Description
This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation. A quote is being requested and a written solicitation will not be issued. Solicitation number F63CES31270100 and clauses are those in effect through Federal Acquisition Circular 2001-15. This acquisition is restricted to Small Business concerns. This acquisition is under North American Industry Classification System code 238330, with a small business size standard of $12M. The following clauses and provisions are incorporated by reference and are to remain in full force in any resultant purchase order: FAR Clause 52.212-1, Instructions to Offerors- Commercial Items (Oct 2000). It is amended to read: Submit signed and dated offer to 97 CONS/LGCB, 303 J Ave. Bldg. 302, Altus AFB OK 73523-5132 at or before 1630 (4:30 P.M) CST, Monday, 29 September 2003. Quotes may be faxed to (580) 481-5138 or e-mailed to tonya.chapman@altus.af.mil. Each offeror shall include a completed copy of the provision at FAR 52.212-3, Offeror Representations and Certifications-Commercial Items (Jun 2003) with the quotation. If a copy of the provisions are needed, they can be found on farsite.hill.af.mil. Contract Terms and Conditions FAR clause 52.212-4 (Feb 2002), Contract Terms and Conditions Commercial Items, is incorporated by reference and applies to this acquisition. FAR 52.212-4 is tailored as follows?changes in the terms and conditions of this contract may be made only by written agreement of the parties with the exception of administrative changes (such as changes in paying office, appropriation data, etc) which may be changed unilaterally by the Government. DFARS clauses and provisions that apply to this solicitation are DFARS 252.204-7003 (Apr 1992) Control of Government Personnel Work Product; DFARS 252.204-7004 (Nov 2001), Required Central Contractor Registration; DFARS Clause 252.212-7001 (DEVIATION), Contract Terms and Conditions Required to Implement Statutes or Executive Orders Applicable to Defense Acquisitions of Commercial Items (Jun 2003) is hereby incorporated by reference and applies to this acquisition with the following addendum: DFARS 252.204-7003 (Apr 1992), DFAR Clause 252.246-7000 (Mar 2003), Material Inspection and Receiving Report; DFARS 252.232-7003 (Mar 2003), Electronic Submission of Payment Requests. AFFARS 5352.242-9000 (Jun 2002) Contractor Access to Air Force installations and AFFARS 5352.223-9001 (Jun 1997), Health and Safety on Government installations also apply to this solicitation. A Firm-Fixed Price purchase order will be issued in writing. No telephonic responses will be accepted. STATEMENT OF WORK DESCRIPTION OF SERVICES: The contractor shall provide all personnel, equipment, tools, materials, supervision, administration and services necessary to remove, dispose of, purchase and install floor covering for Bldg 53, Child Development Center. The contractor shall coordinate all removals and installations with Quality Assurance Personnel (QA). A copy of the building drawings depicting areas receiving services will be supplied to the contractor. Contractor is responsible to field verify exact measurements and quantities of the area(s) to receive installation. All work is to be performed by Contractor/Installers who are CFI Certified Floorcovering installers or manufacturer?s approved installers. The contractor shall remove all flooring materials to accommodate the installation of new materials. The contractor shall clean and prepare existing substrates to receive new materials. The contractor shall furnish adhesives, seam sealer, seam sticks, welding rods, seam tape, tack strips, etc. for installation of floor covering materials Vinyl/Rubber Base Cove : Surfaces on which new base cove is to be applied shall be patched be free of all foreign materials. Rubber tile and products shall conform to ASTM F 1344. Base cove material shall be manufactures standard 120? coils. Manufactures preformed inside and outside corners shall match the straight length of base cove. The contractor shall not use straight lengths of base cove that are bent, folded, tucked or creased around corners. Cleanup and Disposal: Vacuum newly installed carpet using a machine with a face-beater bar element. Remove spots or stains and replace flooring materials where spots cannot be removed and clean base cove of all foreign materials. Clean all other flooring materials according to the manufacturer recommendations and specifications. The contractor is responsible for removing all waste materials and debris from the installation site and discarding at a state permitted disposal site. Performance Time: Installation will commence 30 calendar days after issuance of purchase order. Performance period will be a minimum of 30 calendar days. Installations shall be accomplished during normal duty hours of 7:30 a.m. to 4:30 p.m. Monday through Friday. Carpet, Broadloom Specifications: Carpet shall be comprised of recycled materials when possible. Broadloom carpet shall have 100% coverage on seam sealer. Mohawk ? All inside and outside corners will be factory made with the same height and thickness as straight runs of cove base being used.
