Description
This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. The Request for Quotation (RFQ) is ESV30358 and incorporates provisions and clauses through FAC 2001-07. This procurement is to be processed using Simplified Acquisition Procedures. The NAICS code is 333888.500EM. The National Institute of Environmental Health Sciences is interested in obtaining the following brand name or brand name equal Amsco equipment. Two (2) of each of the following: Amsco SQ240 surgical lighting system, dual intensity controller, surface mount; installation. This is an Amsco brand-name requirement. The brand-name justification is based on the requirement that this brand of equipment will be compatible with existing Amsco equipment used within NIEHS and will allow for interchangeability of parts and supplies as well as continuity of service for the purposes of maintenance and operation. Delivery will be made to NEIHS Warehouse Bldg. 104, 111 T. W. Alexander Drive, Research Triangle Park NC 27709. Offeror should quote FOB Destination. The following provision applies to this acquisition: 52.212-1 Instructions to Offerors. There are no additional addenda. FAR Clause 52.212-2 Evaluation-Commercial Items applies to this acquisition. The government intends to make a "best value buy" to the responsible offeror whose offer is most advantageous to the government. The evaluation criteria will include 1) the technical capability of the items offered to meet the government's needs; 2) price; 3) warranty considerations; and 4) delivery time. Offerors shall include federal tax ID number, DUNS number, and a completed copy of the provisions at 52.212-3, Offeror Representation and Certification-Commercial with offers. FAR Clause 52.212-4, Contract Terms and Conditions-Commercial, applies to this acquisition. The following paragraphs of 52.212.5 apply: (a) and (11)-(16)(21)(22)(25)(27) of (b) and 52.211-6 Brand Name or Equal. Defense Priorities and Allocations Systems (DPAS) does not apply.
