Description

MODIFICATION #1: This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation. Quotes are being requested and a written solicitation will not be issued. Request for Quotation (RFQ) 074031 is assigned and shall be referenced on any quotation. All incorporated provisions and clauses are those in effect through Federal Acquisition Circular 01-15. This solicitation is being conducted under the Simplified Acquisition Procedures (SAP) and a Small Business Purchase Set-Aside. The North American Industry Classification System (NAICS) Code is 334516, with a business standard size of 500. The solicitation will be distributed solely through the General Administration's Federal Business Opportunities web site http://www.fedbizopps.gov/. Hard copies of this solicitation document will not be available. The site provides downloading instructions. The proposed contract action will result in a Firm-Fixed Price, One-Time Buy, (all or none) type contract. The Government's requirement is for the following items, brand name or equal: TSI Incorporated, 500 Cardigan Road, Shoreview, MN 55126, the sole manufacturer, for a quantity of (10) each of PortaCount Plus Respirator Fit Tester Model #8020A and a quantity of (10) each of Particle Generator Model #8026 which shall contain the following salient characteristics:: PortaCount Plus Respirator Fit Tester #8020A, which includes (1)115V, PortaCount Plus; (2)AC Adapter (115 VAC); (3) Alcohol Cartridge; (4)Alcohol Fill Capsule; (6) Storage Cap; (7) two HEPA Filter#1602066; (8)Operation & Service Manual #1980092;(9) Alcohol Supplies #8016;(10) 3/16 and 1/4 inch hose adapters; (11) two spare alcohol wicks; (12) carrying case; (13) FitPlus Software; (14) Model #8927 RS-232 Interface Cable; (15)Training Video #1980246; Particle Generator #8026 which includes: (16) 50 salt tablets; (17) AC line cord; (18) operation and service manual. The above listed characteristics are intended to be descriptive, but not restrictive. Unless visibly marked the vendor is quoting an equal product, the offered item shall be considered the brand name product as referenced in this announcement. If quoting an "or equal" product, the vendor shall submit detailed specifications with their quotation for evaluation purposes. All responsible sources may submit a quote which shall be considered and the FBI reserves the right to request samples. All quotes shall be submitted to the address listed above no later than 4:00 pm, EST., September 25, 2003, and all quotes shall be clearly marked RFQ 074031, Attention: Janice Williams. Fax quotes will be accepted, and in fact, are encouraged at fax number (703)339-2750. No hand carried quotes will be accepted. The Standard Commercial Warranty is one year. FBO destination basis to the FBI Cheverly Warehouse, 2400 Schuster Drive, Cheverly, MD 20785. If offering an "or equal" product, the brand name of the product furnished shall be clearly identified on the quote. The evaluation of quotes and the determinating as to the equality of the product shall be the responsibility of the Government and shall be based on information provided by the vendors. The Government is not responsible for locating or securing any information which is not identified in the quotation and responsible to the purchasing activity. Vendors are hereby notified that if your quote is not received by the date/time stated above, your quote shall be considered late and therefore, not accepted. The following clauses and provisions are incorporated by reference and apply to this acquisition: Federal Acquisition Regulation (FAR) provision 52.212.1, "Instructions to Offeror - Commercial Items" (JULY 2003); FAR provision 52.212.2 "Evaluation-Commercial Item" (JAN 1999); FAR clause 52-212-4 "Contract Terms and Conditions - Commercial Items" (FEB 2002); FAR clause 52.212-5, Contract Terms Conditions required to Implement Statutes or Executive Orders-Commercial Items" (JUNE 2003); Subsection (a), (b), and (e) apply. FAR provision 52.212-2, Evaluation-Commercial Items "(JAN 1999), is tailored to read as follows: All responsible vendors may submit a quotation which, if timely received, shall be considered by the FBI. The Government will award the contract to the responsible offer or whose offer conforming to the solicitation will be most advantageous to the Government. The following factors shall be used to evaluate offers (i) Technical (ii) Past Performance (iii) Price. Technical and Past Performance (listed in descending order of precedence) is more important than price. Offerors shall provide names, addresses and telephone numbers of customers (please limit to a minimum of three (3) and a maximum of ten (10) references), either Government or commercial, that have received the product during the past three years (this information is needed for the evaluation of past performance). All reference information, for past performance purposes, shall be submitted on September 25, 2003, at 4:00p.m. EST. The Government reserves the right to award without discussion. Each offeror shall include a completed copy of the FAR provision 52.212-3, "Offer or Representations and Certifications - Commercial Items" (JUNE 2003). Vendor Question: I would like to know the make and model of the respirator(s) this will be used for? Answer/Response: Use of the portacount is not contingent on the make or model of the respirator, but eh class of respirator - tight fitting elastermeric respirators that may be used in a positive or negative pressure mode. The manufacturer or model of the respirator is irrelevant.
