- NA This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are b eing requested and a written solicitation will not be issued. The solicitation number for this Request for Quotation is DADA15-03-T-0279. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Ci rcular 2001-13. This solicitation is 100% set-aside for small business; the associated NAICS code is 339113, no. of employees: 500. This requirement is for the Department of Surgery, Walter Reed Army Medical Center, Washington, DC. ITEM DESCRIPTION: 00 01 ?????? Ten (10) each - Nucleus 3 Cochlear Implant System OR EQUAL; includes one internal Nucleus, Two External Processors. Components to a system: Internal Nucleus (C124R(CS) Contour or C124K Implant; External Nucleus (Speech Processors: Sprint or ESP rit 3G); HS8 Sprint Headset; Accessory Kit. If offering an equal item, provide complete technical documentation and specifications to be able to evaluate. Delivery, which shall be FOB Destination to Walter Reed Army medical Center, shall occur within 30 days after award of the purchase order. Offerors responding to this announcement shall provide all information contained in the Federal Acquisition Regulation (FAR) Provision 52.121-1, Instructions to Offeror-Commercial Items. As stated in this provision , the Government intends to evaluate offers and award a contact without discussions with offerors. Therefore, the offeror??????s initial offer should contain the offeror??????s best terms from a price and technical standpoint. However, the Government res erves the right to conduct discussions if later determined by the Contracting Officer to be necessary. 52.212-12, Evaluation ?????? Commercial Items. The following factors shall be used to evaluate offers: a) Technical b) Past Performance and c) Price. 52.212-3, Offeror Representation and Certifications ??????
