Description

NA This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are be ing requested and a written solicitation will not be issued. This solicitation is DAHA33-03-T-0030 is being issued as a Request for Quotation (RFQ). The solicitation document and incorporated provisions and clauses are those in effect through Federal Acqui sition Circular 2001-15. This solicitation is set-aside 100% for small business. The NAICS code that applies is 722310 and business size is $5.0M. This action will result in a FFP Purchase Order utilizing simplified commercial acquisition procedures under full and open competition. This requirement is for dining hall attendant services at the 179 AW, Ohio Air National Guard facility located at 1947 Harrington Memorial Road, Mansfield, Ohio 44903-0179. Services are required eight hours per day, two days per month, twelve months per contract period, for a total of 24 days. It is recommended a minimum of six attendants per day on UTAs be furnished to assure performance of the service, however, this recommendation does not in any way minimize the contractors obl igation to use as many employees as are necessary for proper contract performance. The contractor will furnish all work required. Dates service is required: 18 & 19 Oct 03; 15 & 16 Nov 03; 13 & 14 Dec 03; 10 & 11 Jan 04; 07 & 08 Feb 04; 13 & 14 Mar 04; 17 & 18 Apr 04; 15 & 16 May 04; 12 & 13 Jun 04; 10 & 11 Jul 04; 14 & 15 Aug 04; 11 & 12 Sep 04. The contract period is 18 Oct 03 through 30 Sep 04 with Option Year 1 (FY05) and Option Year 2 (FY06). The following provisions are included in this acquisition: F AR 52.212-1, Instruction to Offers-Comm (Jul 2003), FAR 52-212-2, Evaluation??????Comm Items (Jan 1999). The Government will make an award to the responsible offeror whose offer conforming to the solicitation is the most advantageous to the Government. The following factors shall be used to evaluate offers: cost or price to the Government and past performance of vendor. FAR 52.212-3, Offeror Representations and Certifications,Comm Items (Jun 2003). A completed signed copy of the provision FAR 52.212-3 shall be submitted with any quotation.
