Description

Naval Hospital Jacksonville Fl has a requirement for procurement of six-station audiometric test booth, double walled. Seating for six patients and mounting hardware for audiometers and calibration equipment is to be included. Both will be used for the following: to test patients in the hearing conservation program; to perform audiograms for patients receiving routine audiograms; to train audiometric technicians to perform testing and learn the DOEHRS system. Booth must have the capability of testing the hearing of six patients simultaneously with computer based audiometry and must be compatible with standard U.S. Navy systems. Booth must meet or exceed ANSI and Navy hearing conservation standards. Booth must be wired by the manufacturer to make it compatible with the DOEHRS system. The booth must be sound level meter tested to make sure that it passes ANSI and Navy standards for sound reduction. Maximum dimensions allowed: 96"H X 112"W X 114"D. Installation is required. Price will include shipping and handling charges (FOB: destination). Packing material must be removed from the premises by the vendor. Please note: Vendor must be registered and active in the Central Contractor Registration (CCR) Database before an award can be made. Please provide the following: Cage Code, DUNS #, TAX ID #.
