- NA This is a combined synopsis/solicitation for a commercial item, prepared in accordance with the format in FAR Subpart 12.6, and supplemented with additional information included in this notice. This announcement constitutes the only solicitation. Quotes are requested; however, a written solicitation will not be issued. Solicitation number DABJ25-03-T-0087 is applicable and is issued as a request for quotations. The solicitation and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2001-03. The requirement is small business set-aside with a NAICS Code of 3337127, SIC Code 2431, and size standard of 500. The description of the commercial item is as follows: Clin 0001: Solid Oak Pews with removal cushions. Pews are required to fit the Chapel (odd measurements), wood must match existing woodwork, cushion color to match proposed carpet, pew style must match historical style (pictures are available). Current size is 26 long, 14 short pew and 2 frontal screens. Must include fabric covered (match cushions), wood frame kneelers and full length Bible/Hymnal rack with communion cup/card/pencil holders, Delivery and Installed. Clin 0002: Choir Chairs, color/finish, cloth seat and back fabric must match pews. Delivery and Installed. Clin 0003: Refinish Ecclesiastical furniture; refinish existing oak chancel furniture to match pew finish/color. Recover w/pew cushion fabric. 2 low back chairs, 1 high back chair, 1 large pulpit, 1 small pulpit, 2 flower stands, and 2 credence tables, 6? ????? Kneeler. Delivery and Installed. Clin 0004 Construct 2 Chancel Chairs - construct 2 chairs to match the style of the existing oak and fabric seat/back chairs but refinished with the new pew color/finish and pew cushion fabric. Delivery and Installed. SITE VISIT IS REQUIRED PRIOR TO QUOTE SUBMITTION PLEASE CONTRACT JACQUELINE EDGERTON FOR APPOINTMENT AT 253-966-3481 OR CHAPLAIN COLLINS AT 253-967-3686. State if FOB is Destination or Origin. If Origin, state shipping charges. Delivery/Instalion must be completed by 15 January 2004 items will be delivered/installed at Installation Chaplain Office, Attn: MAJ Ben Collins, bldg 2001, MS-52, box 339500, Fort Lewis, WA 98433-9500. Any award resulting from this request for quotations will be made to a responsive, responsible quoter using a SF1449 form. Quotes will be evaluated on a based of historical requirments and best value. Award will be made on an all o r none basis. Provision 52.212-1, Instructions to Offertory-Commercial Items, applies to this acquisition. An addendum to this provision is applicable, in which the term offer or (whenever it appears) shall be replaced with the term quoter. Quoter are r equired to submit completed copies of the provisions at FAR 52.212-3, Offeror Representations and Certifications-Commercial Items, with their quotes. Also, please provide Federal Tax ID Number FAR clause 52.212-4, Contract Terms and Conditions-Commercial Items, applies to this acquisition and does not include any addenda. Far clause 52.212-5, Contract Terms and Conditions Required to Implement Statutes or Executive Orders, Commercial Items, applies to this acquisition. The following clauses within 52.21 2-5(b) are applicable to this acquisition: 52.222-19, 52.222.21, 52.222-26, 52.222-35, 52.222-36, 52.222-37, 52.232-33. The following clause within DFARS clause 252.212-7001(b) is applicable to this acquisition: 252.225-7001. DFARS clause 252.204-7004, Re quired Central Contractor Registration is applicable to this acquisition.
