- Description
- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. Solicitation 552-03-4-121-0115 is being issued as a Request for Quotation. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 2001-15. NAICS Code 339112, 500 employees small business size standard applies. Items will be purchased and delivered to the VA Medical Center, Bldg. 126 Loading Dock, 4100 W. Third St., Dayton, Ohio 45428. Prices shall be submitted for the following: 1) Photocoagulation apparatus, surgical – Novus 2000 Laser console w/dual fiber delivery system port to include 2.5 watts blue/green and 1 watt green only wavelengths, dual fiber ports, power on demand, auto repeat mode, closed loop cooling system, caster wheels, single phase power circuitry, one pair argon safety spectacles & key plus operator’s manual, installation and in-service and service manual containing parts lists, schematics, and wiring diagrams. 2) 1 ea Novus 2000 Laserlink HS Slit Lamp adapter; 3) 1 ea. CSO Slit lamp and table; 4) 1 ea. Opthalmoscope, laser indirect Keeler Model 420mm objective lens, 16.5in working distance protective storage case, and operator’s manual; 5) Warranty: on Novus 2000 – one full warranty from date of shipment including parts, service, and labor at customer’s site. System shall include five-year guarantee on the argon laser tube. Warranty shall be exclusive of labor or additional parts from date of shipment; 6) 1 ea. Photocoagulation apparatus, surgical Lumenis Model Selecta Duet Nd: YAG/SLT Combination Laser System with standard table to include remote control, pair of safety glasses, operator’s manual, and laser safety sign; 7) 1 ea. Lens, gonio, laser Lumenis Model Selecta Latina SLT; 8) Warranty: for Selecta Duet YAG/SLT – includes one year all inclusive coverage (parts, labor, and service engineer travel) from date of shipment; 9) 1 ea. Biomedical Technician Training -Tuition Only - for the YAG/SLT; 10) Trade in of old Coherent Model Novus 7921N, S/N 731057 in the form of a credit; 11) Operator manual – one for each unit; 12) 1 ea. Service manual containing parts lists, schematics, and wiring diagrams for Novus 2000 dual fiber console; 13) 1 ea. Service manual containing parts lists, schematics, and wiring diagrams for Selectra Duet Nd:YAG/SLT; 14) 1 ea. Biomedical Technician Training-Tuition Only - for the Novus 2000. The following provisions and clauses apply to this solicitation: 52.212-1 Instructions to Offeror-Commercial Items, 52.212-4 Contract Terms and Conditions-Commercial Items, 52.212-5 Contract Terms and Conditions Required to Implement Statues or Executive Order Commercial Items with (b) (1),(14),(15),(16),(17),(18),(19),(21),(24), and (30) checked. FAR 52.233-2 Service of Protest, VAAR 852.233-70 Protest Content, VAAR 852.233-71 Alternative Protest Procedure and VAAR 852.270-4 Commerical Advertising also apply to this solicitation. Quotes will be evaluated on the following criteria: technical capability of equipment, price and service agreement offered. Award will be made to the offeror whose product meets the requirements listed above and is most advantageous to the Government. The Government intends to evaluate the information received and award without discussions. Offers including the pricing information are to be submitted on company letterhead and include the information required in provisions 52.212-1, a completed copy of the provisions 52.212-3 Offeror Representations and Certifications-Commerical Items, literature/brochures on proposed equipment and the company DUNS number. Electronic copies of the referenced provisions and clauses are available on the internet at www.arnet.gov/far and www.va.gov/oa&mm/vaar/. Quotations and all required information must be received by 4:30 PM September 19, 2003. Quotations shall be submitted to Jodi A. Cokl, Contracting Officer, VA Medical Center, 4100 W. Third Street, Dayton, Ohio 45428. Faxed quotations are acceptable and may be faxed to (937) 262-5974. Solicitation number 552-35-03 must be listed on all documents for identification purposes. Only offers prepared in accordance with this notice will be considered.
