PART: US GOVERNMENT PROCUREMENTS SUBPART: SUPLIES, EQUIPMENT AND MATERIAL CLASS CODE: 65- MEDICAL, DENTAL, AND VETERINARY EQUIPMENT & SUPPLIES OFF ADD: DEPARTMENT OF VETERANS AFFAIRS, CONTRACTING OFFICER 90C VA MEDICAL CENTER, BLDG 41 2200 FORT ROOTS DRIVE NORTH LITTLE ROCK, AR 72114 SUBJECT: 65- PREVENTIVE & CORRECTIVE MAINTENANCE FOR EM MICROSCOPES SOL: 598-208-04 POC: DONNA LANDRUM 501-257-1032 DESC: Notice of intent to make a sole source award to Jeol USA, Inc., Peabody, MA, due to proprietary and diagnostic software installed on equipment. Jeol shall provide all necessary personnel, equipment, materials, supervision, and labor to maintain the following government owned equipment listed in this solicitation in operating order in accordance with all manufacturers specifications for the period of October 1, 2003 through September 30, 2004. Jeol Microscope Model JSMS1110, and Jeol Microscope Model JEM1010. Equipment is located at John L. McClellan Veterans Memorial Hospital, 4300 West 7th Street, Little Rock, Arkansas. These requirements and specifications are to include (on-site when possible) the complete servicing but not limited to the following to assure the equipment is maintained and performs in accordance with the manufacturers specifications: Labor to include troubleshooting, repairs, parts replacement for unscheduled, emergency and preventive maintenance. Replace all worn, damaged, or spent parts with original manufacturers replacement parts (new or warranted). Scheduled preventive maintenance to be performed in May and November of each contract period and shall include but not be limited to: technical inspection, cleaning, lubricating, adjusting, calibrating, and replacing all worn, damaged or expendable parts in accordance with manufacturers preventive guidelines, federal, state, and local regulations. Jeol must provided documented proof of qualifications, (ie: manufacturer’s training or prior experience of equipment covered by the contract). Jeol must be in possession of all technical service manuals, including schematics, calibrations procedures, etc. Jeol must provide at no additional charge, all software updates, upgrades and patches as offered by OEM. Jeol must be in possession of all specialized test equipment needed to perform all maintenance in accordance with appropriate service manuals.
