Description
This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. This solicitation is issued as Request for Quotation (RFQ) No. F49642-03-T-0156. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 2001-15. This is a small business set aside. The North American Industry Classification System code is 238330 (formerly SIC code 1752) at $12 million size standard. LINE ITEM: 0001 Repair and replace floor system for Bldg 15, Bolling Fitness Center IAW the Statement of Work, QTY: 1/LOT, UNIT PRICE: $____________, EXTENDED PRICE: $____________. INSPECTON AND ACCEPTANCE TERMS: Supplies/services will be inspected by Quality Assurance personnel and accepted at job site. DESCRIPTION/SOW: REPAIR AND REPLACE ACRYLIC FLOOR SYSTEMS BLDG. 15 BASE FITNESS CENTER BOLLING AFB. 1. DESCRIPTION OF SERVICES. The contractor shall provide all management, tools, supplies, equipment, and labor necessary to Repair and Replace Acrylic Floor Systems throughout, Building 15 Base Fitness Center, located at Bolling Air Force Base in a manner that will maintain a neat and professional appearance. The Contractor shall remove existing floor to provide a sound substrate for installation. If substrate beneath floor needs repair, it will be performed on a time and material basis. The contractor is to prepare substrate and install flooring that matches the existing flooring as well as by the entire manufacturer suggested mechanical methods. The services shall include complete cleanup of the work area, leaving no debris or equipment. The contractor shall perform the following services in accordance with the Occupational Safety and Health Administration (OSHA) and the American National Standards Institute (ANSI) safety guidelines. 1.1. The work description shall serve as general information only and shall not be construed to limit the Contractor’s responsibility or obligation to comply with the contract documents to perform “Repair and Replace Innovative Floor Systems Bldg. 15 Base Fitness Center” Bolling AFB, DC. 2. EMERGENCY RESPONSE. The Contractor shall respond to emergency situations or calls within two hours. 3. GOVERNMENT FURNISHED PROPERTY AND SERVICES. The government will furnish to the contractor, all water, electricity and sewage in the performance of this contract. 4. GENERAL INFORMATION. 4.1. HOURS OF OPERATION. Normal Hours of operations are from 7 AM to 4 PM, Monday through Friday The Contracting Officer must grant prior written approval outside of these hours in writing. 4.2 RECOGNIZED HOLIDAYS: The following is a list of recognized Federal holidays: New Year’s Day, Martin Luther King Jr.’s Birthday, Inauguration Day, President’s Day, Memorial Day, Independence Day, Labor Day, Columbus Day, Veterans Day, Thanksgiving Day, Christmas Day. 4.3. QUALITY ASSURANCE. Unless otherwise noted in this statement of work, the contractor shall strictly comply with manufacturer’s specifications and industry standards. 4.3.1 A certified well-trained and licensed to install Innovative flooring systems. The contractor shall be thoroughly experienced and upon request be able to provide evidence of having successfully maintained and repaired systems of equal or greater size. 4.3.2 All work shall be of the highly quality and in strict accordance with manufacturer’s specification and to the government’s satisfaction. 4.3.3 The contractor shall be responsible for all means and methods as they relate to safety and shall comply with all applicable local, state, and federal requirements that are safety related. 4.4. GOVERNMENT REMEDIES. The contracting officer shall follow the requirements of FAR 52.212-4, Contract Terms, and Conditions for Commercial Items (May 1997), for contractor’s failure to correct nonconforming services. 4.5. SECURITY REQUIREMENTS: The contractor shall ensure each employee obtains the following pass and identification items, as applicable, for contractor personnel and non- government owned vehicles. AF Form 75 shall be submitted to the 11th SP/SPAP, by the (QA) person so that he can obtain passes for the contractor employees after obtaining names and verification of employment from the contractor. Request for passes must be forward on company letterhead. Full Name, Social Security Number, Picture ID. Beginning and ending periods requiring passes. 4.6 PHYSICAL SECURITY. The contractor shall be responsible for safeguarding all government property provided for contractor’s use. All government facilities, equipment, and materials shall be secured. The contractor shall be responsible for placing all barricades and safety devices during any activities and comply with all Occupational Safety and Health Standards (OHSA). 4.7 CONTRACTOR FURNISHED VEHICLES: 4.7.1 VEHICLES: Not Used 4.7.2 TRAFFIC LAWS: The contractor and its employees shall comply with base traffic regulations. 4.8. PERFORMANCE OF SERVICES DURING CRISIS DECLARED BY THE NATIONAL COMMAND AUTHORITY OR OVERSEAS COMBATANT COMMANDER. The contractor shall perform all services under this contract during a national crisis according to the Department of Defense Instruction (DODI) 3020.37. 4.9. CONTRACTOR – FURNISHED ITEMS AND SERVICES. Except for those items and services specifically stated in Government Furnished Property and services, paragraph 3, the contractor shall furnish everything needed to perform this contract according to all its terms. 4.10 CONTRACTOR PERSONNEL: 4.10.1 PERSONNEL MANAGER: The contractor shall provide a project manager or person who shall be responsible for the competent performance of the work, and shall have full authority to act for the contractor. The personnel manager shall also be able to read, write, speak, and understand English. The contractor shall provide a manager who shall be responsible for the performance of the work. The name of this person and an alternate(s), who shall act for the contractor when the manager is absent, shall be designated in writing to the contracting officer prior to the contract start date and updated as necessary. Manager or alternate shall have full authority to act for the contractor on all contract matters relating to daily operation of this contract. The personnel manager, alternate(s) must be able to read, write, speak, and understand English. 