Description
NA This is a combined synopsis/quote for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only quote; quotes are being requested fr om one source, Jungle's Gym, and a written solicitation will not be issued. The Government intends to enter into a contract with only one source under the authority of FAR 602. However all responsible sources able to provide a facility within one-mile of HQ, MTMC, are invited to submit a quote. The solicitation number is W81GYE-3274-9075 and is issued as a Request for Quote. The Request for Quote incorporates provisions and clauses in the Federal Acquisition Regulation 52.212-1, 52.212-2, 52.212-3, 52.2 12-4, 52.212-5, 52.217-8, 52.217.9, 52.222-41, and 52.232-18 and is in effect. These clauses can be found at www.arnet.gov/far/. This RFQ is 100% set aside for small business and the associated NAICS code 713940, and the size standard is $6M. Description of work, make available to service members and civilians employed at HQ MTMC, a physical fitness facility that is within one mile from the H Q MTMC Building (Hoffman-II), 200 Stovall Street, Alexandria, VA 22332. The facility shall be easily accessible to our employees and open before, during, and after the normal workday. The facility must have the capability to evaluate each participating HQ MTMC member with respect to his/her health condition and to recommend and provide the types of exercises, facilities, and programs needed to improve his/her physical condition. The facility shall provide the following, as a minimum: variety of circuit a nd free weight equipment, instructional exercise classes such as Yoga and Aerobics, life cycles/rowing machines, instructions on how to use the equipment, and shall have locker and shower facilities. The facility must be available to our employees during the hours of 0630 to 1800 hours, Monday through Friday. Also the facility should be accessible to HQ MTMC employees on weekends and holidays. Only military and civilian employees (i.e., not contractors) of HQ MTMC-Alexandria are eligible to join. The co ntractor shall provide a list of names of all HQ MTMC members, a monthly participation report and shall make available any special information on programs that are available to HQ MTMC employees. The Contractor shall directly bill members for their fee po rtion on a monthly basis. Military members' shall be refunded (their portion of) unused fees as a result of reassignment. This shall be incorporated by modification. The performance period for this contract shall be 1 Oct 03 through 30 Sep 04, with four one-year options. This contract is subject to the Service Contract Act and wage determination has been requested, upon receipt it shall be incorporated by modification. Evaluation factors are technical (facility location within one mile and facility ser vices) and past performance when combined are more important than price. 18. Alexandria, VA!! 19.
