Description
NA This is a combined synopsis/solicitation for commercial services prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. To provide Post Warranty Service for 1- Avaya G3SI revision 11 with 700 ports; 1- Avaya G3SI revision 11 with 400 ports; 1- Avaya G3SI revision 11 with 300 ports; 1- Avaya Definity one revision 10 with 150 ports and 1- Avaya Intuity Audix Mapped R5 16 por ts Telephone Systems for the Vermont Army National Guard for the period of Service of 1 November 2003 through 30 September 2004. The following is a description of the services desired: 1. Coverage Options. Business Day. Contractor will provide remedial maintenance service during coverage period hours, which are 8 a.m. to 5 p.m., Monday through Friday, excluding observed holidays (Coverage Period). Contractor will commence remedial m aintenance service for major failures within four (4) Coverage Period hours of notification from contractor provided monitoring devices or software. Contractor will provide remedial maintenance service for minor failures within twenty-four (24) hours of n otification, except that work will be performed only between 8 a.m. and 5 p.m., Monday through Friday, excluding observed holidays. Contractor must provide a toll-free twenty-four (24) hour Hotline for trouble reporting or maintenance questions. Around-The-Clock. Contractor will commence remedial maintenance service for major failures twenty-four (24) hours per day, seven (7) days per week, within four (4) hours of notification. Contractor will provide remedial maintenance service for minor failu res within twenty-four (24) hours of remote notification, that work will be performed between 8 a.m. and 5 p.m., Monday through Friday, excluding observed holidays. Mail-In Coverage. Contractor will ship a replacement set/board(s) to the Client within 24 hours of notification to in exchange for an equal number and type(s) of non-working set/board(s). The Client must return the nonworking set(s) to Contractor within t en (10) business days from the day replacement set(s) are received. This offer will provide on-site labor to repair or replace telephone or board set(s). 2. Monitoring. This is a service that utilizes its self-developed alarm reporting system that provides 24-hour monitoring and reporting of alarms generated by a DEFINITY System. Under this service, alarms are routed directly to an Service Technician. The alarming system will report the switch location, local INADS information, the maintenance alarm and severity of the trouble. The service technician will intervene remotely to clear the trouble. If the trouble cannot be cleared remotely, the service techni cian will be dispatched for on site intervention with replacement parts within specified time frame depending on the severity of the alarm generated. 3. Post Warranty Service. Post Warranty service includes replacement parts required for products used under normal operating conditions. If you subsequently purchase products (added products) from other contractors and co-locate those products with the ex isting ones, charges for added products will be at the current rate and coverage and will be coterminous with the coverage for existing products. Added products purchased from a source other than will be covered under this agreement. Purchased or replacement parts and products under this agreement may be new, remanufactured, or refurbished, and must have a full one year warranty with them. Any removed parts and/or products will become the property of Contractor. The Contractor and/or Avaya may electronically monitor our system for the sole purpose of collecting and recording the configuration of and the numbers and kinds or products in your system. Such monitoring will serve two purposes: (1) the information will permit more accurate remote diagnostic and corrective actions; and (2) the information will be used to determine applicable charges for added products and renewal charges. *Telephones will not be covered by this agreement.
