Description
This is a combined synopsis/solicitation for commercial item prepared in accordance with the format in FAR 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested, as a written solicitation shall not be issued. This solicitation number 03R305088 is issued as a Request for Proposal (RFP). The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2001-14, dated March 2003. This RFP is for shuttle services for the Agency for Healthcare Research and Quality between 540 Gaither Road and Shady Grove Metro. Award will be made to the offeror whose proposal represents the best value to the Government. See the attached files which includes the SF 1449, Statement of Work, Performance Evaluation Plan, and Performance Requirements Summary. The provisions at FAR 52.212?1, ?Instructions to Offerors-Commercial Items?, and 52.212-2, ?Evaluation-Commercial Items? are hereby incorporated by reference. FAR 52.212-3, ?Offerors Representations and Certification-Commercial Items? is hereby incorporated by reference. The proposal shall include a completed copy of the provisions at FAR 52.212-3, ?Offerors Representations and Certifications-Commercial Items? and the Offeror?s Dunn and Bradstreet number with their proposal. Offerors wishing to respond to this solicitation shall provide this office with an Original, plus four copies of the Technical proposal and Cost proposal, along with the required representation and certification. Offerors not in possession of the full text may contact this office and a copy shall be provided, or they may be accessed electronically at the following address: http://www.arnet.gov/far/. FAR Clause 52.212-4, ?Contract Terms and Conditions-Commercial Items? and Clause 52.212-5, ?Commercial Items? are applicable to this requirement. All questions concerning this RFP must be submitted in writing to the Contract Specialist. PLEASE NOTE THAT ANY AMENDMENTS TO THIS SOLICITATION SHALL BE PROCESSED IN THE SAME MANNER AS THIS SYNOPSIS/SOLICTATION. ALL PROPOSALS MUST CLEARLY INDICATE THE SOLICIATION NUMBER 03R305088 and be sent to the Contract Specialist?s attention. NO FAXES OR E-MAILS WILL BE ACCEPTED. Standard Form 1449 - Soliciation Form for Commercial Items Statement of Work SHADY GROVE METRO/540 GAITHER ROAD ROUTE From the main building entrance, exit the Eisenberg Building parking lot, 540 Gaither Road via the northeast driveway and turn right onto Redland Boulevard. Travel east approximately .9 miles (Redland Boulevard will become Redland Road). Make a left and enter the Shady Grove METRO station entrance and stop at the designated arrival/departure location. Exit the METRO station drive/parking area and turn right onto Redland Road. Travel west on Redland Road approximately .9 miles (Redland Road will become Redland Boulevard) to the intersection of Redland Boulevard and Gaither Road. Turn left onto Gaither Road and travel south approximately .03 miles to the Eisenberg Building driveway entrance. Turn right into driveway and travel to the main building entrance and the point of termination. LIMITATION AND RESTRICTIONS The primary purpose of this service is to facilitate official business of the Agency by providing transportation for Agency personnel and visitors between the Eisenberg Building and the Shady Grove Metro stop. Only persons showing a picture ID issued by the Agency for Healthcare, Research and Quality (AHRQ) or another agency of the U. S. Department of Health and Human Services, or those visitors who verbally indicate that they are going to AHRQ on official business may ride the shuttle. Intermediate stops between 540 Gaither Road and the Shady Grove METRO station shall not be made. The Shady Grove METRO station and the 540 Gaither Road building shall be the pick-up and departure points. OTHER CONTRACT REQUIREMENTS Contractor shuttle service employees shall be licensed and have a good driving record. They must pass a DOT physical and drug and alcohol tests. Suitability will be determined, in part, on the results of a criminal history and background check to be undertaken by the service provider. The Contractor shall keep the shuttle bus clean and in good working order. The Contractor shall inspect the shuttle bus daily and return promptly (i.e., within one day) all items left on the shuttle bus to the Government Project Officer. TERM OF AGREEMENT Firm term of 12 months, with the Government having the unilateral right to exercise four consecutive 12-month renewal options. AGREEMENT/SERVICE START DATE 12 months from award of contract CONTRACT TYPE The contract will be a performance-based fixed price contract. The Contractor will receive a firm fixed amount per calendar month, based on satisfactory performance. Performance standards are listed in Attachment A- Performance Evaluation Plan. Should the Contractor not meet the performance standards listed, deductions from the fixed price amount will be taken, as shown in Attachment A. The Contractor will also be eligible for a bonus payment above the contract amount, based on outstanding performance, as indicated in Attachment A. OTHER The Government INSTRUCTIONS TO OFFERORS AND EVALUATION OF PROPOSALS Your attention is directed to the requirements for technical and business proposals information to be submitted in accordance with the following instructions. These instructions establish the acceptable minimum requirements for the content of proposals. The proposal shall be prepared and submitted in two volumes: Volume I ? Technical Proposal Volume II - Business Proposal. Each of these volumes must be separate and complete in itself so that the evaluation of one may be accomplished independently of evaluation of the other. The Government shall evaluate proposals in accordance with the evaluation criteria set forth in this document. It is essential that the Offeror explicitly address all evaluation criteria. The technical proposal shall not contain price information. You must provide pricing for the base year and each of the four option years. For each year show a breakdown by month. The number of days times the daily rate and a total shall be shown. The number of days per month shall be determined by calculating the actual workdays less Federal holidays. A total for each year shall be provided. at 540 Gaither Road will provide overnight and mid-day parking for the bus and for the driver?s auto at no charge. The Government will not be responsible for any claims arising from the Contractor?s use of this privilege. EVALUATION CRITERIA SHUTTLE BUS SERVICES The Government will award a contract resulting from this solicitation to the responsible offeror whose offer conforming to the solicitation will be most advantageous to the Government, price and other factors considered. The following factors will be used to evaluate offers: 1. Past Performance 50 The proposal will be evaluated on the offeror?s past performance in providing comparable services to Government and/or commercial organizations. Offerors should provide information on recent contracts for similar services, including the name of the client organization, brief description of services provided, dates of performance, contract number, contract value, and point of contact (name, position and telephone number). 2. Technical Capability and Management Plan 50 The proposal will be evaluated on the offeror?s capability to provide the services described in the SOW and management plan for ensuring the quality of the services provided. Offerors should briefly describe: (1) corporate experience in providing comparable services; (2) availability of equipment and personnel to provide the required services; and (3) plan for ensuring the availability, quality and timeliness of the services. Offerors should list the person responsible for managing the contract, including communications with AHRQ. COST: The proposal will be evaluated on the offeror?s total price to provide the services described in the Statement of Work. Offerors shall provide separate prices for the base year and each of the four option years. AWARD: Award may be made without discussion. If negotiations are deemed necessary, they will be conducted with those Offerors that are acceptable. The acquisition will be awarded as a fixed price, performance-based service contract.
