- NA This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in far subpart 12.6-streamlined procedures for evaluation and solicitation for commercial items-as supplemented with additional information included in thi s notice. This announcement constitutes the only solicitation; quotations are being requested, and a written solicitation document will not be issued. This solicitation is a Request for Quotation (RFQ). The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 01-15. ***The associated North American Industrial Classification System (NAICS) code for this procurement is 423850 with a small business size standard of 100 people. All interested off erors may submit a quote. This shall be a Firm Fixed price contract. CLIN 0001, Family Housing Program Assistant 1,920 Hours Work as follows. . Statement of Work ARMY FAMILY ACTION PLAN & FAMILY PROGRAMS 1. General 1.1 The Contractor shall assist the Government in accomplishing the Army Family Action Plan (AFAP) objectives plus assist in implementing other Army Community Service (ACS) programs that include but are not limited to Volunteers (Make A Difference Day), Family Advocacy (Celebrating Military Children), Mil itary Spouse??????s Day, Exceptional Family Member Program, Exceptional Family Member Publicity Program. The Regulation 608-1 entitled Army Community Service Center is available at ACS; build 4260,Ord Military Community, Ca. 2. Background Information. The mission of the ACS programs is to educate, coordinate and deliver services, which promotes self-reliance, resiliency, readiness and stability within military families. 2.2 ACS will provide four (4) hours of orientation on the focus of each of the above programs at no cost to the Contractor. Training will produce a working knowledge of the local AFAP process and the goals of the above listed programs. 2.3 Location of Work. The services will be performed, during the normal duty hours (8:00 to 17:00) at the Army Community Service Office, Build 4260, Ord Military Community. Due to the nature of the services only 10% of the work can be conducted at an indep endent location. Certain special programs/events will be offered after duty hours and located in government facilities. 3. Personnel. 3.1 All information provided to the Contractor by the Government or generated by the Contractor in the performance of this contract shall become the property of the Government. This property shall be turned over to the Director, Army Community Services at the expiration of the contract. 3.2 The Contractor is required to be able to lift, catch, bend, stoop, reach, and have the ability to climb stairs. 3.3 The Contractor is expected to work independently, without direct government supervision. However, coordination with the Contracting Officer or his designee is considered essential to the successful completion of this contract. 3.4 Failure of the Contractor to provide services stipulated will cause a reduction in payment in accordance with the Inspection of Services Clause. 3.5 For payment purpose, the Contractor shall submit a copy of his/her monthly invoice to the Director of Army Community Services or his/her monthly invoice to the Director of Army Community Services or his/her representative for certification of hours worked. For verification of services performed, the Contra ctor shall be required to sign-in prior to starting work. The sign-in sheet will be located at the ACS Office. If the contractor cannot report to work, due to illness or personal reasons, he/she will call the ACS Director 1 hour after start of normal duty hours. 3.6 Employee Identification. 3.6.1 The Contractor must be recognizable as such on the work site. This shall be accomplished by wearing a name badge/tag on the exterior clothing. The nametag must contain the contractor??????s name and title. It will be th e responsibility of the contractor to furnish his/her own nametag. 3.7 Qualifications of Contractor Personnel. 3.8 The Contractor shall possess as a minimum a high school degree. A BS/BA degree or 2-year degree is highly recommended. A copy of the diploma or degree achieved shall be submitted with a copy of the proposal. 4.0 Quality Assurance. 4.1 The Government will monitor the Contractor??????s performance under this contract using the Inspection of Services clause. Government personnel will conduct unannounced inspections for compliance. Government personnel conducting inspections include, but are not limited to, the Contracting Officer and the Director of Army Community Service or his/her representative. Should the Contractor not concur with the minutes, he shall state, in writing, to the Contracting Officer, any area wherein he/she does not concur. 4.2 The Contractor will coordinate and execute the ACS programs listed at 1.1.Services should be conducted at an available office space at Army Community Service with 10% of the time spent at an independent location. Services will total 40 hours a week, 16 0 hours per month for a total of 1,920 hours. The Contractor shall not be required to provide services on days, which are Government Holidays. (Veterans Day, Thanksgiving, Christmas, New Years, Martin Luther King, Memorial Day, President, Washington Birthd ay, Fourth of July) 5.0 SPECIFIC SERVICES. 