- Description
- Procurement of contract to include remedial (unscheduled) and preventive (scheduled) maintenance, over and above work, parts and materials and travel for five compact antenna ranges, radome range, F-15/APQ-122, ALQ-172, and miscellaneous antenna range equipment, in accordance with the attached SOW and Equipment List. One basic year and four option years. Work is to be performed on-site at Robins AFB, Georgia. Buyer is Jywanya Smith (phone 478-926-2031/fax 478-926-3590) and Contracting Officer is Nita Steinmetz (phone 478-926-3695/ fax 478-926-3590). This solicitation will be issued approximately 15 September 2003 to Microwave Instrumentation Technologies, LLC, Duluth, Georgia. All responsible sources may submit a bid, proposal, or quotation with shall be considered. Authority: 10 U.S.C. 2304(c)(1), Justification: Supplies (or Services) required are available from only one or a limited number of responsible source(s) and no other type of supplies or services will satisfy agency requirements. See Note 22. Contract will be a commercial acquisition IAW FAR Part 12. STATEMENT OF WORK: Statement of work covers the equipment identified in attachment 1 which supports the five compact antenna ranges, radome range, F-15/APQ-122, ALQ-172, and misc. antenna range equipment. (a) Preventative maintenance on electronic instruments shall be performed, as specified in section 5 of the manuals listed in attachment 2. It shall include cleaning, lubrication, replacement of worn or questionable components, etc. All replacement parts as furnished by the contractor shall be identical to or be an approved equipment manufacturer substitute for the original configuration of the removed part(s). All required PME calibration shall be performed on a 90 day cycle beginning the first, second, and third quarter of the fiscal year. (b) Preventative maintenance of electromechanical equipment, compact antenna range positioners, shall be performed as specified in section 5 of the manuals listed in attachment 2. This shall include cleaning, lubrication, check belt tensions, check reducer oil, etc. All replacement parts as furnished by the contractor shall be identical to or be an approved equipment manufacturer substitute for the original configuration of the removed part(s). (c) The contractor, unless otherwise specified in this Statement of Work (SOW), shall provide a fixed-price contract covering parts, labor, calibration, unscheduled maintenance, travel and per diem charges. He shall perform all preventive maintenance, calibration, certification, and repair as specified IAW applicable Air Force Technical Orders (T.O.'s) and manufacturer's manuals identified in attachment 2. If Air Force Technical Orders are not available to keep the five compact antenna ranges, radome range, and F-15/APQ-122 antenna range equipment in serviceable condition, the contractor shall maintain the equipment to the specifications identified in the manufacturer's official equipment publications, handbooks, and manuals. (d) Contractor shall contact the designated government representative, QAE, five days before the beginning of each quarter and schedule the date and time to perform preventative maintenance for the upcoming quarter. Government representative, QAE, will have final approval of the date and time for the contractor to perform the preventive maintenance. (e) Services (scheduled and unscheduled) will be performed by the contractor during government working hours. Government working hours are 6:00 AM to 2:30 PM Monday thru Friday, excluding Federal holidays. Response time for repairs shall be within twenty-four (24) hours after notification by the designated government representative, QAE. Once begun, repair efforts will continue uninterrupted until ranges are operational. If the contractor determines that repairs are to extend beyond five (5) days, he shall notify government's quality assurance evaluator (QAE) and furnish an estimated date for completion of repair. (f) A system log, with provisions for double entry, will be used to record all transactions pertaining to the contract. The government representative, QAE, will maintain custody of the "system log" and will record all equipment failures, the name of the contractor representative contacted, date and time of contact. The responding contractor representative will make a counter entry with the date and time of arrival and a description of the repair(s) effected. The government representative, QAE, will document in the appropriate section of the contractor's log all changes, both temporary and permanent, to the equipment. Downtime will begin upon notification of the contractor by the government and recording of the name of the contractor representative contacted, date and time of the contact in the system log. (g) Contractor shall review all equipment items identified in attachment 1 within two months after contract award and identify to the Procuring Contracting Officer (PCO) the projected support life of each equipment item. The contractor shall annotate the projected life support in the system log. (h) Repair of the equipment will be accomplished on-site if possible. If any item cannot be repaired on-site, it will be removed from the system and shipped to the appropriate repair center at no additional cost to the government. Items to be repaired shall not be removed from the Robins AFB site to the off-site repair center unless approval is granted by the QAE and applicable sign out procedures are followed. (i) Equipment (calibration standards) used by the contractor to calibrate equipment identified in attachment 1 shall be traceable to the National Institute of Standards and Technology (NIST). (j) All factory recommended upgrades to hardware, firmware, and software shall be installed by the contractor, at no additional cost to the government, using original manufacturer's modification kits and technical data. Upgrades shall be accomplished not later than then next preventive or scheduled maintenance. (k) The government will provide the contractor with a reasonable amount of utilities, work space, and floor space, if needed, in the performance of duties in this SOW. The provided utilities shall be used only in performance with services required on this SOW. The work space and floor space shall be the responsibility of the contractor to keep clean and in an orderly fashion as required to maintain a safe and secure environment. The government assumes no responsibility for any equipment, tools, parts, etc. provided by the contractor and placed in the area provided by the government. (l) Contractor shall not be liable for any excess costs if the failure to perform the contract arises from causes beyond the control and without the fault or negligence of the Contractor such as acts of God. (m) Contractor shall obtain base identification passes for each contractor employee requiring access to Robins AFB by submitting a written request to the base Security Police Squadron/Pass and Registration section (653 SPS/SPAP), Bldg 263. The Identification Credentials Request Form shall specify the contract number, location, or work sites, date the entry to the base must begin and contemplated termination date (including all option periods), names of employees requiring entry, and the name of the company contract manager who will issue the request. The contractor shall attach a copy of the contract cover page to each request form. (n) Contractor shall obtain either a Visitor/Vehicle Pass or a DoD Registered Vehicle Form for vehicle decals when the Identification Credentials Request Form is submitted. The contractor must provide a valid drivers license, proof of financial responsibility or insurance which meets the minimum requirements of the "required insurance" clause of the contract, and proof of legitimate custody of the vehicle. (o) All Identification Credential Forms and Vehicle Registration decals issued by the base Pass and Registration Office are accountable to the contractor and must be returned or accounted for by clearance certification through 653-SPS/SPAP before a final invoice for payment at the conclusion of the contract is submitted. Credential forms and vehicle decals for employees who no longer require access must be returned promptly to 653-SPS/SPAR. The contractor employed will be required to display and surrender Identification Credentials as required by AFR 125-37 (Installation and Resources Protection Program). (p) Contractor is responsible for all controlled/restricted area badges issued for this project. Twenty-five dollars ($25.00) per badge will be deducted from the final contract payment for badges lost or not returned, regardless of the reason for said loss/return. If a receipt is desired for badges turned in during the contract period, the contractor must furnish same for coordination. (q) The safety requirements for this requirement are identified in Appendix C, attached to the Statement of Work. ATTACHMENTS: Equipment List; Document List; Appendix C
