Description
Provide Health Care Resources in accordance with the Authority of Public Law 104-262 and 38 USC 8153. The contractor agrees to provide .10 FTE Board Certified Rheumatologist to perform Rheumatology Care Services in the Outpatient Clinic located at the Department of Veterans Affairs Medical Center (VAMC), 4100 West Third Street, Dayton, OH 45428. There is ONE clinic per week; for FOUR hours each clinic, or until ALL scheduled patients are seen and evaluated. The VAMC will use the following evaluation factors as they are defined in the solicitation. Past Performance, Technical Expertise, Price/Cost. The Government may award a contract on the basis of initial offers received, without discussion. Therefore each initial offer should contain the offerors best terms from a technical and cost/price standpoint. Each offeror must submit a separate technical proposal. The Prices/Costs will be furnished in the Schedule of Supplies/Service and Prices/Costs, as defined in the solicitation. It is anticipated that the award will be a base year, with 4 option year renewals. North American Industry Classification System Code (NAICS)applied is 621111/$8.5 million. This is an unrestricted commercial item procurement. The solicitation is avaiable as of June 30, 2003. The solicitation due date is Close of Business 4:00 PM, E.S.T., July 21, 2003. Qualified businesses wishing to receive a copy of the solicitation must submit their request in writing, on firm's letterhead, with the complete name, address, telephone number, fax number and a point of contact and must reference RFQ 552-08-03. Verbal, telephone and/or e-mail requests will not be accepted.
