Description
This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. This solicitation is issued as Request for Quotation (RFQ) No. F49642-03-T-0197. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 2001-15. This is a small business set-aside. The North American Industry Classification System code is 238220 (formerly SIC code 1711) at a size standard of $12 Million. LINE ITEM 0001: Computerized Control System Inspection and Service: Non-personal Service. Contractor shall provide all plant, labor, tools, personnel, equipment, material, transportation, supervision, and all other items and services necessary to provide emergency maintenance in strict compliance with the requirements as set forth in the contract specifications (Statement of Work.) There will be 3 inspections. Period of Performance: 1 October 2003 through 30 September 2004. QUANTITY 3/INSPECTIONS UNIT PRICE $___________________ EXTENDED PRICE $________________________. LINE ITEM 0002: Emergency Intervening Service Calls (PPM): Contractor shall provide all plant, labor, tools, personnel, equipment, material, transportation, supervision, and all other items and services necessary to perform Principal Period Maintenance (PPM) for computerized control system in strict compliance with the requirements as set forth in the contract specifications during normal duty hours. ESTIMATED QUANTITY 15/HOURS UNIT PRICE $___________________ (ESTIMATED) EXTENDED PRICE $________________________. LINE ITEM 0003: Emergency Intervening Service Calls (OPPM): Contractor shall provide all plant, labor, tools, personnel, equipment, material, transportation, supervision, and all other items and services necessary to perform Outside Principal Period Maintenance (OPPM) for computerized control system in strict compliance with the requirements as set forth in the contract specifications. ESTIMATED QUANTITY 20/HOURS UNIT PRICE $___________________ (ESTIMATED) EXTENDED PRICE $________________________. LINE ITEM 0004: Reimbursable Parts: Contractor shall furnish all parts for the central Heat Plant Computerized Control System in strict compliance with the requirements as set forth in the contract specification (Statement of Work.) QUANTITY /LUMPSUM, Not To Exceed $5,000.00. NOTICE TO OFFERORS: Funds are not presently available for this project. No award will be made under this solicitation until funds are available. The Government reserves the right to cancel this solicitation, either before or after the closing date. INSPECTON AND ACCEPTANCE TERMS: Supplies/services will be inspected by Quality Assurance personnel and accepted at job site. STATEMENT OF WORK FOR INSPECTION, MAINTENANCE AND REPAIR OF COMPUTERIZED CONTROLS 1. DESCRIPTION OF SERVICES: The contractor shall furnish all management, tools, supplies, equipment, transportation, labor and all other items and services necessary to perform, inspect, repair, and maintain computerized controls at the Central Heat Plant (Bldg 18), Bolling AFB, in accordance with industry standards and terms and conditions of the contract. 1.1. INSPECTIONS. Inspections shall be made in December, March and September. The contract contractor shall perform all required repairs and services during these inspections. 1.2. PREVENTIVE MAINTENANCE. Preventive maintenance and calibration shall be performed on the equipment listed in Appendix A, during each scheduled visit. The contractor shall ensure operations of all equipment after maintenance and calibration operations are performed. 1.2.1. The following preventive maintenance and calibration shall be performed on the listed equipment during each visit: a. Check flame safeguard system on three (3) 25 million BTU, HTHW Generators. (1) Test all limits for both gas and oil firing. (2) Test FSP enunciator (3) Check all interlocks for start up and shut down cycles, including but not limited to purge, low fire start, fuel valve closing, pilot and flame scanner. Compare data to original test data; adjust as required to maintain original data. b. Check combustion of both gas and oil to six load points (minimum, 20%, 40%, 60%, 80% and 100% firing rate. Compare data to original test data, adjusting fuel to air ratios as required to keep unit efficiency to original levels. 1.2.2. The contractor shall provide a checklist for each system inspection and shall submit a copy to the Contracting Officer for Government approval before the first inspection. The checklist shall be used during each system inspection and shall be signed by the contractor after task is completed. A copy of the completed report shall be submitted to the Quality Assurance Person QAP. 1.2.3. A completed report of all tests shall be submitted to the Service Contracting Office within five (5) working days after each inspection/service visit. Reports will include results and records of condition found, adjustment, calibration sheet repairs or replacement made, and any pertinent recommendation for major tear-down, repair, overhaul, rebuild and/or replacement of components. 1.2.4. The contractor shall review operating procedures for computer Software and hardware along with plant foreman and system operators. Contractor shall perform required maintenance, diagnostic assistance of system software when unexplained shutdown is experienced and make minor program changes in accordance with Air Force Policy or recommended changes by the plant manager. 1.2.5. Interruption of Utilities: The contractor shall not interrupt existing utilities in the plant without first obtaining written permission from the QA or Plant supervisor, a minimum of seven days in advance. Request for outage shall be submitted in writing, and shall state the date, time and duration of proposed outages. The contractor shall make all final connections to existing utilities service. 1.2.6. Clean-Up. All material and scrap remaining from new work and classified as trash/debris shall be removed from the Base and disposed of by the contractor in a legally acceptable manner. It shall be the responsibility of the contractor to assure proper loading of all vehicles used for transport of materials and debris. The contractor at no additional cost to the government shall accomplish all clean-up activities. 2. EMERGENCY SERVICES. The Contractor shall provide emergency service calls at any time and shall respond within fours hours of notification. The contractor shall furnish telephone numbers for emergency call desk. Calls received by answering service or machine shall be considered as notification. 