- Solicitation F29651-04-Q0030 is being issued as a Request for Quote (RFQ). The electronic solicitation and synopsis constitute the only announcement. Quotations are being requested and a written solicitation will not be issued. A Firm fixed price is contemplated. This procurement is being conducted as 100% small business set-aside. The applicable North American Industry Classification System (NAICS) code is 485210. The size standard is $5,000,000. Description: Service Non-Personal Request contract with four option years to furnish vehicle and all other items necessary to transport patients to and from William Beaumont Army Medical Center in El Paso, TX, and provide courier service between 49the Medical Group, Holloman AFB, downtown labs, and WBAMC for the period of 1 Oct 2003 through 30 Sep 2004. Courier service to WBAMC will be for 249 days and patient transportation as needed per attached Performance Work Statement (PWS) and schedule. Based on 249 courier runs per year and approx 450 passengers per year. Four Attachment is included: Solicitation, Standard form 1449 Solicitation/Contract/Order for Commercial Items, Performance Work Statement, Shuttle Schedule and Wage Determination. Any interested party must be registered in Central Contractor Registration (CCR) in order to be eligible for award (ref. DFARS 252.204-7004, Required Central Contractor Registration). No hard copy will be available. All responsible sources must submit the SF 1449 Request for Quote (RFQ), including the FAR Clause 52.212-3, ?Offeror Representations and Certifications---Commercial Items,? which shall be considered by the agency.