4.10.2 CONTRACTOR PERSONNEL: Contact personnel shall present a neat appearance and be easily recognized while on base. The contractor shall not employ persons for work on this installation is such employee is identified to the contracting officer as a potential threat to the health, safety, security, general well being, or operational mission of the installation and its population. Contractor personnel shall preset a neat appearance and be easily recognized as contractor employees. Identification shall be worn or attached to outer garment at all times. The contractor shall provide the contractor officer with a list of personnel working under this statement of work (SOW) before contract start date. 4.10.3 The management and control of contractor employees in the performance of this contract shall be the responsibility of the contractor; however, the contractor shall comply with the contract requirements and Bolling Air Force Base regulations concerning conduct of personnel. 4.11 WARRANTIES. 4.11.1 The contractor shall be responsible for replacing any warranty item within the appropriate period to of such warranty. This shall include manufacturer warranty. 4.12. ENVIRONMENTAL CONTROLS: Compliance with Laws and Regulations. The contractor shall be knowledgeable of and shall comply with all applicable federal, state, and local laws, permits, DOD, Air Force, and base environmental requirements and instructions. The contractor shall ensure that all subcontractors comply with the same. The contractor shall ensure policies and procedures are established that protect the safety and health of employees and the community to minimize or eliminate the risk of environmental pollution. If the contractor spills or releases any substance listed in 40 CFR 302 into the environment, the contractor shall immediately report the incident to the Environmental Coordinator at 202-767-8600, in accordance procedures. The contractor shall notify the primary QA personnel or in his absence the alternate QA personnel. The contractor shall be liable for containment and environmental clean up of the spill or release of such substance. If spillage creates contaminated soil, the contractor shall at his own expense, remove the contaminated soil, properly dispose of the soil and replace it with uncontaminated topsoil and restore the grounds to their original condition. The contractor shall not dispose of contaminated soil or any hazardous materials on Bolling Air Force Base. FAR PROVISIONS: The provisions at 52.212-1, Instructions to Offerors-Commercial Items and 52.212-3, Offerors Representation and Certifications-Commercial Items are hereby incorporated. Addendum to FAR Clause 52.212-1 applies to this acquisition is as follow: 52.237-1, 52.252-1, 52.252-5: (b) DFARS (Chapter 2), 252.212-7000 and 252.225-7000. Add paragraph (k), Site Visit: “A site visit will be conducted on 27 Aug 03 9:30 A. M. EST. We will meet in front of building 15. Attendees must enter Bolling AFB through the south gate off of Interstate 295. Attendees must also arrive to the gate one hour prior to the start of the site visit to allow time to get through the gate. For security reasons, offeror must submit full name and SSN of each representative attending by e-mail or facsimile listed below, to Lt. Lynda Akerson no later than noon, 26 Aug 03.” Add paragraph (l), “Offeror must include, with its offer, a completed copy of provision FAR 52.212-3 in full text.” Add paragraph (m), “The point of contact for this project is 11th Civil Engineering Squadron QA, TSgt Craig Moffet at (202) 767-1090.” Clause 52.212-2, Evaluation—Commercial Items applies to this solicitation, specifically the following criteria: a.) The Government will award a contract resulting from this solicitation to the responsible offeror whose offer conforming to the solicitation will be most advantageous to the Government. The competing offerors’ past and present performance history will be on a basis significantly more important than cost or price considerations. The government seeks to award to the offeror who gives the greatest confidence, that will best meet or exceed the requirements affordably. In making this decision, the government is more concerned with obtaining the best value principles. Offerors shall be treated equally except for their prices and performance records. However, the Government will not make an award at a significantly higher overall cost to achieve slightly higher performance rating. The contractor shall provide the following past and present performance data: The recent information (letters, customer surveys, independent surveys, etc.) which demonstrate customer satisfaction with overall job performance and quality of completed product for same or similar type discipline in the past three years. At a minimum, the recent information shall contain: the contract number, point of contact, phone number for point of contact, place and period of performance, and amount of contract. FAR CLAUSES: The clause at 52.212-4, Contract Terms and Conditions-Commercial Items is hereby incorporated. Addendum to FAR Clause 52.212-4 applies to this acquisition, specifically the following cited clauses: 52.209-6, 52.237-2, 52.237-3, 52.242-15, 52.252-2, 52.252-6: (b) DFARS (Chapter 2), 52.253-1, 252.204-7003, 252.204-7004, 252.219-7011, 252.212-7001 (DEV), 252.225-7001, 252.225-7002, 252.225-7009, 252.243-7001, 252-247-7023 and 5352.242-9000; Payment will be made upon receipt of proper invoice, as required by FAR 52.212-4 (g), Invoice for all accepted services.