3. GENERAL INFORMATION: 3.1. CONTRACTOR REQUIREMENT: 3.1.1. Contract Manager. The contractor shall provide a manager who shall be responsible for the competent performance of the work. The name of this person and an alternate(s), who shall act for the contractor when the manager is absent, shall be designated in writing to the contracting officer prior to the contract start date and updated as necessary. The personnel manager or alternate shall have full authority to act for the contractor on all contract matters relating to daily operation of this contract. The personnel manager and alternate(s) shall be able to read, write, speak and understand English. 3.1.2. Contract Personnel. The contractor shall not employ persons for work on this installation if such employee is identified to the contracting officer as a potential threat to the health, safety, security, general well being, or operational mission of the installation and its population. Contractor personnel shall present a neat appearance and be easily recognized as contractor employees. Identification shall be worn or attached to outer garment at all times. The contractor shall provide the contracting officer a list of all personnel working under this statement of work (SOW) prior to contract start date and updated as changes occur or upon request by the contracting officer. 3.1.3. The management and control of contractor employees in performance of this contract shall be the responsibility of the contractor; however, the contractor shall comply with the contract requirements and Bolling Air Force Base regulations concerning conduct of employees. 3.1.4. The contractor shall have experience in servicing and maintaining the equipment listed in Appendix A and with Babcock software programming used in this application. Contractor shall be required to show certification and license to perform work on this equipment prior to award of the contract. 3.1.5. Coordination of Work. The work to be performed under this contract is on an active facility, therefore, prior to starting any work, the contractor shall coordinate all operations and inspections with the Quality Assurance Person and plant foreman. 3.2. CONTRACTOR FURNISHED ITEMS AND SERVICES. Except for those items and services specially stated in Government Property and Services, the contractor shall furnish everything needed to perform this contract according to its terms. 3.3. GOVERNMENT FURNISHED PROPERTY AND SERVICES. The government will furnish to the contractor, all water, electricity, and sewage in the performance of this contract. 3.4. HOURS OF OPERATION. Normal Hours of operations are from 7 AM to 4 PM, Monday through Friday excepted for Federal recognized Holidays: New Year?s Day Martin Luther King Jr.?s Birthday Inauguration Day President?s Day Memorial Day Independence Day Labor Day Columbus Day Veterans Day Thanksgiving Day Christmas Day 3.5. QUALITY ASSURANCE: The government will periodically inspect the contractor?s performance under this contract. 3.6. SECURITY REQUIREMENTS: All personnel employed by the contractor in the performance of this contract, or any representative of the contractor entering the government installation, shall abide by all security regulation of the installation. 3.6.1. Base Access. The contract shall obtain personnel identification passes for all employees and vehicle passes for all contractor and personal vehicles entering Bolling AFB. The contractor must complete AF Form 75, Request for Visitor/Vehicle Pass, and DD Form 1172, Application for Uniformed Services Identification Card, and submit it through the contracting officer to the Security Forces and Pass and Registration. Vehicle registration, proof of insurance and a valid driver?s license must be presented for all vehicles to be registered. 3.6.2. Lost vehicles and base entry passes shall be reported immediately to Security Forces and the QA. 3.6.3. PHYSICAL SECURITY. The contractor shall be responsible for safeguarding all government property provided for contractor?s use. All government facilities, equipment, and materials shall be secured. The contractor shall be responsible for placing all barricades and safety devices during any activities and comply with all Occupational Safety and Health Standards (OHSA). 3.7. Retrieving Identification Media. The contractor shall retrieve all identification media, including vehicle passes from employees who depart for any reason before the contract expires; e.g. terminated for cause, retirement, etc. 3.8. PERFORMANCE OF SERVICES DURING CRISIS DECLARED BY THE NATIONAL COMMAND AUTHORITY OR OVERSEAS COMBATANT COMMANDER. The contractor shall perform all services under this contract during a national crisis according to the Department of Defense Instruction (DODI) 3020.37. 3.9. WARRANTIES. The contractor shall be responsible for replacing any warranty item within the appropriate period of such warranty. This shall include manufacturer warranties. 3.10. ENVIRONMENTAL CONTROLS: Compliance with Laws and Regulations. The contractor shall be knowledgeable of and shall comply with all applicable federal, state, and local laws, permits, DOD, Air Force, and base environmental requirements and instructions. The contractor shall ensure that all subcontractors comply with the same. The contractor shall ensure policies and procedures are established that protect the safety and health of employees and the community to minimize or eliminate the risk of environmental pollution. If the contractor spill or release any substance listed in 40 CFR 302 into the environment, the contractor shall immediately report the incident to the Environmental Coordinator at 202-767-8600, in accordance procedures. The contractor shall notify the primary QA personnel or in his absence the alternate QA personnel. The contractor shall be liable for containment and environmental clean up of the spill or release of such substance. If spillage creates contaminated soil, the contractor shall at his own expense, remove the contaminated soil, properly dispose of the soil and replace it with uncontaminated topsoil and restore the grounds to their original condition. The contractor shall not dispose of contaminated soil or any hazardous materials on Bolling Air Force Base. APPENDIX A: TO RETAIN A COPY OF APPENDIX A, CONTACT 1LT JAMILA HODGES AND YOU WILL BE EITHER FAXED OR EMAILED A COPY. FAR PROVISIONS: The provisions at 52.212-1, Instructions to Offerors-Commercial Items and 52.212-3, Offerors Representation and Certifications-Commercial Items are hereby incorporated. Addendum to FAR Clause 52.212-1 applies to this acquisition is as follows: 52.222-25, 52.237-1, 52.252-1, 52.252-5: (b) DFARS (Chapter 2), and 252.225-7000. Offerors must include, with its offer, a completed copy of provision FAR 52.212-3 in full text. Clause 52.212-2, Evaluation--Commercial Items applies to this solicitation, specifically to the following criteria: The Government will award a contract resulting from this solicitation to the responsible offeror whose offer conforms to the solicitation and price is determined most advantageous to the Government.