5.1 Make A Difference Day www.makeadifferenceday.com Is a national day of helping others. The hotline for Make A Difference Day is 1-800-416-3824. The contractor shall develop and execute an installation wide program. This would include: An event with many activities to involve the total DLI military community. a. Contact community service organizations to inquire if there is an interest in a collaborative partnership with the military units and the help they need. This is a way to initiate action on a major community concern or issue. b. The contractor will design, develop and implement an effective marketing strategy and public information program to recruit volunteers for the Make A Difference Day program. c. Establish meetings with organizations and volunteers. Identify the projects, locations, and times of activity. Have coordination meetings. Get organizations to provide supplies for activity if possible. Continue to coordinate follow-up with units. d. Coordinate with Public Affairs to publicize the event. Have them provide the HEARLD with written article. Request and invite HEARLD photograph some of the activities. e. Provide certificates for the units and organizations that participates. Take ACS camera and take photos of the event. f. Submit a description of the completed projects to be considered for an award from the Points of Light Foundation. g. Depending on funding assist ACS in setting up a morning Kick-Off Breakfast for the day??????s events. 5.2 Armed Forces Action Plan. This program identifies concerns of America??????s Military Families, determine actions required, and assign agencies that are in the Pentagon and Department of the Army to implement action to resolve issues. The AFAP is a grassroots process identifying issues of concern to America??????s Armed Forces Family that add or detract from the quality of life for the military. a. The Contractor will design develop, and implement effective marketing strategies and public information program to recruit delegates, recorders, and issues for the local symposium. b. The contractor shall assist in soliciting issues from the community with the emphasis being on the Navy/NPS, Marines, Air Force, and Coast Guard. Plus California Medical Detachment, ROTC, DMDC, DFAS. Issues will be typed up plus follow-up will be done f or clarification with the government organization that is responsible. c. Contractor shall assist the Army Community Service in recruiting military family members from 5.2, b to be d elegates to the AFAP Symposium. d. Contractor will perform general office support functions; meet all internal/external suspense??????s established by COR and assist in collecting/maintaining/reporting statistical data on AFAP issues, volunteers, and delegate demographics. Attend for coordination purposes AFAP Council meetings, advisory meetings and provide reports on program status, current progress of events. 5.3 ACS Installation Volunteer Program Assist the ACS Director with the following: a. Establish and implement a volunteer Advisory Council. Maintain orders/minutes of meetings, which will be forwarded through the chain of command to the garrison commander. b. Assist garrison organizations with implementation of required forms for volunteers. Assist organizations with organization/record keeping of volunteer hours. c. Gather information on training requirements for installation volunteers. Make recommendations on volunteer management issues effecting organizations. d. Maintain the volunteer job bank with listing of currents volunteer positions on post and in the Monterey community. 5.4 ACS Installation participants Ceremony, Community Health Fair: a. The Contractor shall assist Army Community Service in the execution of these community events to include recruiting participants, assisting with publicity, communicating/corresponding with units and outside organizations to achieve the objectives of the event. 6. GOVERNMENT FURNISHED PROPERTY AND SERVICES. 6.1 The Government shall furnish office space at the Army Community Services, Building 4260, Ord military Community. This includes a desk, chair, cabinet, computer, typewriter and use of a copying machine and all office supplies necessary to comply with th is contract. 6.2 The Government shall furnish postal and installation distribution services for all official items. 6.3 The Government will provide limited supplies and equipment necessary for the performance of this contract. The Government property made available under this contract shall be for use only in the performance of the contract. 6.4 The Government shall not be responsible in any way for damage to the Contractor??????s supplies, materials, equipment, or the Contractor??????s personal belongings due to fire, theft, accident or other disaster. 7. CONTRACTOR FURNISHED ITEMS. 7.1 The Contractor shall furnish his/her own transportation in the performance of this contract. Additional information: Contractual point of contact is: Phillip A.
