- NA THIS IS AN AMENDMENT TO DAHA11-03-T-0013, REFUSE COLLECTION SERVICES, POSTED 01 AUG 03. This amendment adds the Performance Work Statement (PWS) dated 30 Jul 03 in full text as follows: SOLID WASTE REFUSE/RECYCLED MATERIALS CONTAINER SERVICES PERFORMANCE WORK STATEMENT (DATED 30 JUL 03) 1. Service shall consist of providing thirteen (13) refuse dumpsters, one (1) construction dumpster and ten (10) paper dumpsters for the entire service period and emptying all refuse and paper from the dumpsters on a scheduled basis. Rental of the dumpst ers will be included in the monthly charge. 2. Refuse includes all material placed inside the refuse dumpsters. The Contractor will also be responsible for cleaning up any mess caused by emptying the dumpsters before leaving the Base. The dumpsters will be returned to their original position wit h the lids closed after dumping. The paper removed from the base must be taken for recycling. The Contractor is required to submit a certificate of recycling. 3. The Contractor shall comply with all laws, ordinances, statutes and regulations pertaining to the collection; transportation and disposal of refuse and shall obtain such permits, and licenses or other authorizations as may be required. The Contractor w ill submit a monthly report along with the invoice indicating the quantity by weight and cost of each type of solid waste (refuse and paper) generated and removed from the base. 4. The Contractor shall comply with the posted speed limit on base of 25 miles per hour and shall operate their equipment in a safe manner. Particular care is required around overhead power lines and when backing up. 5. The refuse dumpsters are required to have a minimum capacity of eight (8) cubic yards, constructed of sturdy steel without castors and have lids that close. The construction dumpster is required to have a minimum capacity of twenty (20) cubic yards, c onstructed of sturdy steel without castors. The paper dumpsters are required to have a minimum capacity of two (2) cubic yards, constructed of sturdy steel without castors and have closable lids. The dumpsters provided under this contract must present a n eat and clean appearance and are required to be maintained to keep the neat and clean appearance. Any rusted, bent or otherwise unsightly dumpsters will be removed from the base by the Contractor and replaced with another suitable dumpster. 6. The location of the dumpsters will be at the 182 Airlift Wing, 2416 S. Falcon Blvd. Peoria, IL 61607 and 169th Air Support Operations Center/Squadron, 2323 S. Airport Rd. Peoria, IL 61607. Maps for these locations will be issued upon award of contract . 7. The refuse dumpsters will be emptied every Thursday starting with 2 Oct 2003 and the recycle dunpsters will be emptied every other Wednesday starting with 1 Oct 2003. The hours of operation for collection are between 7:30AM and 4:00PM. The construction dumpster will be emptied as needed, but not to exceed six times per year. The COTR or Contracting Officer will notify the contractor when the construction dumpster needs dumped. The only personnel authorized to place calls are the designated Contracting Officer's Technical Representative (COTR) or Contracting Officer. 8. The refuse dumpsters will also be emptied one Saturday each month after 1:00 PM. The days for the Saturday service will depend on the schedule of Unit Training Assemblies, which will be issued, upon award of contract. 9. The Contractor will have the dumpsters in place within 3 working days after the notice to proceed is given. If contract is not renewed with the same Contractor, their containers shall be removed within 3 days after notification or as approved by the C ontracting Office. 10. The services to be performed by the Contractor during the period of this contract shall, at all times and places, be subject to inspection by the Contracting Officer or their designated representati ve(s). 11. The designated Contracting Officer's Technical Representative (COTR) will be responsible for technical assistance to the Contracting Officer in administration of this contract. The responsibility involves quality control, inspection and monthly certi fication of the Contractor's performance. 12. The Contracting Officer's Technical Representative is authorized to request correction of deficiencies. They are not authorized to alter the terms or conditions of the contract. Should the Contractor and the Contracting Officer's Technical Represent ative disagree, the matter will be referred promptly to the Contracting Officer for resolution.
