Description
This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in the notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. This solicitation is issued as a request for quotation. This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 01-14 and Defense DCN 08-21. If two (2) or more responsible and technically acceptable small businesses respond, this acquisition will be 100% set-aside for small business, if not, it will be unrestricted. The Federal Supply Class (FSC) is J035. The associated Standard Industrial Classification (SIC) is 7623. The North American Industry Classification System is 811412 and the associated business size standard is 6 Million Annually. This request for quotation consists of the following: Item 0001 Services Nonpersonal: To provide all labor, tools, parts, materials, and transportation to provide maintenance, repair, and installation of washers & dryers at Travis AFB in accordance with the Statement of Work dated 01 Oct 03. Quotes should be divided into two line items, Maintenance costs, and parts. The following provisions and or clauses apply to this acquisition. Offerors must comply with all instruction contained in FAR 52.212-1, Instructions to Offerors-Commercial. FAR 52.212-2 Evaluation-Commercial Items, paragraph (a) to this provision is the price: FAR 52.212-3, Offeror Representation and Certification-Commercial Items, a completed copy of this provision shall be submitted with the offer. Quotes without this provision will not be considered for an award. FAR 52.212-4, Contract Terms and Conditions-Commercial Items will be incorporated into any resultant Purchase Order by reference. FAR 52.212-5, Contract Terms and Condition Required to Implement Statues or Executive Orders-Commercial Items include clauses (14), (15), (16), (17), (18), (29) and (c). DFAR 252.212-7001, Contract Terms and Conditions (Dev). 52.222-19 Child Labor, DFARS 252.204-7004, Required Central Contractor Registration. 52.247-34 F.O.B. Destination. 52.252-2 Clauses Incorporated by reference, clauses may be accessed electronically in full text through http://farsite.hill.af.mil/VFFARA.HTM. 52.252-6 Authorized Deviation in Clauses. 52.253-1 Computer Generated Forms. Vendors must be actively registered with the Central Contractor Registration (CCR). The CCR website is http://www.ccr.gov. Contractors are asked to submit their TAX ID number with their proposal. Consideration of quotes is contingent upon compliance to all requested information. Quotes may be transmitted by e-mail or FAX to (707) 424-0288 and are to be received at the 60th Contracting Squadron no later that 4:30 PST on 23 Sept 2003. The point of contact for this solicitation is SSgt Lynnial Trusty, Contract Specialist, (707) 424-7764, lynnial.trusty@travis.af.mil Statement of Work 01 Oct 03 1. DESCRIPTION OF SERVICES. The contractor shall provide preventive maintenance (PM), repair, and replacement and /or installation of washers and dryers to the on-base locations listed in Appendix A. (Installation shall be considered as maintenance and repair). These requirements shall be in accordance with the statement of work, commercial practices, the manufacturer's recommendations and local, state and federal laws. The contractor is encouraged and expected to use innovative approaches to efficiently and effectively accomplish the requirements of this contract in a timely manner, at reduced costs. The evaluation of services will be based on customer satisfaction, contractor's quality control, government inspections, and best trade practices. (The user will be responsible for properly disposing of the replaced unit). 1.1 Service Response/Request. The contractor shall respond to a service request within 24 hours. The contractor shall return washer / dryer units to operational condition within 3 workdays after notification of service requirement, unless otherwise approved by the government representative or Contracting Officer (CO). The contractor shall notify the government representative when replacement part(s) must be ordered and/or repair delays are expected. If the repair time is expected to exceed 7 calendar days, the contractor shall supply a replacement machine at no additional expense to the government. The contractor shall notify the government representative when maintenance is to be performed and when maintenance and / or repairs are complete. The contractor shall only accept service requests from authorized government representatives and / or the CO. If the contractor responds to a service request from someone other than authorized personnel, the government shall not be responsible for payment of work performed. The CO will provide the contractor a letter with names of authorized personnel. 1.2 Maintenance and Repair Record and Monthly Invoice. The contractor shall provide a weekly record of maintenance, repairs, or PM that has been performed. (Receipts for new parts purchased for the repair of washers and dryers will be provided with the contractors invoice). The record shall identify at least the following for each unit, (Date/Time of Call, Name of Customer, Phone Number, Appliance Make & Model, Appliance Serial Number, Type of Service/Repair, Parts Replaced, Breakout of all Costs, Building & Floor Number, Service Completion Date). This may be accomplished via fax, email or in person and shall be on an official company document. The weekly record shall be legible and be provided to the government representative by close of business (COB) every Friday and the monthly invoice shall be due not later than COB the 15th of every month. 