- Description
- The GLAC (Great Lakes Acquisition Center) is soliciting quotations to provide all labor, materials, equipment, travel, parts and supervision for full maintenance for the following Dialysis Unit listed in the specifications. Contractor shall provide monthly preventative maintenance. See Equipment Description with the terms and conditions listed herein at the VAMC, 5000 West National Avenue, Milwaukee, WI 53295. This contract is effective for the period of October 01, 2003 through September 30, 2004, with four (4) one-year options. Contract type is firm-fixed price. This solicitation is 100% set-a-side small business and offerors are reminded to complete all sections of the solicitation, including the Offeror Representations and Certifications-Commercial Items. The NAICS code is 561990; size standard $6 Million applies. Electronic version and hard copy of solicitation will be available on or about July 22, 2003.
