Description
NA The VHA Medical/Surgical User Group, a group of VA physicians, nurses, and clinicians who have expertise and knowledge of the products, will review and technically evaluate any samples submitted. VA deems the members of the User Group to be medical experts. Therefore, they are qualified to make a subjective determination as to which products are acceptable. **Please send samples (number of samples required follow each product name) and corresponding detailed product literature to: Department of Veterans Affairs, Consolidated Acquisition and Analysis Service (049A5S), ATTN: Carl Hunter, Building 37, First Avenue - One Block North of Cermak Road (22nd Street), Hines, IL 60141** SAMPLES AND PRODUCT LITERATURE MUST BE RECEIVED NO LATER THAN JULY 25, 2003. Companies not submitting samples by this date may be eliminated from consideration on any subsequent procurement action for these items. For additional information regarding submission of samples please contact Carl Hunter (708) 786-7783. Product Descriptions/Criteria for items being considered at this time are as follows: ITEM 1. CLOSED TRACHEAL SUCTION SYSTEM (Samples: 10 of each product to be evaluated in either 14 or 16 French) The item described will be evaluated based on the following minimum requirements/subjective criteria and how well the offered product would satisfy the intended use: 1. Double swivel connection (Will be evaluated by a review of the product literature and physical inspection) 2. Leak-proof system for catheter (Will be evaluated by physical inspection) 3. Smooth, rounded tip (Will be evaluated by a review of the product literature and physical inspection) 4. At least 2 side holes and one end hole (Will be evaluated by physical inspection) 5. Catheter remains securely attached to the base (Will be evaluated by physical inspection) 6. Catheter fully retracts into the sheath (Will be evaluated by physical inspection) 7. Catheter does not cause airway obstruction (Will be evaluated by physical inspection) 8. Separate needleless lavage port (Will be evaluated by physical inspection) 9. Suction control mechanism contains a locking or protection feature (Will be evaluated by a review of the product literature and physical inspection) 10. Assorted sizes must include 14 and 16 French (Will be evaluated by a review of the product literature) 11. Leak-proof suction control mechanism (Will be evaluated by physical inspection) 12. Sterility of catheter must be maintained in a closed system (Will be evaluated by physical inspection) 13. Latex free product available (information only) (Will be evaluated by a review of the product literature) 14. Non-sticking suction control valve (Will be evaluated by physical inspection) 15. Instruction for use in package (Will be evaluated by physical inspection) 16. Opaque marker at tip (Will be evaluated by a review of the product literature and physical inspection) 17. Graduations in centimeters at no greater than 2-centimeter intervals (Will be evaluated by physical inspection) 18. Single lumen catheter (Will be evaluated by a review of the product literature and physical inspection) 19. Double lumen catheters available (information only) (Will be evaluated by a review of the product literature) 20. Variety of lengths available (Will be evaluated by a review of the product literature) 21. Product may be used for up to 72 hours (Will be evaluated by a review of the product literature) INTENDED USE _ Closed ventilation suction systems are designed for tracheal/bronchial suction of patients on ventilation support systems. ITEM 2. GOWN, PPE, DISPOSABLE (Samples: 20 of each gown to be evaluated) The item described will be evaluated based on the following minimum requirements/subjective criteria and how well the offered product would satisfy the intended use: 1. Non-sterile (Will be evaluated by a review of the product literature) 2. Disposable (Will be evaluated by a review of the product literature) 3. One-time use (Will be evaluated by a review of the product literature) 4. Latex free (information only) (Will be evaluated by a review of the product literature) 5. Garment has full front coverage from neck to knees and long sleeves to the wrist (Will be evaluated by a review of the product literature and physical inspection) 6. Design of sleeves prevents exposure of wrists (Will be evaluated by physical inspection) 7. Has a secure, attached waist closure (Will be evaluated by physical inspection) 8. Has a secure neck closure or slips over head (Will be evaluated by physical inspection) 9. Fabric meets ASTM 1670 dated 1997 (or the most current) for barrier effectiveness (Will be evaluated by a review of the product literature) 10. Fabric meets ASTM 1671 dated 1997 (or the most current) for barrier effectiveness (Will be evaluated by a review of the product literature) 11. Fabric has low flammability _ meets CPSC 16 CFR 1610 for Class 1 (Will be evaluated by a review of the product literature) 12. Does not tear easily when donning or removing (Will be evaluated by physical inspection) 13. Assorted sizes available to include universal, large, and extra large (Will be evaluated by a review of the product literature) INTENDED USE _ A personal protection garment to protect employees from blood and body fluids in the normal performances of their duties. NOT intended for use as environmental protection in the isolation setting. ITEM 3. ISOLATION GOWN, ENVIRONMENTAL (Samples: 20 of each gown to be evaluated) The item described will be evaluated based on the following minimum requirements/subjective criteria and how well the offered product would satisfy the intended use: 1. Non-sterile (Will be evaluated by