- This is a combined synopsis/solicitation for commercial items and services prepared in accordance with the format in FAR subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation. Proposals are being requested and a written solicitation will not be issued. Solicitation number F63CES31530100 is hereby issued as a Request for Quote. The incorporated provisions and clauses are those in effect through Federal Acquisition Circular 01-14. This acquisition is 100% set aside for small business concerns and is under North American Industry Classification Standard (NAICS) code 238990, with a size standard of $12 million. Contractor shall provide all labor, travel, material, supervision, and administration necessary to accomplish elevator maintenance and repair requirements in accordance with the attached Statement of Work. Award shall be made only to contractors who have registered with the Central Contractor Registration at: www.ccr.gov/. The following clauses and provisions are incorporated and are to remain in full force in any resultant purchase order: Far 52.236-27 Alternate I (Feb 1995), Site Visit. It is amended to read (b) an organized site visit has been scheduled for Tuesday, July 15, 2003, at 9:00 a.m. CST; (c) participants will meet at the contracting office located at 303 J Avenue, Bldg 302, Altus AFB, Oklahoma. FAR 52.212-1 (OCT 2000), Instructions to Offerors- Commercial Items is incorporated by reference. It is amended to read: Submit signed and dated offer to 97 CONS/LGCB, 303 J Ave., Bldg 302, Altus AFB OK 73523-5132 at or before 1630 (4:30 P.M.) CST, Monday July 21, 2003. Submit signed and dated quotations on company letterhead or the attached Bid Schedule Document. Quotes must contain pricing for all line items for the Base Year and the four Option Years. Quotations may be faxed to (580) 481-5138. The government reserves the right to make award on the initial quote without discussions. FAR 13.5 Test - Basis for Award, This is a competitive best value acquisition utilizing simplified procedures authorized by FAR 13.5, ?Test Program for Certain Commercial Items.? For quotations determined technically acceptable, a trade-off between price and past performance will be conducted. An offeror will be determined technically acceptable if no exception is taken to the specifications and requirements of the RFQ. Award will be based on both price and past performance. Each offeror?s quoted price will be evaluated for reasonableness. Price will always be an important factor; therefore, offerors should make diligent efforts to control costs and submit fairly priced quotes. Past performance will be evaluated based on relevancy (size/scope/complexity), quality of product/service, timeliness of delivery/performance and business relations (customer service). To be considered for award, an offeror must have a satisfactory or better past performance record based upon information available to the contracting officer, and offer a reasonable price. For past performance evaluation, the contractor shall include information concerning all relevant contracts/purchase orders for the same or similar service performed within the last five years (Government or Commercial). For each contract/purchase order, information submitted should contain at least a contract number, whether or not you were the prime or subcontractor, the total contract amount, point of contact and telephone numbers. Successful performance will be validated through interviews and/or correspondence with previous customers of the contractor. Past performance should demonstrate as a minimum, customer satisfaction and satisfactory quality of services. For price evaluation, the Government will evaluate offers for award purposes by adding the total price for all options to the total price for the basic requirement. The Government may determine that an offer is unacceptable if the option prices are significantly unbalanced. Evaluation of options shall not obligate the Government to exercise the options. Price will be evaluated for reasonableness and realism. Prices will be determined to be fair and reasonable based on competitor?s bids, current market conditions, and comparison to the government estimate, if applicable. Technical capability requirements will be satisfied with the submission of a position description of all on-site personnel to perform elevator maintenance on Altus Air Force Base. The position description should include as a minimum, years of experience or experience levels, and any certificates from schools or training courses for elevator repair and maintenance. Award may be made to other than the low offeror when, after evaluating past performance, the contracting officer determines it to be in the government?s best interest. Each offeror shall include a completed copy of the provisions at FAR 52.212-3 (Jul 2002), Offeror Representations and Certifications?Commercial Items with the quotation. A copy of the Representations and Certifications to be completed is attached and available for download. FAR 52.212-4 (FEB 2002), Contract Terms and Conditions?Commercial Items, is incorporated by reference and applies to this acquisition. FAR 52.212-4 is tailored as follows -- changes in the terms and conditions of this contract may be made only by written agreement of the parties with the exception of administrative changes (such as changes in paying office, appropriation data, etc.) which may be changed unilaterally by the Government.
