Description
This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR subpart 12.6 as supplemented with additional information included in this notice. This announcement constitutes only solicitation. Quotations are being requested and a written solicitation will not be issued. The solicitation number, N00259-03-T-0140 is issued for reference purpose only. Contractor shall provide supervision, labor, equipment, parts, and transportation to perform Full Service Maintenance. Statement of Work Instrument Refurbishment GENERAL REQUIREMENTS The contractor shall provide all services, materials and equipment necessary for the refurbishment of various types of instruments located in the Otolaryngology Department, Division at Naval Medical Center, San Diego (NMCSD). Refurbishment process is to be preventive maintenance program and to correct existing problems. Refurbishment shall not be performed on the following federal holidays: New Year?s Day, Martin Luther King?s Birthday, President?s Day, Memorial Day, Independence Day, Labor Day, Columbus Day, Veterans?s Day, thanksgiving Day and Christmas Day. REFURBISHMENT MAINTENANCE: ? Perform instrument refurbishment on-site and complete on the same day except for special instruments that require more care or special attention at an off-site repair facility. ? Perform instrument refurbishment . Otolarynogology will provide a list of instrument sets and the primary point of contacts [POCs] at time of award. ? The days of the week and the amount of instrument sets to be refurbished will be determined by the primary POCs and the surgical schedule. ? Make repairs to the extent necessary (as determined by inspection tests or disassembly) to ensure a functional system that will efficiently serve its intended purpose. ? Improvements/modifications shall be made only upon Otolarynology written approval and direction. ? Repair or replace any instrument broken during the refurbishment process. SELF-CONTAINED MOBILE UNIT ? Must be equipped with the latest in repair machinery ? hones, grinders, belt sanders, sand blasters, buffers, curing ovens, demagnetizers, air compressors and 70X microscope to complete instrument refurbishment on-site. ? Must be equipped with a full line of replacement parts and must use only Class VI and medical grade materials with certifications on file. ? Must be equipped with a powerful generator, ultrasonic cleaner and 30 to 40 gallon sink. ? Must be approved by the government. ? Must report to Security in Building 26 and provide a copy vehicle license, owner registration, and vehichle description for a 6-motn parking pass. Security POC is Master-At-Arms Chief 619-532-9966. PERSONNEL QUALIFICATIONS ? Must provide at least two instrument craftsmen/machinists with at least 10 years experience. Primary POC will evaluate experience by requesting references. CONTRACTOR CAPABILITIES ? Must include simple sharpening, realignment, color-coding, screw, spring and ratchet replacement, complete instrument refinishing, needle holder insert replacement, etching, demagnetizing. ? Must include on-site testing of laparoscopic instrumentation (material inegrity and leakage), complete laparoscopic instrument rebuild including replacement of major components and re-insulation of shaft and handles ? Must include bipolar forceps re-potting and coating. ? Must include complete eye and plastic instrument rebuild including reconstruction of .12mm teeth. ? Must include replacement of carbide jawas on needle holders and forceps including gold plating. ? Must include fabrication and customization of instruments at POC request. ? Minor repair of pneumatics and air hose repair. CONTRACTOR QUALIFICATIONS ? Must be located within the State of California. ? Must be registered with the Food & Drug Administration and licensed. SPECIAL INSTRUCTIONS: The contractor shall provide at least 4 free educational in-servces for the Otolaryngology Department on all aspects of instrument care and maintenance. The contractor shall coordinate the dates and locations with the primary POCs. The usage and/or requirements may change during the contract period of performance. The government will notify the contractor of any changes to the surgical instrument sets as well as any newly formed instrument sets. CONTRACTOR CHECK-IN/CHECK-OUT: The Contractor, or his representative, is required to report to the Material Management Department for visitor badges during the hours of 0730 - 2000 Monday through Friday, prior to and upon completion of any refurbishment performed. FIELD REFURBISHMENT REPORTS: The contractor, or his representative, shall furnish to the Otolaryngology Department a legible copy of the Field Refurbishment Report upon completion of all work performed. The contractor, or his representative, shall complete the Government copy of this Field Refurbishment Report to include the following: ? Date and location of service ? Description and quantity of instruments ? Name of technician FIELD REFURBISHMENT REPORTS ARE REQUIRED PRIOR TO APPROVAL OF ALL INVOICES CORRECTIVE MAINTENANCE: Monday - Friday, 0800-1700 hrs. -End of Statement of Work- The solicitation document and incorporated provisions and clauses are those in effect through FAC 2001-15. This acquisition incorporates the following FAR clauses: 52.212-1 Instruction to Offers- Commercial Item (OCT 1995). 52.212-3 Offeror representations and certification ? Past performance, technical capability based on the description requirements when combined are significantly more important than cost or price. Prior to Contract Award, the government will review the quotes for the purpose of ascertaining their technical capability.
