- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6 as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotations are being requested and a written solicitation will not be issued. The solicitation number N00259-03-T-0130 is issued as a request for quotation (RFQ). The solicitation document and incorporated provisions and clauses are those in effect through FAC 2001-15. 339112 is NAICS (North American Industry Classification System)Code. This is Service Maintenance Contract, Base plus four (4) option years for government owned GALAXY IV INTRAVASCULAR ULTRASOUND MACHINE to include all parts, labor, transporation and unlimited telephone calls for the period 14 Oct 03 thru 30 Sep 07. A listing of the line item numbers (CLINS) and description are as follows: Clin 0001: Base Year (14 Oct 03 ? 30 Sep 04); Clin 0002: Lot I Option Year 2 (01 Oct 04 ? 30 Sep 05); Clin 0003: Lot II Option Year 3 (01 Oct 05 ? 30 Sep 06); Clin 0004: Lot IV Option Year 4 (01 Oct 06 ? 30 Sep 07). Quotes will be evaluated based on past performance, technical capability and price. Prior to award, the government will review the quotes for the purpose of ascertaining their technical capability. Statement of Work will be provided upon request. This acquisitions incorporates the following FAR Clauses: FAR 52.211-17 Delivery of Excess Quantities (SEP 1989); 52.212-1 Instructions to Offerors-Commercial Items (Oct 1995); 52.212-3 Offer or Representations and Certifications-Commercial Items (Oct1995) must be completed and submitted along with the proposal, a copy can be obtained by contacting the Contract Specialist, Evelyn D.
