- In accordance with FAR 12.603 (c) (2): This is a combined synopsis/solicitation for commercial items prepared IAW the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. This acquisition is a 100% competitive Small Business set-aside. Solicitation ?Rental of Hydro Ax? is issued as a Request for Proposal (RFP). This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 2001-15, SIC Code 1629 ?NAICS Code 234990 Heavy Equipment/Machinery?. Contract Line Item Number 0001: The United States Air Force Academy in Colorado Springs has a requirement for rental of a Hydro-Ax 721-E with rotary ax to reduce wild-land fuels and create fuel breaks in the vicinities of the north, south, and west boundaries on the Academy. One (1) Month = $_______________ DESCRIPTION OF SERVICES: Contractor will rent to the United States Air Force Academy a Hydro-Ax 721-E with rotary ax to reduce wild-land fuels and create fuel breaks in the vicinities of the north, south, and west boundaries of the Academy. The contractor will provide training to heavy equipment operator(s) from J&J Maintenance, who will perform the work, in safe operation of the equipment. Contractor will provide instruction/training in any and all maintenance required of the operators for the rental period of one month. This includes providing any required operators? manuals or other written material that may be needed for operator?s reference. PERIOD OF PERFORMANCE: Rental of the Hydro-Ax 721E with rotary axe will be for a period of one month (30 days) from the date the machine arrives on USAFA. LOCATION/PLACE OF CONTRACT PEFORMANCE/WORK: This equipment will be utilized on the north, south and west boundaries of the United States Air Force Academy, Colorado, 80840. Services shall be performed in accordance with the best commercial practices. Offerors should include their DUNS, CAGE (Commercial and Government Entity) Code and Tax ID Numbers, and be CCR (Central Contractor Registration) registered and EFT (Electric Funds) compliant. FAR 52.212-1, Instruction to Offerors?Commercial Items applies to this acquisition. FAR 52.212-2 Evaluation?Commercial Items applies to this acquisition and the following factors shall be used to evaluate offers: Price and past performance wherein price is considered approximately equal to past performance. FAR 52.212-3 Offerors Representations and Certifications?Commercials Items applies to this acquisition. FAR 52.212-4 Contract Terms and Conditions?Commercial Items applies to this acquisition with the following addenda: DFARs 252.204-7004, Required Central Contractor Registration. FAR 52.212-5 Contract Terms and Conditions Required to Implement Statutes or Executive Orders?Commercial Items applies to this acquisition. Offerors must acquaint themselves with the regulation concerning Commercial Item acquisition contained at FAR 12. Offerors not in possession of the reference clauses may obtain them at http://www.farsite.hill.af.mil. Offerors Representations and Certifications Commercial Items (FAR 52.212-3) must be submitted with proposal.
