Description
Synopsis/solicitation Description: This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. Solicitation 506-07-04 is being issued as a Request for Quotation. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 2001-15. NAICS Code 532490, 500 employees (small business size standard) applies. The Contractor shall provide all parts, materials, labor, supervision, test equipment, technical manuals and transportation necessary to perform preventive maintenance and unlimited emergency repairs on the Government owned Trane A/C units, located at the VA Ann Arbor Healthcare System, 2215 Fuller Road, Ann Arbor, MI 48105, for the period of October 1, 2003 through September 30, 2004, in strict accordance with all schedules, specifications, terms, conditions and provisions. All services must be performed in conformance with the National Fire Protection Agency (NFPA), Occupational Safety and Health Administration (OSHA), and Original Equipment Manufacturer standard and specifications. Hours of work: 7 a.m. to 3 p.m., excluding Federal holidays, or as otherwise arranged with the Contracting Officer Technical Representative (COTR). Work performed outside normal working hours at the COTR_s request shall be billed separately. Contractor shall perform preventive maintenance procedures as arrange with the COTR. Procedures shall be in accordance with the published preventive maintenance manuals for the following equipment: a) Screw Chiller, Model #RTAA1854, S/N U96H25975; b) Screw Chiller, Model #RTAA1854, S/N U96H25976; c) Screw Chiller, Model #RTAA1854, S/N U96H25977; d) Scroll Chiller, Model #CGAEC404A, S/N J96G82502; e) Scroll Chiller, Model #CGAEC604A, S/N J96G82501; f) Reciprocating Chiller, Model #CGA120B4, S/N N232H00BH; g) Reciprocating Chiller, Model #CGA180B4, S/N L342L0KBH. Service work shall be completed according to manufacturers established procedures and checklist, (or Contractor-supplied equivalent satisfactory to the COTR) specifications. A Field Service Report shall be supplied to the COTR at the completion of each preventive maintenance procedure. Preventive maintenance procedures shall include, but is not limited to, the following: 1) cleaning of equipment; 2) completing Original Equipment Manufacturer (OEM) field service updates for operational and reliability engineering change notices; 3) calibrating, inspecting pressures and lubricating the equipment; 4) performing remedial maintenance of non-emergent nature; 5) testing and replacing faulty and worn parts; 6) inspecting/replacing electrical wiring and cables for wear and fraying; 7) inspect all mechanical components including, but not limited to _ cables and mounting hardware, chains, valves, belts, bearings and tracks, interlocks, clutches, compressors, refrigerants, sensors, controls, and motors for mechanical integrity, safety, and performance to OEM specifications; 8) return equipment to operating condition defined in OEM specifications; 9) replace any OEM labels, decals, or warning tags that are not legible; 10) provide documentation of services performed. Contractor shall notify Contracting Officer (CO) or COTR of the existence or development of any defects in, or repairs to the equipment, which the Contractor considers he/she is not responsible (such as operator misuse). All exceptions to the Preventive Maintenance Inspection schedule shall be arranged and approved in advance with the CO. Emergency repair shall consist of replacing parts, calibration, cleaning, oiling, adjusting and maintaining the equipment except those services necessitated by accident, fire or abuse. Contractor response time to the COTR shall be within two (2) hours after receipt of notification in an effort to correct the problem by phone. If problem cannot be corrected by phone, the Field Service Engineer shall commence work (on-site physical response) within twenty-four (24) hours after receipt of notification and shall proceed progressively to completion without undo delay. Contractor shall furnish all parts necessary to maintain the equipment. The Contractor shall certify that he/she has ready access to new standard parts (manufactured, supplied by the manufacturer, or equal thereto). All parts supplied shall be of current manufacture and have full compatibility with existing equipment. Documentation of intended parts source(s) shall be provided to the Contracting Officer upon request. Contractor shall submit legible field service report, including detailed descriptions of preventive maintenance inspection or emergency repair services performed, including replacement parts and estimated prices required for the service call. NOTE: Any additional charges must be approved by the COTR before service was performed. Check-in is mandatory at the Electrical/HVACR Shop (Building 28, Room C100, during normal working hours. Upon service completion Contractor shall log out at the check-in point and submit the field service report to the COTR for signature, during normal working hours, or leave the service ticket with Boiler Plant operator after hours. Contractor_s staff shall include a _fully qualified_ Field Service Representative assigned and a _fully qualified_ Field Service Representative who shall serve as the backup. The Contractor shall provide written assurance of the competency of personnel by detailing a list of Field Service Representatives along with applicable training and years of service experience on the specific devices. This information shall be included in the quoted proposal for providing service. Upon request, Contractor shall provide copy of the current Calibration Certification of all test equipment, which is to be used to perform service. Calibration of equipment shall be traceable and in conformance with test equipment OEM standards. Contractor shall take such safety precautions as the CO may determine to be reasonably necessary to protect the lives and health of occupants of the building. Contracting Officer shall notify the Contractor of any safety issues and the action necessary to correct these issues. Such notice, when served on the Contractor or his representative at the work site shall be deemed sufficient for the corrective actions to be taken. If the Contractor fails or refuses to comply promptly, the Contracting Officer may issue an order stopping all or part of the work, and hold the Contractor in default. The following provisions and clauses apply to this solicitation: 52.212-1 Instructions to Offeror-Commercial Items, 52.212-4 Contract Terms and Conditions-Commercial Items, 52.212-5 Contract Terms and Conditions Required to Implement Statues or Executive Order Commercial Items with (b) (1), (14), (15), (16), (17), (18), (19), (21), (24), and (30) checked. FAR 52.217-9 Option to Extend Term of the Contract, FAR 52.217-8 Option to Extend Services, FAR 52.232-18 Availability of Funds, FAR 52.232-19 Availability of Funds for the Next Fiscal Year, FAR 52.233-2 Service of Protest, VAAR 852.233-70 Protest Content, VAAR 852.233-71 Alternative Protest Procedure and VAAR 852.270-4 Commercial Advertising also apply to this solicitation. Quotes will be evaluated on the following criteria: technical/service capability of equipment, price and past performance. Award will be made to the offeror who meets the requirements above and is most advantageous to the Government. The Government intends to evaluate the information received and award without discussions. Offers including the pricing information are to be submitted on company letterhead and include the information required in provisions 52.212-1, a complete copy of the provisions 52.212-3 Offeror Representations and Certifications-Commercial Items, literature/brochures on proposed service maintenance and the company DUNS number. Electronic copies of the referenced provisions and clauses are available on the internet at www.arnet.gov/far and www.va.gov/oa&mm/vaar/. Quotations and all required information must be received by 4:00 PM September 29, 2003. Quotations shall be submitted to Cynthia W. Maschat, Contracting Officer, VA Ann Arbor Healthcare System, 2215 Fuller Road, Ann Arbor, MI 48105. Faxed quotations are acceptable and may be faxed to (734) 213-3833. Solicitation number 506-07-04 must be listed on all documents for identification purposes. Only offers prepared in accordance with this notice will be considered. Contract will be base year with four (4) pre-priced option years.
