- REQUEST FOR MAINTENANCE SERVICE SUPPORT FOR ULTRASONIC DIAGNOSTIC SCANNERS BY ADVANCED TECHNICAL LABS AND DIFFERENT TRANSDUCERS WITH THE ULTRASONIC SCANNERS. NOTE: CONTRACTOR IS TO BE ABLE TO PROVIDE A CERTIFICATE OF COMPLETION OF TRAINING ON REPAIR/MAINTENANCE OF MANUFACTURER?S EQUIPMENT AS WELL AS PROOF OF BEING AN AUTHORIZED REPRESENTATIVE OF THE MANUFACTURER. GENERAL: THE CONTRACTOR SHALL PROVIDE PREVENTIVE AND CORRECTIVE MAINTENANCE IN ACCORDANCE WITH MANUFACTURERS LITERATURE DURING NORMAL HOURS 7:30 AM TO 4:00 PM, WEEKDAYS, EXCEPT GOVERNMENT HOLIDAYS FOR THE PERIOD OF 1 OCTOBER 2003 TO 30 SEPTEMBER 2004. ALL MAINTENANCE AND SERVICE SHALL BE PERFORMED BY COMPETENT, PROFESSIONALLY TRAINED AND TECHNICALLY QUALIFIED SERVICE TECHNICIAN. THE CONTRACTOR SHALL PROVIDE A FIELD SERVICE REPORT FOR ALL WORK ACTIONS TO BIOMEDICAL ENGINEERING DIVISION UPON COMPLETION OF WORK. SUCH DOCUMENTATION SHALL BE IN ACCORDANCE WITH CURRENT REGULATIONS AND INDUSTRY STANDARD PRACTICE. CONTRACTOR?S RESPONSIBILITIES: PERFORMANCE: THE CONTRACTOR SHALL BE CALLED AND RESPOND TO CORRECTIVE MAINTENANCE REQUESTS BY TELEPHONE WITHIN TWENTY-FOUR (24) HOURS, ON SITE WITHIN FORTY-EIGHT (48) HOURS, AND HAVE THE EQUIPMENT IN GOOD OPERATING CONDITION WITHIN SEVENTY-TWO (72) HOURS OF INITIAL REQUEST FOR SERVICE. IF, AFTER SEVENTY-TWO (72) HOURS, THE EQUIPMENT IS NOT IN GOOD OPERATING CONDITION AS DETERMINED BY THE OPERATOR(S), THE CONTRACTOR SHALL PROVIDE LOANER EQUIPMENT. THE HEAD OF MATERIELS MANAGEMENT DEPARTMENT SHALL BE NOTIFIED BY THE CONTRACTOR IMMEDIATELY SO THAT APPROPRIATE ACTION MAY BE TAKEN. THE CONTRACTOR SHALL FURNISH MAINTENANCE SERVICE SUPPORT TO INCLUDE AS A MINIMUM CORRECTIVE AND PREVENTIVE MAINTENANCE FOR THE EQUIPMENT TO ENSURE THAT IT IS KEPT IN GOOD OPERATING CONDITION. THE CONTRACTOR SHALL PROVIDE ALL SOFTWARE AND PART UPGRADES INCLUDING PRICE LISTING (EXCEPT CONSUMABLES), LABOR, SERVICE LITERATURE (WITH SCHEMATICS, DIAGRAMS, AND IDENTIFICATION OF PARTS) AND TRAVEL NECESSARY TO KEEP THE EQUIPMENT IN GOOD OPERATING CONDITION AND MEET ALL PUBLISHED SPECIFICATIONS LISTED BY THE ORIGINAL EQUIPMENT MANUFACTURER. PREVENTIVE MAINTENANCE: THE CONTRACTOR SHALL COMPLY WITH THE ORIGINAL EQUIPMENT MANUFACTURER'S PREVENTIVE MAINTENANCE PROCEDURES, AND SHALL INCLUDE AS A MINIMUM LUBRICATION, CLEANING, FUNCTIONAL