Description
(i) This is combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotations are being requested, and a written solicitation will not be issued. (ii) The reference number is F73TJ632160200, and the solicitation is issued as a Request For Quote (RFQ). (iii) The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 2001-15. (iv) This is a total small business set-aside. The associated North American Industry Classification System (NAICS) code is 423410, and the small business size standard is 100. (v) The solicitation can be viewed at http://www.eps.gov/. (vi) The solicitation package includes the Statement of Work (SOW), List of Government-Furnished Equipment (GFE), and Request For Quotation (RFQ). (vii) This combined synopsis/solicitation is for the reconfiguration of thef audio-visual (AV) suite in the Tunner Conference Room and adjacent projector room in Bldg 1900 at Scott AFB IL. (viii) The provision at Federal Acquisition Regulation (FAR) 52.212-1, Instructions to Offerors?Commercial Items, applies to this acquisition. Addendum to 52.212-1: The RFQ shall consist of three (3) separate parts; Part I?Price Quote, Part II?Technical, and Part III-Past Performance Information. Part I consists of (a) Price Quote and (b) Completed copy of provision FAR 52.212-3, Offeror Representation and Certifications?Commercial Items. The provision may be copied from website http://farsite.hill.af.mil/VFFARA.HTM. Parts II and III are addressed in FAR Clause 52.212-2 as listed in the following subparagraph. (ix) The FAR provision 52-212-2, Evaluation? Commercial Items, applies to this acquisition. Paragraph (a) is completed as follows: (1) The CO establishes the following evaluation factors, listed in order of importance: (a) Technical Capability and Relevance of Work (Critically Important). As pertains to technical capability and relevance of work, an offeror must address all capabilities and characteristics addressed in the SOW, including providing a list of equipment (with descriptive "cut" sheets and equipment availability) to be used in conjunction with award requirements. An offeror must also address their approach on how to meet the required period of performance of 14 days after award. An offeror must also provide resumes documenting personnel expertise available to support this requirement. (b) Past Performance (Very Important). Current and past performance on relevant Government and private sector contracts shall be submitted by 3:30 CST, 4 Sep 03. Contractor shall provide on at least three but no more than five of the most relevant contracts. Relevant contracts include audio-visual reconfiguration, upgrade and maintenance contracts in other facilities. Past Performance Information is limited to three pages per contract listed and shall be submitted in original and three copies. Prospective contractors shall list any problems encountered on these contracts, including corrective actions. If there is no past performance criteria, a prospective contractor may not be evaluated favorably or unfavorably. (c) Price (Very Important). (d) Design Approach (Important). When submitting design approach, a prospective contractor must provide a general and preliminary layout of the equipment envisioned for the design. Utilization of space and mounting of equipment in the equipment room is desired. (2) Evaluation of technical information and design approach shall be conducted by subject matter experts. (3) Technical and past performance, when combined, are significantly more important than price or design. (x) The clause at FAR 52.212-4, Contract Terms and Conditions?Commercial Items, applies to this acquisition. (xi) The clause at FAR 52.212-5, Contract Terms and Conditions Required to Implement Statutes or Executive Orders?Commercial Items, applies to this acquisition. The clauses at subparagraph (b)(5), (b)(11), (b)(12), (b)(13), (b)(14), (b)(15), (b)(25), and (c)(1) of 52.212-5 are applicable. (xii) The clause at FAR 52.222-20, Walsh-Healey Public Contracts Act, applies to this acquisition. (xiii) The clause at DFARS 252.225-7001, Buy American Act and Balance of Payments Program, applies to this acquisition. (xiv) The clause at Defense Federal Acquisition Regulation Supplement (DFARs) 252.204-7004, Required Central Contractor Registration applies to this acquisition. (xv) The clause at FAR 52.237-1, Site Visit, applies to this acquisition. (xvi) The clause at AFFARS 5352.215-9001, Notice of Pre-Bid/Pre-Proposal Conference, applies to this acquisition. A pre-bid/pre-proposal conference will be conducted at Bldg 1900, Scott AFB IL, on 5 Sep 03 at 10:00 AM CST for the purpose of conducting a site survey. Submit the names of all attendees (not to exceed two names per company) in writing via facsimile transmission (618-256-5237) or via email to Denise.Hamilton@scott.af.mil prior to 3:30 PM CST on 3 Sep 03. Any questions which result from the pre-proposal conference (site visit) are due via facsimile transmission (618-256-5237) or email to Denise.Hamilton@scott.af.mil NLT 4:30 PM on 8 Sep 03. (xvii) General Notes. A proposal shall be submitted in one original and three copies and is limited to 8 pages (not including proposed prices and FAR 52.212-3 reps and certs. A page is defined as one face of a sheet of paper containing information. Typing shall not be less than 12 pitch. Proposals will be on 8-1/2"x11" paper except for fold-outs used for charts, tables or diagrams (if necessary) which may not exceed 11"x17". (xviii) All responsible sources may submit a quotation, which shall be considered by the Agency. (xvix) Contract award cannot be made to any contractor who is not enrolled in the Department of Defense (DoD) Central Contractor Registration (CCR) program.