1.3 Maintenance and Repair. The contractor shall ensure washers and dryers are maintained according to the manufacturer's recommendations. The contractor shall ensure all washers and dryers operate safely, are clean, and free of corrosion. The contractor shall minimize disruptions and inconveniences to users. All replacement parts shall be new, unless the contractor and the government representative / CO mutually agree upon refurbished parts as an acceptable replacement or repair. The contractor shall not exceed the established repair cost of $100 without prior approval by the government representative or CO. The contractor shall be reimbursed for all parts. 1.4 Property Control. Before removing any item from the existing location, the contractor shall obtain approval of the government representative or CO. 2. SERVICE DELIVERY SUMMARY (SDS) Performance Objective SOW Para Performance Threshold Service Response, Maintenance & Repair Records, and Monthly InvoicesService shall be received within specified time. Customer is satisfied with response time. Documents shall be received within specified time. 1, 1.1, 1.2, 1.3, & 1.4 98% of all service calls shall be satisfactorily completed within required time. Reports/invoices shall be provided to the government representative 100% of the time and within specified time. Maintenance and RepairAppliances shall be in proper working order and maintained according to manufacturer recommendations. Repairs shall be made within specified time in the SOW and temporary replacement appliances provided when repair time exceeds 7 calendar days. Contractor shall notify government representative when repairs are completed. 1, 1.1, 1.2, 1.3, & 1.4 98% of all appliances shall be properly working. 100% of all appliances shall be repaired in a timely manner. Temporary replacement appliances shall be provided when repair time exceeds 7 calendar days 100% of the time. Contractor shall notify government representative when repairs are completed 100% of the time. Property ControlBefore removing any item from the existing location, the contractor shallobtain approval of the government representative or CO. 1, 1.1, 1.3, & 1.4 Before removing any item from the existing location, the contractor shall obtain approval of the government representative or CO 100% of the time. 3. GOVERNMENT FURNISHED FACILITIES PROPERTY AND SERVICES. The government shall provide the contractor normal utilities, such as water and electricity, to perform all maintenance and repairs under this statement of work (SOW). 4. GENERAL INFORMATION. 4.1. Contractor Personnel. The contractor shall not employ persons for work on this contract if such employees are identified to the contractor by the CO as a potential threat to health, safety, security, general well being, or operational mission of the installation and its population. 4.2. Contractor personnel shall present a neat appearance and be easily identified as contractor employees. This may be accomplished by wearing clothing or uniforms bearing the name of the company or at a minimum by wearing an appropriate badge containing the company name, employee name and if possible a picture. Contractor's employees while performing work under this contract may not wear the military uniform or any part thereof. Additionally employees shall be fully attired. If the contractor elects to wear uniforms, they shall be standard in style and appearance. The cost and maintenance of uniforms shall be the responsibility of the contractor. The contractor and contractor representative shall speak, read, and write English fluently. 4.3. The contractor shall not employ any person who is an employee of the United States Government if employing that person would create a conflict of interest, the appearance of a conflict of interest or policies in AFI 64-106 - Air Force Industrial Labor Relations. An employee of the United States Government must seek and receive approval according to DOD 5500.7 - Joint Ethics Regulations. 4.4. Blocked Access to Perform Work. The contractor shall notify the government representative immediately in the event that they cannot perform the required service due to being denied access. At the time of notification, if the government representative cannot resolve the issue of access for the contractor to perform the service, an alternate time will be mutually arranged at no additional expense to the government. 4.5. The contractor shall notify the government of any circumstances of needed repair or maintenance of assigned space or of any area that may affect performance of contract work, present unsafe or unhealthy conditions, or cause any delays/interference of work by employees of the contractor. The contractor shall make such notification to the government representative immediately. 4.6. Workload Estimate. The contractor shall notify the government representative in writing when actual conditions differ from those provided by the government. If the contractor does not inform the government representative/CO of differing conditions, the government shall not be responsible for any additional cost that may be incurred by the contractor. 4.7. Training. The contractor shall provide environmental, health and safety training to their employees to ensure compliance with all federal, state, and local laws or regulations. This training will be provided at the contractor's expense. The contractor will maintain the applicable certificates or proof of training and will be made available to the government when requested. 4.8. Certificates. All personnel must be certified to due warranty work on all machines under warranty. 4.9. Performance of Services During Crisis Declared by The National Command Authority. All services to be performed under this contract have been determined to be non-essential for performance during crisis according to the Department of Defense Instruction (DODI) 3020.37 and Air Force implementation thereof, unless otherwise directed by the CO and government representative. 