- This is a combined synopsis/solicitation. The GLAC (Great Lakes Acquisition Center) intends to negotiate a firm fixed price contract to provide REGISTERED NURSE SERVICES for the VA North Chicago, 3001 Green Bay Road, North Chicago, Illinois 60064. Request for Quotation #RFQ 69D-361-03 entitled "Registered Nurse Services" is hereby issued for commercial items as full and open competition and is prepared in accordance with the format in Federal Acquisition Regulation (FAR) Part 12, Acquisition of Commercial Items, Subpart 12.6, Streamlined Procedures for Evaluation and Solicitation for Commercial Items, Part 13, Simplified Acquisition Procedures, and as supplemented with additional information included in this notice. This announcement incorporates provisions and clauses in effect through FAR FAC 2001-15 and VAAR 97-10. NAICS Code 621399 applies for this procurement and the small business size standard is $6.0 million. This announcement/solicitation constitutes the only bid document that will be issued. A separate written solicitation document will not be issued. Contractor's proposals (cost and technical) are herein requested. A response to requirements listed below shall be submitted on contractor's letterhead or formal quotation form and must address all requirements listed in a separate technical proposal. Contractor's price and technical proposal must contain the name; title and signature of person authorized to submit proposals/bids on behalf of the contractor. The Registered Nurse is to work Monday through Friday 7:30 a.m. to 4:00 p.m. The Registered Nurse will provide services for the Home Based Primary Care Program at the VA North Chicago. Services will be provided for patients within a 25-mile radius of the VA. VA will guarantee work for four (4) weeks. The following is a list of Duties and responsibilities: Assessment of home care patient: physical, psychosocial, functional, environmental/safety, nutritional screening, and clinical reminders. Caseload management: assess patient care needs and deal with problems in priority order, make independent decisions regarding patient care needs, analyze patient situations in the home and develop an appropriate plan, organize time and tasks to function efficiently, adjust daily schedule if unexpected problems occur, keep supervisor informed of major changes in patients, communicate skillfully regarding patient care with members of interdisciplinary team, and make appropriate patient referrals to other interdisciplinary team members. Written documentation: Complete paperwork tasks, assessments, care plans, progress notes, discharge summaries, PCE's, in a timely manner to meet program deadlines. Patient/family management: Provide clear direction to patients and families, plan and implement activities/teaching based on treatment goals and care plan for the patient, provide both physical and psychosocial care to patients, and recognize and deal with family concerns related to the patient's health problems. Skills/Equipment: Demonstrates ability to perform phlebotomy, use portable EKG, use pulse oximeter, use blood glucose monitoring machine, perform urethral catheterization, perform suprapubic catheterization, perform vital signs measurement, utilize rehabilitation equipment, perform gastrostomy insertion, administer tube feedings, perform wound care, perform ostomy care, utilize home 02 equipment, administer injections, IM, SQ, implant, perform nursing bag technique, use Doppler, I.V. Infusion pump, and training can be provided if RN does not have skill. PHYSICAL DEMANDS: Requires walking to patient's residence carrying a bag of necessary equipment. The bag may weigh from 10-15 pounds. There will be time spend sitting at a computer terminal performing various keyboard functions, as well as driving time to patient's home. SPECIAL REQUIREMENTS: (1) Respects confidentiality and follows established policy regarding patient records. (2) Observes VA policies and procedures relating to infection control, Fire and Safety, Information Security, Patient Safety (mandatory training). (3) Perform the required work in accordance with JCAHO standards. (4) Communicates and works cooperatively with other team members to provide comprehensive patient care. A full-time equivalent (FTE) is defined as a contractor's employee working eight (8) hours per day, five (5) days per week (40 hours per week). Contractor's employees must be present at VA and must be actually performing the required services for the period specified in the contract or the contract cost will be decreased accordingly during each billing cycle. A VA record-keeping system of contractor hours worked is established through time and attendance logs in the departments being served. The payments to contract personnel for any leave, including sick leave, holiday, or vacation time is the responsibility of the contractor. Leave granted to contract employees would be in accordance with contractor policies. VA is not obligated to the Contractor during Technician absence. Evaluation of proposals offered will include the following factors: CV of the proposed service providers, copy of all current licenses, competency check list, references, pre-employment physical, current driver's license, proof of car insurance, transcript to verify degree/diploma, criminal background check, Department of Vehicle check, drug screen, TB test, Hep B immunization or statement of declination, child abuse and neglect tracking system, clinical skills/interpersonal skills, availability, past performance and price. Award will be made to the offeror(s) determined to be the "best value" to the Government. Awarded offeror(s) is hereby notified that the position would be considered "low risk" under the positions sensitivity designate in accordance with IL 90-01-6 entitled Contractor Personnel Security Requirements. Evaluation may be made with or without discussions with the offeror(s). All responsible sources that can meet the requirements and provide services as listed above may respond to this solicitation by submitting cost and technical proposals (curriculum vitae) on their company letterhead or bid form.
