- The Sierra Pacific Network Consolidated Activity (CCA) is issuing a Commercial Item Request for Quotation (RFQ) in accordance with FAR 12.203 for bread and bakery products for the following Department of Veterans Affairs facilities within the Veterans Integrated Service Network (VISN 21): VA Medical Center, San Francisco CA; VA Palo Alto Health Care System CA, with divisions at Menlo Park and Livermore, CA, and VA Sierra Nevada Health Care System, Reno, NV. Contract period is October 1, 2003 through September 30, 2004 with one (1) option year. Contract award is subject to availability of funds. Offerors shall bid on all locations. The following provisions and clauses apply in addition to any addenda: FAR 52.212-1 Instructions to Offerors- Commercial Items; 52.212-2 Evaluation - Commercial Items; 52.212-3 Offeror Representations and Certifications - Commercial Items; 52.212-4 Contract Terms and Conditions - Commercial Items; and 52.212-5 Contract Terms and Conditions to Implement Statutes or Executive Orders - Commercial Items. NAICS code is 311812. FSC is 8920. Electronic solicitation may be obtained at no cost from the following URL: http://www.bos.oamm.va.gov/solicitation?number=261-0205-03 on or about August 11, 2003. HARD COPY SOLICITATION WILL NOT BE PROVIDED. No telephone requests will be accepted. No e-mail requests will be accepted. No web-based offers will be accepted.
