Description
NA This is a combined synopsis/solicitation for a commercial item, prepared in accordance with the format in FAR Subpart 12.6, and supplemented with additional information included in this notice. This announcement constitutes the only solicitation. Quotes are requested; however, a written solicitation will not be issued. Solicitation number DABJ25-03-T-0100 is applicable and is issued as a request for quotations. The solicitation and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2001-03. The requirement is small business set-aside with a NAICS Code of 611620, SIC Code 7999, and size standard of 5 mil. The description of the commercial service is as follows: CLIN-0001 BASE YEAR - 1 OCT THRU 30 SEPT 04 JIU-JITSU INSTRUCTION 3 DAYS PER WEEK: MONDAY, WEDNESDAY, FRIDAY 0630-0800 CONTINGENT UPON CONGRESSIONAL APPROVAL OF FY04 FUNDS CLIN 1001 - 1ST OPTION YEAR - 1 OCT 04 THRU 30 SEPT 05 JIU-JITSU INSTRUCTION IN ACCORDANCE WITH THE ATTACHED PERFORMANCE WORK STATEMENT CLIN 2001-SECOND OPTION YEAR-1 OCT 05 THRU 30 SEPT 06 JIU-JITSU INSTRUCTION IN ACCORDANCE WITH THE ATTACHED PERFORMANCE WORK STATEMENT. PERFORMANCE WORK STATEMENT SECTION C Jiu-Jitsu Instructor C1.0 Supervision and Work Hours. This is a non-personal service contract with a performance period of October 1, 2003 through September 30, 2004, with 2 option years to renew the contract. The 1st Special Forces Group (SFG) will provide practical technica l guidance to the Contractor but will not serve as a supervisory office. The SFG S-3 will provide operational guidance. Within established Army guidelines and regulations, the contractor shall perform independently and keeps the SFG advised of workload, status, concerns and issues that arise. Normal duty hours are Monday, Wednesday, and Friday from 6:30 AM to 08:00 AM except for normal Federal Holidays. Duty hours may be modified to meet operational requirements of the Government. C2.0 Standard of Service Provided. Services provided under this contract shall adhere to the high ethical standards of the profession they represent. The Contractor shall comply with the applicable requirements of the State of Washington and other applic able Federal and state labor and safety laws. The Contractor is expected to interface effectively with Army leadership and the installation. C2.0.1 The Government reserves the right to deny (bar from the post) an individual??????s access to Fort Lewis, WA, or its subinstallations for reasons of impropriety that relate to laws, professional ethics, or conflict with Army requirements. In these s ituations, the Contractor must provide an acceptable worker within thirty calendar days after receipt of notification that an individual has been barred from post. C2.0.2 Vehicle Licensing and Registration. The Contractor??????s vehicles shall be operated and maintained within safety limits prescribed by State of Washington and Fort Lewis, WA. All vehicles operated on installation streets, roads, and thorough-fares shall be properly licensed and registered with the State of Washington and Fort Lewis, WA, and shall adhere to smog regulations, equipment requirements and all other regulations and requirements imposed on all vehicles operating in the State of Washington . C2.0.3 Personnel Identification. Contractor personnel shall wear while at Fort Lewis, WA, a photo identification badge, which includes at a minimum a current photo, company name, and employee??????s name. C2.0.4 Safety Requirements: The Contractor shall work in a safe manner and in compliance with OSHA, Federal, State, and Army laws, regulations and policies. If the contractor fails, or refuses, to promptly comply with safety requirements, the Contracting Officer may issue an order stopping all or part of the work until satisfactory corrective actions have been taken. No part of the lost time due to any such stop order shall be made the subject of a claim for extension of time or for excess costs or damages to the contractor. C2.0.5 LEGAL PUBLIC HOLIDAYS: Except as otherwise specified in the contract, the contractor shall not schedule or perform routine work on holidays or days observed in lieu thereof, when a service is required less than three times per week and the schedule for that work falls on a holiday, the work shall be accomplished on the work day following or preceding the holiday, at the contractor??????s option. Legal public holidays are as follows: a. New Year??????s Day ?????? 1st day of January b. Martin Luther King??????s Birthday ?????? 3rd Monday of January c. Washington??????s Birthday ?????? 3rd Monday of February d. Memorial Day ?????? Last Monday of May e. Independence day ?????? 4th day of July f. Labor Day ?????? 1st Monday of September g. Columbus Day ?????? 2nd Monday of October h Veteran??????s Day ?????? 11th day of November i. Thanksgiving Day ?????? 4th Thursday of November j. Christmas Day ?????? 25th day of December C2.0.6 INSURANCE: Contractor shall obtain and maintain insurance coverage as set forth by the State of Washington. C2.1 Key Personnel Requirements. C2.1.1 Education. Instructor will possess the ability to communicate both orally and in writing. C2.1.2 Combination of Education and Experience. An acceptable instructor/advisor will have a combination of the following: --25 years of Martial Arts experience --Brazilian Jiu-Jitsu 3rd Degree Black Belt --No Hold Barred --Proven past experience training military personnel --Proven past experience training para military forces (law enforcement agencies) --Ability to conduct research and communicate (both orally and in writing). C2.1.3 Other Skills, Knowledge and Training Required: --Ability to instruct edge weapons tactics for military applications --Ability to instruct military personnel combative techniques in full combat gear --Ability to instruct empty-handed lethal techniques --Ability to instruct personnel on weapons retention while engaged in hand-to-hand combat --Ability to instruct on weapons disarming techniques --Ability to develop and write programs of instruction for military hand-to-hand tactics --Ability to instruct in simulated combat conditions --Develop instructional videotapes, DVDs, written products specific to unit requirements --Minimum of three years instructing military personnel --Must be able to train 50 or more personnel during blocks of instruction --Must provide lesson outlines and summary sheets for blocks of instruction --Must provide necessary equipment and training materials in order to conduct Advanced Instructor Certification --Must be able to read, write and speak the English language --Must possess instructional and motivational skills C2.1.4 Description of Services to be performed: Serves as the Special Forces Group hand-to-hand