- Description
- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6 as supplemented with additional information included in this notice. This announcement constitutes the only solicitation, quotations are being requested and a written solicitation will not be issued. The solicitation number, N00259-03-T-0143 is issued as a request for quotation (RFQ). The North American Industry Classification System (NAICS) Code for this acquisition is 541990, Size: 6M. This procurement is a small purchase, unrestricted solicitation. The solicitation document and incorporated provisions and clauses are those in effect through FAC 2001-15. The contractor shall provide CLIN 0001: NON-PERSONAL SERVICE contract for an OPTOMETRIST peforming a full range of Optometrist services and procedures for the period of 29 September 2003 through 28 September 2004. QUANTITY: 1920 Unit of Issue: Hours. The duties to be performed shall be in accordance with the Statement of Work. Copy of the statement of work is available upon request. The following FAR and DFAR clauses and provisions are applicable and will be incorporated into the solicitation by reference, and will be available in full text upon request. The solicitation document and incorporated provisions and clauses are those in effect through FAC 2001-15.
