30, 2004, with 4 one-year options to extend. This procurement is being conducted under FAR Part 12 and FAR Part 15. The solicitation is issued as a Request for Proposal (RFP). A technical proposal will be required. Technical evaluation criteria consists of: 1. QUALITY OF SERVICES - includes providing copies of all certifications that verify the latest inspection and recertification by the various agencies (i.e., CAP, JCAHO, AABB, CLIA); list of proficiency testing surveys; copies of the summary of performance for proficiency testing for the past 2 years; copies of Quality Improvement reports; 2. AVAILABILITY OF SERVICES - includes providing a copy of the Laboratory Testing Manual; policies and procedures for verifying test results prior to test reporting; policies and procedures for reporting immediate response (critical or panic) values and a list of the immediate response values as established by the contracted lab; the schedule of current and available operating hours for courier service, lab testing, and test results reporting; test result formats; a list of test with available (guaranteed) turnaround times; sample copies of invoices; policies and procedures for verifying sample identification; list of processing sites/testing location sites; list of sites that are interfaced with the VA VISTA computer system; 3. EXPERIENCE AND STAFFING - includes providng copies of laboratory's organization chart; list of experience/qualifications (i.e. resume) of the Medical Director and Professional personnel; listing of the pathologist staff to include educational background; job descriptions, experience/qualifications, and current certification/registration of the supervisory saff, technical staff and laboratory support staff; documentation of past performance over the past five (5) years that indicates company has performed reference laboratory testing services of this magnitude; 4. PRICING/FEE SCHEDULE. The NAICS Code is 621511 and the size standard is $11.5 million. The anticipated issue date is July 9, 2003.
