PREQUOTE SITE VISIT RSVP w/ LT Potts (520-228-5476) or email Michael.Potts@dm.af.mil or call LT Barajas (520-228-3598) or email Gayle.barajas@dm.af.mil Site Visit Info 0900 local time 19 August 2003 Meet at the SWAN gate Visitor Center Bring Driver?s License, Vehicle Registration, and Proof of Insurance. --------------------------- STATEMENT OF WORK FOR PUMPING AND CLEANING GREASE TRAPS 1 JUL 2003 1.0 DESCRIPTION OF WORK 1.1. Basic Services: Contractor shall furnish all labor, material, equipment and transportation necessary to pump and clean all grease traps per the statement of work. 1.2. Contractor shall remove all grease and residues from trap, pit walls, and all associated piping/equipment. Contractor shall ensure unrestricted flow and proper operation of grease trap. 1.3. Contractor shall transport and dispose of all waste materials to an off base approved site in accordance with all applicable local laws, ordinances, and regulations to include (but not limited to) the Solid Waste Disposal act, as amended, 42 USC Sec 3251 et seq., ACRR R9-8, Article 4 (Refuse and Other Objectionable Wastes); and Pima County Ordinance 1980-95, Chapter 8 (Liquid Wastes). 1.4. Contractors shall perform all service on the last Thursday of each month. Dining Hall (4100) and Officer?s Club (2050) service shall be performed between 0800 and 1100. If the above two facilities are holding a function between these hours, the contractor will be notified at least 24 hours prior and this work will be re-scheduled with the Quality Assurance Personnel (QAP) at no additional cost to the government. 1.5. In an unforeseen event such as: Increased threat condition, National Emergencies, Etc, that may prevent the contractor from doing his work, the contractor shall be notified, and at the government?s (GOV) discretion, re-schedule work. This change shall not incur additional cost to the GOV. 1.6. If additional grease trap servicing is necessary due to a contingency, it will be at the respective monthly price and at no additional cost to the government. This additional payment may be paid via the Government Purchase Card or additional funds to the contract.
