Description

(i) This is a combined synopsis/solicitation for commercial item prepared in accordance with the format in FAR 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. (ii) Solicitation # DHQLGL04MPD0098. This solicitation is a Request For Quote(RFQ). (iii) The RFQ document and incorporated provisions and clauses are those in effect through FAC 14 (6-23-03) and Defense Change Notice 20030620. (iv) This resultant contract will be 100% set aside for small business. In addition, UNICOM owns all proprietary rights to the software's source code and is the only known source for maintenance. NAICS 541519, other Computer Related Services. Size Standard is $21M average annual receipts over past three years. (v)(vi) CLIN 0001: One (1) site license Annual Maintenance for CICS-Windows (CPU-7Z5, 686 MIPS), CLIN 0002: Three (3) site license Annual Maintenance for Carts-TS ((CPU-GS2048E, 676 MIPS), (2064-1C2, 475 MIPS), (9672-R45, 209 MIPS)). (vii) Required Period of coverage for maintenance is 1 October 2003 through 30 September 2004. Delivery is to DISA SSO - Montgomery CDTM 401 East Moore Drive, Bldg. 857 Maxwell AFB Gunter Annex, AL, 36114-3001. (viii) FAR 52.212-1 Instruction to Offerors-Commercial Item, applies to this acquisition. (ix) FAR 52.212-2 does not apply to this sole source acquisition. (x) Quotes shall include a completed copy of the provisions at FAR 52.212-3 Offeror Representation and Certifications-Commercial Item; DFARS 252.212-7000(b), Offeror Representation and Certifications-Commercial Item.
