- Description: The GLAC (Great Lakes Acquisition Center) is soliciting quotations to provide all labor, materials, equipment, travel, parts and supervision for full preventive maintenance and emergency repair for the Ohmeda Anesthesia Equipment, services are to be provided in accordance with all the requirements of the contract: Preventive maintenance will be performed quarterly, February, May, August and November. See Equipment Description and Location provided within the RFQ. Location of the equipment: VAMC, 5000 West National Avenue, Milwaukee, WI 53295. This contract is effective for the period of October 01, 2003 through September 30, 2004, with four (4) one-year options. Contract type is firm-fixed price. This solicitation is set-a-side 100% small business and offerors are reminded to complete all sections of the solicitation, including the Offeror Representations and Certifications-Commercial Items. The NAICS code is 811219; size standard $6 Million applies. Electronic version and hard copy of solicitation will be available on or about July 12, 2003.
