Description
This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information attached to this notice. The provisions and clauses incorporated herein are those in effect through the latest Federal Acquisition Circular 2001-15. This acquisition is being conducted in accordance with the Simplified Acquisition Procedures and 100% Small Business Set-Aside. This announcement constitutes the only solicitation; quotes are being requested at this time. A written solicitation will not be issued. The requirements, applicable terms and conditions, representations and certifications to be completed by the offeror, and evaluation factors are contained within this RFQ (SSA-RFQ-03-0740). The small business size standard is $6 million. The North America Industry Classification System (NAICS) code for this acquisition is 541430. STATEMENT OF WORK: BACKGROUND: The Social Security Administration has a requirement for forty (40) two-panel digital posters. The purpose of this effort is to obtain contractor services and materials to construct and produce 40 digitally imaged poster-panel exhibits each consisting of 2 sections with images on both sides, (total of 80 images) using ?Color Span Digital 6000? production. Each exhibit shall be suitable for use as a wall hanging or viewed as a stand alone counter top unit. A sample exhibit is available for inspection at SSA Headquarters, located in Baltimore, MD 21235. The Contractor must call the Contracting Officer ahead of time to setup an appointment. PERIOD OF PERFORMANCE: From date of award for approximately 120 days. MATERIALS PROVIDED BY SSA: All art work and a full-size comprehensive, showing the location of the images, titles, signatures and SSA logo to be digitally imaged on the front and the back of each unit. This comprehensive will show the exact color, flesh tones and ranges of color that will exist in the final art. The images to be used will be provided in Quark (MAC) in their finished form, all retouching and color correction will be accomplished by SSA. SSA will provide 40 address labels for contractor to distribute the panels to SSA field offices located throughout the U.S (provided at the time of award). SSA will provide all art work on disk in Quark (MAC), with directory and laser proof within 5 days after the award is made. SPECIFICATIONS: The contractor shall construct forty (40), 2-panel units consisting of 2 sections each (images on front and back) made of komatex white boards (thickness is 6 mm/millimeter) as follows: --each section shall be joined by a single hinge --the hinge measures 20? long and is made of a synthetic material called stretch-lite elastic. The thickness is 1/16? x 1.5? wide. The hinge is inserted into the komatex white board material and attached by 4 rivets--each unit measures 20? tall and 48? wide when displayed in an open position --each unit measures 20? tall and 24? wide when folded down for storage. The contractor shall provide full size color proofs on the exact material to be used for the final product. SSA will either accept or request alterations of the proof within 3 working days. If alterations are requested, a new proof shall be required within 5 working days. Once accepted, the contractor shall begin distribution. The contractor shall distribute 40 units using the labels provided by SSA. The images shall be accomplished through the digital imaging method using ?Color Span Digital 6000? imaging in full color with rigid laminate (5 mil thin film of plastic run thru a roller press with heat applied) and the images will be mounted on the komatex white boards. The contractor shall be responsible for the purchase of a black, nylon cordura cloth, portfolio carrying bag with sturdy wire frame(Item#START Soft-Side Portfolio by Prat/Europa SF-26). This portfolio bag shall have a smooth running nylon zipper. The portfolio bag shall be 26?x 20? with a reinforced bottom that expands to a 3? capacity. The portfolio bag shall have dual purpose handles that can convert to a shoulder strap positions. The inside of the bag shall have a full size inside pocket and zippered pencil case. The sample portfolio bag is available for inspection Call the Contracting Officer ahead of time to setup appointment. The contractor shall provide 2 rigid protective material/boards; one on each side of the poster to fit inside of the zipper bag. The contractor shall provide standard cartons and packaging for each unit to prevent any damage in shipping (SSA reserves the right to inspect a container to be used prior to shipping). The contractor shall pack each exhibit in bubble-wrap, then box in cartons, and ship to the 40 locations (complete addresses will be provided at the time of award) as designated on each address label. SSA shall require an on-site inspection of the digital imaging process and the contractor will be required to give SSA a 72-hour notice before the imaging begins. SSA reserves the right to waive the inspection. NOTE: The contractor shall be required to attend at a minimum 5 face-to-face meetings and inspections (at the expense of the contractor). All meetings will be held at the SSA Headquarters located in Baltimore, MD. DELIVERY SCHEDULE: contractor shall prepare and ship First Class approximately 5 exhibits per week for 8 weeks (at the Contractor?s expense). SECURITY CLEARANCES: Contractor personnel who require access to SSA facilities for meetings shall be required to undergo a background investigation, which will be conducted by the Government. Such investigation shall require that Contractor employees submit a statement of personal history. Contact Contracting Officer for copy of security requirements clause and forms. Forms will be provided at the time of awards. PRICING: Contractor shall propose Firm-fixed price per exhibit FOB Destination. TERMS AND CONDITIONS: (Note: full text of all clauses and provisions can be accessed via the Internet using the following website: http://www.arnet.gov). The provisions of FAR 52.212-4 Contract Terms and Conditions?Commercial Items (FEB 2002) and FAR 52.212-5 Contract Terms and Conditions Required to Implement Statues or Executive Orders-Commercial Items (JUN 2003) applies including the subparagraphs as follows: (b)(5), (b)(14),(b) (15),(b) (16),(b) (17),(b) (18),(b) (19),(b) (24), and (b) (30). INSTRUCTIONS TO OFFERORS: The provisions at FAR 52.212-1, Instructions to Offerors --Commercial Items (JUL 2003) are incorporated by reference. Offers shall follow the instructions set forth therein. Special attention to the (b) (10) all information required by this paragraph shall be submitted. Past performance information shall be limited to three current projects and/or project completed within the past 3 years that are equal in size and complexity, the Government reserves the right to make only 2 attempts to contact any given reference (an initial attempt and one follow-up); ( b) 11 does not apply; (c) period of acceptance of offers 30 days; (d) product sample applies; (e) does not apply. Certifications as required by 52.212-3 Offerors Representations and Certifications?Commercial Item (JUN 2003) shall be completed and submitted with offer. This form can be downloaded at www.arnet.gov/far/loadmain52.html. Offerors shall submit the following: three (3) copies of quotation which addresses the specifications; three (3) samples of work that a similar to Government's requirement; past performance information; representations and certifications as specified herein. Offerors that fail to include all the required information may be rejected without further consideration. Award will be made without discussions; offerors should provide their best pricing with their offers. If an offer contains terms or conditions unacceptable to the Government, the Government may eliminate it from consideration without notice to the offeror. EVALUATION: FAR 52.212-2 Evaluation-Commercial Items (JAN 1999) has been tailored pursuant to FAR 12.301(c) by the substitution of a similar provision in the following addendum:(a) The Government will award a firm-fixed price purchase order resulting from this solicitation to the responsible offeror whose offer conforming to the solicitation will be most advantageous to the Government, and represents the best overall value, price and other factors considered. The Government will determine the value of an offer based on its acceptability and how well the offeror complies with the solicitation instructions and requirements. Award will be on a best value basis taking into account the following factors: samples, past performance and price. All factors being equally important. The Government may award to other than the lowest price offered based on the tradeoff among these factors. All responses to this combined synopsis/solicitation are due no later than 5:00 p.m., Thursday, September 11, 2003.
