NA This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are b eing requested and a written solicitation will not be issued. Period of lease or rental is from September 1, 2003 to August 30, 2004. The requirement is for human body mannequin to train DOD physicians in the Advanced Life Support Course (ATLS) program . CLIN 0001 Trauma Man System. The TraumaMan system consist of simulated human tissue structure made of an elastomeric composition designed specifically for surgical dissection; System includes at least four areas for practicing surgical skills: abdomen , chest, neck, and ankle base. mfg: Simulab corporation; quantity: to train 864 students annually; CLIN 0002: IV Ankle Cut Down, mfg: Simulab Corporation, quantity: to train 864 students annually; CLIN 0003: Trauma Man System, Just in time training, mfg : Simulab corporation; quantity: to train 128 students annually; CLIN 0004: IV Ankle Cut Down, Just in time training, Mfg: Simulab Corporation, quantity: to train 128 students annually; CLIN 0005: War Skills Lab, mfg: Simulab corporation, quantity: t o train 480 students annually; CLIN 0006: Shipping cost for 2 each Trauaman Systems, from Seattle to Korea and return to Seattle; from Seattle to Japan and return to Seattle, total of 4 trips per year). This is a 100% total small business set aside. The recommended North American Industry Classification System Code (NAICS) for this procurement is 532490 with a corresponding size standard of $6,000.000.