4.10. Conservation of Utilities. The contractor shall ensure employees practice conservation of all utilities, e.g. gas, water, electricity. The contractor shall be responsible for operating under conditions that prevent the waste of utilities to include but not limited to the following: 4.10.1. Lights shall be used only in areas where work is actually being performed. 4.10.2. Employees shall not adjust mechanical equipment controls for heating, ventilation, and air conditioning systems. In addition, all personal equipment owned by government employees or military members shall not be adjusted. 4.10.3. Water faucets or valves shall be turned off when not in use. 4.10 Government Remedies: The contracting officer shall follow the requirements of FAR 52.212-4, Contract Terms and Conditions for Commercial Items (May 1977), for contractor?s failure to correct nonconforming services. 5. CONTRACTOR'S QUALITY CONTROL. The contractor shall employ a quality control program/procedure(s) to identify, prevent and ensure non-recurrence of substandard services. A copy of the Quality Control Plan will be provided to the Contracting Officer NLT five (5) workdays after the start of the contract. Through implementation of the contractor?s quality control program/procedure(s), the government will receive quality services that meet the requirements of this contract. The contractor shall provide a written Quality Control plan to the CO prior to contract award. In accordance with commercial standards, training is a vital component of quality control. Employee training shall be integrated with Quality Control to ensure the provision of quality service. All employees shall be properly trained and/or certified. The contractor shall make all Quality Control documentation available to the government upon request. 6. GOVERNMENT REMEDIES. FAR Clause 52.212-4 - Contract Terms and Conditions - Commercial Items, paragraph a) Inspection/Acceptance becomes the basis for the CO's actions. The government has the right to inspect and test all services, to the extent practicable at all times and all locations during the term of this contract. The government will perform inspections and tests in a manner that will not unduly delay the work of the contractor. 7. HOURS OF OPERATION. The hours of operation for the base are 0700 to 1630 hours, Monday through Friday. The contractor shall work during these work hours and workdays. Any work performed before or after these hours of operation, on weekends or any overtime the Government Representative or CO shall first approve work. 8. SECURITY REQUIREMENTS. All personnel employed by the contractor in the performance of this contract, or any representative of the contractor entering the government installation, shall abide by all security regulations of the installation. The contractor shall submit a roster to include full name, aliases, social security number, and home address. The roster shall be furnished through the CO to the Security Forces Pass and Registration Office at the pre-performance conference and as changes occur. Vehicle registration, proof of insurance, and a valid driver?s license must be presented for all vehicles to be registered and are required to enter and drive a vehicle on the installation. 8.1. Base Requirements. The Contractor's employees, performing services within secured areas at Travis AFB, shall obtain a security badge or shall be escorted by personnel from the organization requesting the services work. The contractor shall ensure each employee obtains the pass and/or identification required for contractor personnel and for personal owned vehicles from Security Forces Pass and Registration Office. Personnel without base issued identification will be sponsored onto the installation during Force Protection Condition (FPCON) Normal and Alpha and physically escorted onto the installation during FPCON Bravo. 8.2. Background Checks. All contractor personnel shall obtain a criminal background check from the California Department of Justice, Sacramento, CA or equivalent. The CO, prior to use, must approve any equivalent source. The results of this criminal background check shall be provided to the 60th Security Forces Squadron for review and approval. Continued employment is contingent upon successful completion of the criminal background check. The Fair Credit Reporting Act (FCRA) provides that written notice and the name of the criminal check agency is given to individuals when employment is denied. 8.3 Physical Security. The contractor shall be responsible for safeguarding all government property provided for contractor use. At the end of each work shift, all government facilities, equipment and materials shall be secured. 9. PERFORMANCE EVALUATION MEETINGS. The contractor shall meet with the CO and/or government representative at least weekly during the first month after implementation of the contract and when deemed necessary by the CO or government representative. The meetings will be used to establish a partnering relationship with the contractor to address overall performance and/or any performance problems encountered and their resolutions. The time and location will be mutually agreed upon. The government reserves the right to change the frequency of the meetings depending on the quality of service provided by the contractor. The government will prepare, sign and distribute minutes of the meetings to all attendees. If the contractor does not concur with any portion of the minutes, the contractor shall provide in writing to the CO within five working days following receipt of the minutes.
