- MODIFICATION 03: THE PURPOSE OF THIS MODIFICATION IS TO INCORPORATE ADDITIONAL INFORMATION TO THIS SOLICITATION. THE FOLLOWING CHANGES HAVE BEEN MADE. A. RESPONSE DATE. The response date has been extended. All responses (proposals) must be received no later than 4:00 PM EST on 29 SEPTEMBER 2003 to 11th CONS/LGCF, 110 LUKE AVE., STE 200, BOLLING AFB, DC 20032 by mail or facsimile. An official authorized to bind your company shall sign the offer. B. CHANGES HAVE BEEN MADE TO THE FOLLOWING LINE ITEMS. LINE ITEM 0001: Clean Ducts and Exhaust Hoods for Bldg 50; The contractor shall provide all plant, labor, equipment, supervision, materials, transportation, and perform all operations necessary to clean the two (2) Exhaust Hoods and Ducts over ranges, boilers, and grills in Building 50 (Officer?s Club). QTY: 6 CLEANINGS, UNIT PRICE: $___________________, EXTENDED PRICE: $__________________. LINE ITEM 0002: Service Dryer Vents for Bldg 523; The contractor shall provide all plant, labor, equipment, supervision, materials, transportation, and perform all operations necessary to service the 13 dryer vents behind the east wall in Building 523 (the Base Laundromat) QTY: 6 CLEANINGS, UNIT PRICE: $___________________, EXTENDED PRICE: $__________________. LINE ITEM 0003: Clean Dryer Vents in Bldgs 15 and 38; The contractor shall provide all plant, labor, equipment, supervision, materials, transportation, and perform all operations necessary to service the 2 dryer vents in Building 15 (Fitness Center) and the one (1) dryer vent in Building 38 (Health and Wellness Center) semiannually. QTY: 2 CLEANINGS, UNIT PRICE: $___________________, EXTENDED PRICE: $__________________. LINE ITEMS 0004: THIS LINE ITEM HAS BEEN DELETED. SERVICES FOR BLDGS 370 AND 371 ARE NO LONGER REQUIRED. LINE ITEM 0005: Clean Ducts and Exhaust Hoods for Bldg 1301; The contractor shall provide all plant, labor, equipment, supervision, materials, transportation, and perform all operations necessary to clean the twelve (12) Exhaust Hoods and Ducts over ranges, boilers, steam tables and grills located in Building 1301 (Base Dining Hall) monthly. QTY: 12 CLEANINGS, UNIT PRICE: $___________________, EXTENDED PRICE: $__________________. LINE ITEM 0006: Service Dryer Vent System in Bldg 1302; The contractor shall provide all plant, labor, equipment, supervision, materials, transportation, and perform all operations necessary to service the two (2) dryer vent systems located in each laundry rooms on floors one (1) through nine (9), in both the north and south wings, located in Building 1302 (Blanchard Barracks). LINE ITEM 0007: Clean Ducts and Exhaust Hoods for Bldg 1310; The contractor shall provide all plant, labor, equipment, supervision, materials, transportation, and perform all operations necessary to clean the Exhaust Hood and Duct over the grill and deep fryer located in Building 1310 (Base Bowling Alley). QTY: 6 CLEANINGS, UNIT PRICE: $___________________, EXTENDED PRICE: $__________________. LINE ITEM 0008: Clean Ducts and Exhaust Hoods for Bldg 2482; The contractor shall provide all plant, labor, equipment, supervision, materials, transportation, and perform all operations necessary to clean the one (1) Exhaust Hood and Duct over the grill and range located in Building 2482 (Slip Inn) quarterly. LINE ITEM 0009: Service Dryer Vent System in Bldg 3618; The contractor shall provide all plant, labor, equipment, supervision, materials, transportation, and perform all operations necessary to service the four (4) dryer vent systems located on each the first and second floors (a total of 8 dryer vents) of building 3618 (Temporary Lodging Facility) as well as the Kitchen hoods located in each of the 50 rooms. LINE ITEM 0010: ____ Service Dryer Vent Systems in Buildings 3619 & 3620; The contractor shall provide all plant, labor, equipment, supervision, materials, transportation, and perform all operations necessary to service the dryer vent systems throughout each TLF Suite located in buildings 3619 (20 Suites) and 3620 (30 Suites) of the (Temporary Lodging Facility), semiannually. LINE ITEM 0011: Service Dryer Vent System in Bldg 3621; The contractor shall provide all plant, labor, equipment, supervision, materials, transportation, and perform all operations necessary to service the 5 dryer vent systems in the laundry room in building 3621 (Mathis Manor) semiannually. LINE ITEM 0012: Clean Ducts and Exhaust Hoods for Bldg 3623; The contractor shall provide all plant, labor, equipment, supervision, materials, transportation, and perform all operations necessary to clean the ten (10) dryer vent systems located in Building 3623 (Cheshire Dorm). QTY: 3 CLEANINGS, UNIT PRICE: $___________________, EXTENDED PRICE: $__________________. LINE ITEM 0013: Clean Duct and Exhaust Hoods for Bldgs 4455 & 4456; The contractor shall provide all plant, labor, equipment, supervision, materials, transportation, and perform all operations necessary to clean the two (2) Exhaust Hoods and Ducts over ranges, broilers, and grills located in the Child Development Center Buildings 4455 and 4456. LINE ITEM 0014: Clean Ducts and Exhaust Hoods for Bldg 4500; The contractor shall provide all plant, labor, equipment, supervision, materials, transportation, and perform all operations necessary to clean the two (2) Exhaust Hoods and Ducts over range grill and fryer located in Building 4500 (Burger King) monthly. Addendum to FAR Clause 52.212-4, Contract Terms and Conditions-Commercial Items (FEB 2002), Paragraph (g), Invoice; add subparagraph (9) to read: Payment will be made upon receipt of proper invoice for all accepted services.
