COMBINED SYNOPSIS SOLICITATION This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in the notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. This solicitation, F04626-03-Q-0025, is issued as a request for quotation. This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 01-13 and Defense DCN 04-30. The Federal Supply Class (FSC) is R499 The associated Standard Industrial Classification (SIC) is 8699. The North American Industry Classification System is 813999 and the associated business size standard is 500 employees. This request for quotation consists of the following item: Item 0001- Services as a Relocation Assistant. To be considered the applicant must submit a personnel resume and proposed price for the year 01 Oct 03 through 30 Sep 04. The applicant must read and comply with the Statement of Work given below. The following provisions and or clauses also apply to this acquisition. Offerors must comply with all instruction contained in FAR 52.212-1, Instructions to Offerors-Commercial. Quotes without this provision will not be considered for an award. FAR 52.212-4, Contract Terms and Conditions-Commercial Items will be incorporated into any resultant Purchase Order by reference. FAR 52.212-5, Contract Terms and Condition Required to Implement Statues or Executive Orders-Commercial Items include clauses (b)(8), (11), (12), (13), (14), (15), (16), (17), (22). Clauses may be accessed electronically in full text through http://farsite.hill.af.mil/VFFARA.HTM. Vendors must be actively registered with the Central Contractor Registration (CCR). The CCR website is http://www.ccr.gov. Contractors are asked to submit their TAX ID number with their proposal. Quotes may be transmitted by e-mail or FAX to (707) 424-2712 and are to be received at the 60th Contracting Squadron no later than 4:30 PST on 12 Aug 03. Statement of Work For Administrative Assistant for Relocation Assistance Office 1. SCOPE The purpose of this contract is to have an administrative assistant for the relocation assistance office in the Family Support Center (FSC). The primary emphasis of the position is to assist customers in need of loan locker items, preparing the Loan Item Receipt, insuring entry of the control numbers on required items and briefing customers on care of loaned items. In addition, the Administrative Assistant will assist customers in need of relocation information and assembling the appropriate handouts. 2. PERSONNEL RESUME The contractor shall submit to the Government an official resume. The resume shall not be more than two pages in length and must contain the following information: 2.1 Full name and home telephone number. 2.2 Chronological work experience that substantiates by involvement and duration the skills and services being proposed. 2.3 A dated and signed statement certifying the information in the resume is true and accurate. 3. RESPONSIBILITIES Contractor’s responsibilities between 1 October 2003 to 30 September 2004: 3.1 Provide 40 hours of service per week, excluding Federal Holidays for a maximum total of 2088 hours annually. 3.2 Assists customers with relocation problems and questions. 3.3 Assembles appropriate handouts. 3.4 Assists customers in need of loan locker items, preparing the Loan Item Receipt, ensuring entry of the control numbers on required items, and briefing on care of loaned items. 3.5 Operates the loan locker suspense system to insure timely return of loaned items. 3.6 Provides information on incoming Permanent Change of Station customers as needed. 3.7 Contacts loan locker delinquent customers to advise them that return of items are pass due. 3.8 Contact by phone all personnel signed up for FSC classes to confirm their attendance and pass on pertinent information. 3.9 Consolidate list of Transition Assistance Program participants with appropriate information to request medical records for review from David Grant Medical Center. 3.10 Make copies of pre-separation counseling checklist and make distribution. Complete VMET search for all personnel attending pre-separation briefings. 3.11 Complete all surveys using DOCUTECH as required. 3.12 Inventory, distribute and store supplies when necessary. 3.13 Cover resource area and assist customers with job, finance and relocation searches. Give and certify typing tests. 3.14 Make sure children’s play area is kept neat and clean. 3.15 Refill all printers, copiers and fax machines with paper. 3.16 Responsible for stocking brochure racks and lektriever. 3.17 Assist Air Force Aid Society Officer with labels and shredding as needed. 3.18 Special projects from Director or Superintendent as required. (Incomplete requirement) 3.19 Open mail and make distribution. 3.20 File weekly FSC related Tailwind articles. 3.21 Performs routine office duties as needed and assigned. 4. GOVERNMENT SUPPORT The Family Support Center will provide printing support for materials as required; be available to assist the contractor; provide working space; FSC calendar, pencils and pens; filing and storage space; and computer. 4.1 The points of contact for this contract will be the Family Support Center Director, Joan Bueto, or the Relocation Assistance Manager Dale Fogh, 351 Travis Avenue, Travis AFB, CA 94535-1928, 424-2486. Once the contract is awarded, the point of contact will be Dale Fogh. 5. PERSONNEL QUALIFICATIONS The contractor must be familiar with and responsive to military protocol, organization and structure, and chain of command. The contractor must also have a working knowledge of Microsoft Windows; Word, Excel, and Access; and be able to quickly become proficient on other software as available.
