Description
The Centers for Medicare and Medicaid Services intends to award on a sole source basis under Simplified Acquisition Procedures with the University of North Texas, Health Science Center at Fort Worth, School of Public Health, 3500 Camp Bowie Boulevard, Fort Worth, Texas 76107-2699 a contract to identify two appropriate urban and rural Hispanic Serving Institutions or community based organizations and other entities as program participants to carry out a community based diabetes self management education program for use in the Hispanic American community. Approximately 250 persons from each site will participate in the program. The program should target Hispanic American adults (males and females) 40 years and older diagnosed with diabetes or at a high risk of developing the disease. The University of North Texas Health Science Center is one of the nation's distinguished academic medical centers dedicated to the advancement of all three disciplines of medical science education, research and patient care. The UNTHSC School of Public Health was established in response to community requests for a local school of public health. With seed funding provided by the Sid W. Richardson Foundation, the Amon Carter Foundation, and the Tandy Foundation, UNTHSC was able to implement a Master of Public Health (MPH) program approved by the State of Texas with the first students matriculating in the Fall 1995. In 1999, the Texas Higher Education Coordinating Board granted approval for the School of Public Health and a doctoral program in public health (Dr.P.H.). The School of Public Health consists of five departments: Epidemiology, Biostatistics, Social and Behavioral Sciences, Environmental and Occupational Health, and Health Management and Policy. The Dean directs the School of Public Health, with the assistance of the Associate Dean for Administration and Community Health and the five Department Chairs. The faculty complement involved in teaching, research and service for 2001/2002 is 29.0 FTE. In addition, the School of Public Health has established the Institute for Public Health Research (IPHR) to address the health and health care needs of the local, state and global health community through applied research, communication, and professional and community training. It also provides specialized training, consulting, research and technical assistance to organizations and agencies that practice health care and public health. The goal of the IPHR is to develop public health-related solutions for communities through academic-community partnerships with health departments, community organizations, health care delivery organizations, other health-related organizations and academic units within universities. If there are no affirmative written responses received within (45) calendar days from the published date of this notice, a purchase order shall be awarded.
