Description: Amendment#4: The purpose of this amendment is as follows: To incorporate revised Statement of Work dated 25 July 2003 which corrects mis-referenced sections on pages 2,3,5,7,8,9,11,12,14,16,19,26,27,41,42,57,and,74 and incorporates the equipment listings Attachments1-3 to the Statement of Work. This amendment also incorporates a revised Atch 4 to the SF1449, Manning Matrix requirements, this revision adds clarification on submission of employee certifications and licenses required at time of quotation. All other terms and conditions to remain the same. A copy of this amendment is available upon request.