Combatives instructor and coordinator. Provides Enhanced Combatives training to selected individuals in order to attain Advanced Instructor Certification. Perf orms reviews and quality control checks on programs of instruction, and makes necessary adjustments to meet mission requirements of the unit. Ensures established policies and procedures are adhered to throughout the training. Uses a variety of tactics an d techniques to accomplish the required training. Develops written feedback and as required briefs the commander on combatives program progress. C2.2. Working Conditions: Work is conducted inside in an adequately lighted and ventilated facility or in a field environment. Work environment is subject to a wide range of eyestrain, back strain and pulled muscles. Substitute instructor may be utilized provided that prior permission is received from the SFG Commander or his designated representative. Substitute instructor will be familiar with the subject matter, instructional techniques an d current progress and ability level of personnel training. C2.3 Vacations/Illness/Injury. C2.3.1 The Contractor may request in writing for a vacation period not to exceed two weeks (without pay). The request must be provided to the SFG at least one month prior to the proposed vacation period. If ill and unable to work, the Contractor will be paid a pro-rated amount for the month. The formula for the pro-rated amount is the number of days divided into the monthly amount time the number of days worked. C2.3.2 In the event of illness or injury of any student during the conduct of a class, which may require emergency medical procedures or treatment, the contractor shall summon MP/ambulance assistance by dialing 911 and notifying staff. The Contractor is r esponsible for all costs of medical treatment for himself and any Contractor personnel. C2.4. Invoicing. The Contractor shall provide the Government a written invoice indicating payment due date on the 1st of each month in accordance with the contract terms and conditions. C3.0 Contractor??????s Management, Administrative and Evaluation Requirements. C3.1 Personnel Procurement. The Contractor shall have all necessary materials and equipment and begin full performance on the contract start date. C3.2 The Contractor shall be responsible for all management and administrative matters relating to compliance with state and local laws, policies and procedures. Any differences that arise at the installation shall be the responsibility of the Contractor for resolution. C3.3 The Contractor shall appear at all times in clothing commonly associated and appropriate with the position. The Contractor shall not sell any commodity or service to any individual on Fort Lewis, WA, or associated with this contract. C3.4 At the conclusion of the performance period, the contractor will receive a performance evaluation and will be provided an opportunity to provide comments prior to submission to the Past Performance Information Management System. C3.5 The Contractor may provide certificates, awards, or other forms of recognition to the participants at the training events at the Contractor??????s expense. The Government must approve the form and content of all Contractor furnished certificates, awa rds, and recognition. C4.0 Government Furnished Items. Except for the specific items noted in this paragraph and paragraph C5.4, the Contractor is responsible for the labor, administrative costs, materials, supplies, and services required to perform all of the requirements of the contract. The Government assumes no responsibility for costs incurred by the Contractor to perform the contract requirements that were not included in the contract price. This contract does not create an employer-employee relationship between the Gov ernment and any corporation, partnership, business association, or other party or legal entity with which the Contractor may be associated. C5.1 The Government (SFG) will provide facilities suitable for the safe and proper conduct of classes described in this contract. The Government reserves the right to cancel training at any time and the contractor will be notified in advance. Government facilities and equipment will be maintained and repaired/replaced by SFG when necessary for the safe and proper execution of this contract. C5.2 The Government will ensure that contractor employees have adequate access (as determined in the sole discretion of the Government) to commercial phone lines to perform their functions. The Government will bear the costs of phone use for direct servic es associated with this contract. Government telephones shall only be used for performance of the contract and shall not be used to make or accept personal calls. C5.3 The Government will provide utilities, including heat, electricity and water. The Contractor is responsible to use the utilities in a responsible manner and to conserve utilit ies when possible. The Contractor shall be aware of and notify the Government (SFG) of failure of any utilities and/or need for maintenance/repair in the interest of the health and safety of the participants. The Contractor shall use the utilities only f or the performance of this contract. C5.4 The Government shall provide: --Access to adequate training facilities space, automation and audiovisual equipment necessary to perform the work outlined in this contract --All necessary administrative office supplies --All necessary funding and supplies to supplement combatives equipment C6.0 PUBLICITY. The Contractor shall not disclose information concerning this contract or their activities at Fort Lewis, WA, unless approved of the Contracting Officer. FIRM FIX PRICE CONTRACT Requirment is FOB Destination only. Activity is Special Force Group, Fort Lewis, WA 98433-9500. Any award resulting from this request for quotations will be made to a responsive, responsible quoter using a SF1449 form. Quotes will be evaluated on a based of Technical and best value. Award will be made on an all or none basis. Provision 52.212-1, Instructions to Offertory-Commercial Service, applies to this acquisition. An addendum to this provision is applicable, in which the term offer or (whenever it appears) shall be replaced with the term quoter. Quoter are required to submit completed copies of the provisions at FAR 52.212-3, Offeror Representations and Certifications-Commercial Items, with their quotes. Also, please provide Federal Tax ID Number FAR clause 52.212-4, Contract Terms and Conditions-Commercial Items, applies to this acquisition and does not include any addenda. Far clause 52.212-5, Contract Terms and Conditions Required to Implement Statutes or Executive Orders, Commercial Items, a pplies to this acquisition.
