- IN LIEU OF REQUIRING A NON-REFUNDABLE FEE, THIS REQUEST FOR PROPOSAL (RFP) SOLICITATION NUMBER 541-R-109-03, ENTITLED MONITORED BED RENOVATION WILL BE AVAILABLE FOR PURCHASE (SEE BELOW) by each prospective (8a) and other interested concerns on or about August 21, 2003. A GEOGRAPHICAL RESTRICTION IS APPLICABLE TO THIS ACQUISITION. The RFP is being offered for Competition limited to eligible 8(a) Concerns under the cognizance of the Cleveland, Ohio District Office of the Small Business Administration, and pursuant to Federal Acquisition Regulation (FAR) 19.8 and the Small Business Act, as amended, 15 U.S.C. 637(a), and 13 CFR 124.501(a), which permits set-asides under the Small Business Administration_s 8(a) Developmental Program. Competition is limited solely to those 8(a) firms under the local jurisdiction and cognizance of the U.S. Small Business Administration of the Cleveland District Office, Cleveland, Ohio and capable of performing work as illustrated under the North American Industry Classification System (NAICS) Code of 236220, such as General Hospital Construction Services and the Size Standard in Millions of Dollars is $28.5. The Contractor shall completely prepare the site for building operations, including demolition and removal of existing structures, and furnish labor and materials and perform work for the Louis Stokes Cleveland (LSC) Department of Veterans Affairs Medical Center (DVAMC) MONITORED BED RENOVATION_as required by the drawings and specifications. Statement of Bid Item(s)_Item No. 1--Work includes general construction, alterations, mechanical and electrical work, utility systems, necessary removal of existing structures and construction and certain other items as required by specifications and drawings. The LSC DVAMC Project No. is 541-03-1008, and the Project is entitled MONITORED BED RENOVATION (W). The Place of Performance is the LSC DVAMC Wade Park Facility located at 10701 East Boulevard, Cleveland, Ohio 44106. The estimated construction cost is between $2,000,000 and $5,000,000. The requirement for the submission of both Performance and Payment Bonds, in an amount equivalent to 100% of the (subsequent) award amount is applicable and the successful awardee shall have ten _(10) calendar days after notification of contract award to submit both respective bonds to the Contracting Officer. Notwithstanding, an offer guarantee 1 is required for 20% of Negotiated Proposal but shall not exceed $3million and shall be submitted within ten _(10) calendar days after notification of award. The successful 8(a) firm (awardee) must have the capability to acquire both bonds and the offer guarantee. The date and hour for Receipt of Proposals is on or about September 18, 2003 and 3:30PM (Local Standard Time). Upon issuance of the solicitation, the exact date/time and location for receipt of offers shall be depicted on page 1 of Standard Form (SF) 1442. Following the evaluation phase by the agency (LSC DVAMC), that is, an assessment of the responsive proposal(s) received and each respective offeror_s ability to perform the prospective contract successfully, and which results in the selection of the proposal representing the best value to the Government and in accordance with (IAW) FAR Part 19, Subpart 19.805-2(b)(2), the SBA Cleveland, Ohio District Office will determine eligibility when the successful (8a) offeror has [first] been established by the Contracting Officer of the LSC DVAMC (agency). Prospective 8(a) concerns under the cognizance of the SBA Cleveland, Ohio District Office are strongly encouraged to inspect the work site on the date so stipulated (next sentence). The date/hour and meeting location for the _ONE-TIME_ SCHEDULED INSPECTION OF THE WORK SITE VISIT IS TUESDAY, AUGUST 26, 2003; HOUR: 9:15AM, AND THE MEETING LOCATION IS DIRECTLY IN FRONT OF THE LSC DVAMC PARKING GARAGE. Each prospective (8a) concern and other interested concerns (relative to subcontracting opportunities) is/are responsible for the expense of acquiring the solicitation documents. The Solicitation, includes three _(3) Attachments--(1) Davis Bacon Prevailing Wage Rates; (2) Construction Drawings and (3) Specifications. The solicitation and attachments will not be issued by this office. EACH PROSPECTIVE (8a) CONCERNS, AT YOUR EXPENSE, MAY purchase the solicitation documents (at actual cost of duplication) from Lakeside Blueprint, 700 W. St. Clair, Suite B-25, Cleveland, Ohio 44113, Telephone No. 216-522-0911. When calling the print company to inquire, you must cite the VA Solicitation 541-R-109-03 and Project No. 541-03-1008. Other interested parties, such as _other_ small business concerns, builders exchange, etc., may also purchase the solicitation documents from Lakeside Blueprint. NO COPIES OF THE SOLICITATION WILL BE PROVIDED BY THE LSC DVAMC. ELECTRONIC OFFERS WILL NOT BE ACCEPTED.