CHECKS AND ADJUSTMENTS, LEAKAGE CURRENT MEASUREMENTS, AND OTHER MAINTENANCE TO KEEP THE EQUIPMENT IN GOOD OPERATING CONDITION. A MINIMUM OF ONE PREVENTIVE MAINTENANCE INSPECTION SHALL BE PERFORMED DURING THE MONTH OF JANUARY 2004. THE CONTRACTOR SHALL SCHEDULE ALL PREVENTIVE MAINTENANCE INSPECTIONS WITH THE BIOMEDICAL ENGINEERING DIVISION TO ENSURE THAT THE EQUIPMENT WILL BE MADE AVAILABLE BY THE EQUIPMENT CUSTODIAN. ALL FIELD SERVICE REPORTS SHALL BE DOCUMENTED USING REFERENCE TO THE EQUIPMENT BARCODE# (PLANT PROPERTY NUMBER). ALL FIELD SERVICE REPORTS SHALL BE DROPPED OFF IN THE BIOMEDICAL ENGINEERING DIVISION PRIOR TO DEPARTURE FOR SERVICE HISTORY DOCUMENTATION REQUIREMENTS. WORK VERIFICATION AND MAINTENANCE RECORDS: THE CONTRACTOR SHALL ONLY RESPOND TO SERVICE CALLS PLACED BY BIOMEDICAL ENGINEERING DIVISION PERSONNEL. FOR EQUIPMENT LOCATED IN THE NAVAL HOSPITAL, BLDG 2268, THE CONTRACTOR SHALL CHECK IN AT THE SECURITY OFFICE FOR A TEMPORARY VENDORS BADGE, AND THEN TO BIOMEDICAL ENGINEERING DIVISION PRIOR TO PERFORMING ANY WORK. FOR EQUIPMENT NOT LOCATED IN THE NAVAL HOSPITAL, BLDG 2268, THE CONTRACTOR SHALL BE GIVEN A POINT OF CONTACT WHEN THE SERVICE CALL IS PLACED. UPON COMPLETION OF THE WORK THE CONTRACTOR SHALL GIVE THE POINT OF CONTACT A SIGNED LEGIBLE COPY OF THE SERVICE REPORT. THE POINT OF CONTACT WILL THEN SUBMIT THE SERVICE REPORT TO THE BIOMEDICAL REPAIR DIVISION. SPECIAL TOOLS, HARDWARE, SOFTWARE, PARTS, TEST EQUIPMENT, AND EQUIPMENT MAINTENANCE LITERATURE SHALL BE PROVIDE BY CONTRACTOR. PART AVAILABILITY: MAINTENANCE ACTIONS SHALL NOT BE DELAYED FOR MORE THAN TWENTY-FOUR (24) HOURS DUE TO AWAITING PARTS. ALL REPLACEMENT PARTS SHALL BE NEW AND OF FIRST RATE QUALITY. POINT OF CONTACT: THE CONTRACTOR SHALL PROVIDE A POINT OF CONTACT FOR ALL MAINTENANCE THAT IS REQUIRED. THIS INFORMATION SHALL INCLUDE THE NAME, ADDRESS, AND TELEPHONE NUMBER OF CONTRACTOR SERVICE PERSONNEL. CREDENTIALING: THE CONTRACTOR SHALL PROVIDE DOCUMENTATION INDICATING THAT TECHNICIAN IS CERTIFIED/QUALIFIED TO PERFORM SUCH MAINTENANCE AND OR REPAIRS. TRAINNING: CONTRACT IS A ?FIRST LOOK? CONTRACT WITH A TRAINED IN-HOUSE BMET RESPONDING TO ALL TROUBLE CALLS FIRST. IF REPAIR IS OUT OF THE SCOPE OF TRAINED BMET, BIOMEDICAL ENGINEERING WILL THEN CALL IN CONTRACTOR FOR SERVICE OR FOR PHONE SUPPORT.
