Description
This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation. Quotations are being requested and a written solicitation will not be issued. Solicitation number CS-03-PR-00964 is issued as a Request for Quotation (RFQ). The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2001-12. This acquisition has not been designated as a small business set-aside. The North American Industry Classification System (NAICS) Code is 611430. The contract will be awarded using simplified acquisition procedures in accordance with FAR Part 13.5. The contract will be awarded as a firm fixed price contract. Award will be a single vendor award. Vendors shall obtain the spreadsheet for pricing from the Contracting officer at the address identified prior to responding to this FFP RFQ. Statement of Work: The Bureau of Customs and Border Protection (BCBP) is seeking a contractor to provide CPR, First Aid and First Responder training at 12 (twelve) sites identified in the National Training Plan. It is our intent to provide training at the sites indicated in the established plan, however, due to reorganization, military deployments, and other considerations alternate sites may have to be substituted. These alternative sites if needed will be New York City, Los Angeles and Miami. 1. Los Angeles Area 2. Seattle Area 3. Buffalo Area 4. New Orleans Area 5. Miami Area 6. Houston Area 7. Detroit Area 8. JFK International Airport, Chicago, Dallas, Portland, Memphis, Anchorage 9. Tucson 10. Laredo 11. Puerto Rico 12. Washington, D.C. Services are to be provided simultaneously at all site locations between September 20, 2003 and September 19, 2004. Centralized invoicing will be done through BCBP. Training materials to be used in conjunction with these courses ? such as video tapes, books, and handouts ? shall be reviewed for quality by Bert Knitter, Director, Mandatory Fitness Program, Customs and Border Protection, 1300 Pennsylvania Avenue, N.W., 2.2 C, Washington, DC 20229, prior to providing training Books and hand out materials are expected to have professionally illustrated diagrams that can be used by students to learn and review the various rescue techniques that compose these training programs. Photocopies are expected to be clean (no 10th generation versions that can not be read). Materials will also be expected to be up to date, depicting the latest approved rescue techniques. CPR training shall be in accordance with American Heart Association (AHA) American Red Cross (ARC), National Safety Council (NSC) guidelines and OSHA Standard 29 CFP 1910.151,1910.1030, 1910.51, and 19101.120. This training shall include re-certification of employees whose certification has expired. First Aid and First Responder Training shall be in accordance with American Red Cross (ARC) or National Safety Council (NSC) guidelines or American Safety and Health Institute (ASHI) guidelines. Upon successful completion of the course students will be issued a card indicating that they have met the standards of the course. Certifications should be valid for a minimal period of 2 years for CPR and First Aid and 1 year for First Responder. Contractor's Responsibility:  The contractor will be responsible for providing certified instructors, making all logistical arrangements for scheduling them and getting them to the teaching site, as well as providing all books, certificates, and materials related to the training.  Instructors will record class attendance and test scores indicating that the students either successfully completed the course or failed to meet course requirements. These performance records will be sent to Bert Knitter at 1300 Pennsylvania Avenue, NW, Washington, D.C. 20229 within 30 days of course completion. Test records will be maintained by the contractor for 5 years following the date of course completion.  The contractor will also provide the BCBP Local Training Coordinators with a description of each course to be taught and an outline of activities to be accomplished with a time line.  The contractor's representative (s) will work directly with BCBP Local Training Coordinators to schedule class time and dates as well as all related logistical matters necessary to get the instructor to the site.  The contractor will record the names of all individuals trained by class time, date and location. This information will accompany all invoices for said services.  The vendor will be responding in action and in writing to problems and complaints relating to training as referred by BCBP. Local Training Coordinators Responsibility:  The BCBP Local Training Coordinators will develop training schedules to meet the needs of the workforce, arrange for classroom space and AV equipment needed for training.  Publicize classes and register participants. Further they will assure that the instructor is escorted to the classroom. Within 24 hours of the completion of training the BCBP Local Training Coordinator will e-mail Bert Knitter, Director, Mandatory Fitness Program, with the names and numbers trained, by course and location.  Report any problems encountered with the training. INSTRUCTIONS FOR FILLING OUT THE SPREAD SHEET OBTAINED FROM THE CONTRACTING OFFICER The spread sheet indicates in Column D, the number of employees by location that need to receive training in one or more of the following courses: First Aid, CPR and/or First Responder. Agency need was determined through an employee survey. 1 Column B indicates the specific location where the training will be performed (e.g. International Falls, Montana). Column A. EXAMPLE: General Area: Seattle, Training Site: International Falls, Montana, An X in the course Columns E, F, and G indicates the training desired at each location. Total cost per each student requested in Column H is the sum of all training indicated by the Xed columns. For example if there are 200 students requiring training in both CPR and First Aid, and the fee for both courses is $50.00 per student, the cost indicated in Column H would be $100.00 . Column I indicates any associated costs - i.e. books, supplies, travel, accommodation incurred to perform this training. Note: Classes that are taught within a 50-mile radius of a major metropolitan area shall not include any additional price for travel. The offer shall include a fixed price for travel in this column for all travel for service and training outside of a 50-mile radius from major metropolitan areas. This travel price shall in accordance with the GSA reimbursement policy and in keeping with per diem rates for the area. Column J indicates the cost for the number total number of employees being trained at each site. Using the example sited in 5. Above, this would be $100.00 / student times 200 employees or $20,000.00. . Offerors shall include a completed copy of the provision of FAR 52.212-3, Offeror Representations and Certifications ?
