Description

(i)This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. (ii) Solicitation of Purchase Request F73HOS31690800 is issued as a request for quotation (RFQ). (iii) This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 2001-05. (iv) (v & vi) This is a BRAND NAME acquisition for a GENERAL ELECTRIC Mobile MRI Unit. Line item 0001, Rent/Lease one (1) General Electric Signa 1.5T High Speed LX Mobile MRI Unit for 3 Months with the option to extend not to exceed one (1) year, Signa 1.5 High Speed LX Mobile MRI: LCC Ultra Short Bore, LX Octane, Phased Array, Color Monitor, Reflex 50 or 100 array processor, LX BAM, Scan Tools 2000, Smart Prep 2000, LX Tools 2000, Echo Plus, Func Tool 2000 and Probe 2000. Coil selection: Head Neuro-Vascular Array, Shoulder Array, Extremity, CTL Array, Torso Array and (2) 3? GP with Dual Array adapter, and GP Flex (Kodak 8100 Dry View Laser Printer ? 55 sheets per hour). Items MUST BE delivered by 1 August 2003. Acceptance will be FOB destination to 3rd Medical Group (viii) FAR clause 52.212-1, Instructions to Offerors--Commercial, applies to this acquisition. (ix) FAR clause 52.212-2, Evaluation--Commercial Items, is not applicable. (x) Offerors are advised to include a completed copy of the provision at FAR clause 52.212-3, Offeror Representations and Certifications-Commercial Items, with the quote. Offerors must also include a completed copy of the provision at DFARS 252.212-7000, Offeror Representation. (xi) FAR clause 52.212-4, Contract Terms and Conditions--Commercial Items, applies to this acquisition. (xii) FAR clause 52.212-5, Contract Terms and Conditions Required To Implement Statutes Or Executive Orders-- Commercial Items, applies to this acquisition.