a review of the product literature) 2. Ease of application (Will be evaluated by physical inspection) 3. Assorted sizes available to include universal, large, and extra large (Will be evaluated by a review of the product literature) 4. Disposable (Will be evaluated by a review of the product literature) 5. Lightweight material (Will be evaluated by physical inspection) 6. Non-linting material (Will be evaluated by physical inspection) 7. Durable under conditions of simulated use (Will be evaluated by physical inspection) 8. Long sleeves with secure cuffs (Will be evaluated by physical inspection) 9. Full coverage front and back (Will be evaluated by physical inspection) 10. Secure neck closure (ties, hook and loop, or tape) (Will be evaluated by physical inspection) 11. Tie waist closure (Will be evaluated by physical inspection) INTENDED USE _ For environmental protection by preventing transmission of microorganisms or other contaminants from one area to another. ITEM 4. CATHETER TRAY WITHOUT CATHETER (Samples: 1 Case or a minimum of 6 each of each product to be evaluated) The item described will be evaluated based on the following minimum requirements/subjective criteria and how well the offered product would satisfy the intended use: 1. Minimum 1000cc capacity plastic tray (Will be evaluated by a review of the product literature) 2. Minimum 5 cotton (or equivalent) balls in separate container/compartment (Will be evaluated by a review of the product literature and physical inspection) 3. Waterproof underpad of no less than 12_ x 12_ (Will be evaluated by a review of the product literature and physical inspection) 4. Fenestrated Drape (Will be evaluated by a review of the product literature and physical inspection) 5. Sterile, water soluble lubricating jelly, minimum of 2.5 Gm. (Will be evaluated by a review of the product literature and physical inspection) 6. Leakproof, screw cap specimen container, minimum 40cc (Will be evaluated by a review of the product literature and physical inspection by simulated use) 7. Sterile Kit (Will be evaluated by a review of the product literature) 8. Sterile Presentation (Will be evaluated by a review of the product literature and physical inspection) 9. Povidone Iodine 1% solution, minimum 30cc (Will be evaluated by a review of the product literature and physical inspection) 10. Disposable plastic forceps (Will be evaluated by a review of the product literature and physical inspection) 11. CSR wrapped tray (Will be evaluated by a review of the product literature and physical inspection) 12. Blister pack outer packaging (Will be evaluated by a review of the product literature and physical inspection) 13. Assembled in order of use (Will be evaluated by a review of the product literature and physical inspection by simulated use) 14. Latex free kit available (information only) (Will be evaluated by a review of the product literature) 15. One pair of powder-free gloves (Will be evaluated by a review of the product literature and physical inspection) 16. 10cc water or saline pre-filled syringe (Will be evaluated by a review of the product literature and physical inspection) 17. Disposable (Will be evaluated by a review of the product literature) 18. Single patient use (Will be evaluated by a review of the product literature) INTENDED USE _ For sterile catheterization of the urinary bladder. ITEM 5. SHOE COVER, ENVIRONMENTAL (Samples: 3 Boxes of each shoe cover to be evaluated) The item described will be evaluated based on the following minimum requirements/subjective criteria and how well the offered product would satisfy the intended use: 1. Various sizes to include small through x-large (Will be evaluated by a review of the product literature) 2. Disposable (Will be evaluated by a review of the product literature) 3. Packaged in non-corrugated boxes or liner that can be removed (Will be evaluated by a review of the product literature and physical inspection) 4. Non-skid sole (Will be evaluated by simulated use) 5. Elastic Ankle Gusset (Will be evaluated by physical inspection) 6. Durable (Will be evaluated by simulated use) 7. Easy to don (Will be evaluated by simulated use) 8. One time use (Will be evaluated by a review of the product literature) INTENDED USE _ Shoe covers designed to protect the environment by preventing the transmission of microorganisms and other contaminants from one area to another. ITEM 6. SHOE COVER, PPE CD (Samples: 3 Boxes of each shoe cover to be evaluated) The item described will be evaluated based on the following minimum requirements/subjective criteria and how well the offered product would satisfy the intended use: 1. Various sizes to include small through x-large (Will be evaluated by a review of the product literature) 2. Disposable (Will be evaluated by a review of the product literature) 3. Barrier effectiveness meets ASTM standards 1670 and 1671 dated 1999 (or the most current) (Will be evaluated by a review of the product literature) 4. Packaged in non-corrugated boxes or liner that can be removed (Will be evaluated by a review of the product literature and physical inspection) 5. Non-skid bottom (Will be evaluated by simulated use) 6. Elastic or tie ankle gusset (Will be evaluated by physical inspection) 7. Durable (Will be evaluated by simulated use) 8. Easy to don (Will be evaluated by simulated use) 9. Sealed impervious seams (Will be evaluated by a review of the product literature and physical inspection by simulated use) 10. Mid-calf length with elastic gusset (Will be evaluated by a review of the product literature and physical inspection) INTENDED USE _ Shoe covers used to protect the shoes and feet from blood or body fluids.
