- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in the notice. This announcement constitutes the only solicitation; quotations are being requested and a written solicitation will not be issued. This procurement is 100% set-aside for Small Business. This solicitation is to provide services of Radiology Technician to the inmate population at the Federal Bureau of Prisons, Metropolitan Detention Center, Los Angeles, CA (MDC-LA). Statement of Work: All duties will be performed in accordance with standards and methods generally accepted within the contractor?s field of expertise, including but not limited to the following: A) Maintain X-Ray log with the following information 1. Inmate/Patient Name and Register Number, 2. Type of Study, 3. Number of exposures and exposure technique, 4. Name of Technician, 5. X-ray Reference Number and 6. Number of retakes. B) Assist medical staff in the performance of special radiographic studies requested by the Clinical Director or designee such as intravenous pyelogram, and barium contrast studies. C.Perform continuous quality improvement monitoring which include but not limited to: 1. Perform routine quality control on the x-ray processor including the performance and documentation of corrective actions taken to improve quality. 2. Perform routine quality control on the x-ray units including performance and documentation of corrective actions taken to improve quality. (e.g. correctness for exposure) 3. Performance and documentation of routine infection control procedures such as ?wipe down? logs. 4. Updating changes in the x-ray technique procedures as needed, 5. Inspection of leaded gloves, aprons and gonad shield, 6. Assurance of proper patient preparation prior to studies and 7. Calibration of safety equipment. D. The Performance Improvement Reports will be turned in to the Performance Improvement Coordinator on a monthly basis to properly track and record compliance. E. Contractor must maintain coverage in case of absence and notify the Health Services Adm. or designee in writing and faxing it to 213-253-9511 or 213-253-9525, at least 24 hours in advance of any changes in personnel or schedule. F. Prepare and conduct continuing medical education program on a quarterly basis or upon request by the Health Services Administrator or designee. Continuing medical education would include training of new Mid-Level practitioner on basic x-ray techniques for chest and extremity x-rays and proper developing of films. QUALIFICATIONS: The provider must be: a. Certified by an approved technology training school. B. Licensed/ registered to practice this specialty in the state of California. C. Previous experience in a correctional setting is helpful. SCHEDULE: The contractor is to complete all scheduled x-ray procedures on a daily basis. There will be a minimum of 12 session hours per week. A session is one hour. The number of sessions will be based on the number of x-rays and special studies scheduled per day. Four days a week, a session will consist of 1.5 hour per day and one day of the week will consist of 6 hours for performing special studies. The session will be Monday thru Friday (except on federal holidays) between the hours of 6:30 am and 1:30 pm. The number of x-rays schedule per day will be approximately 10 to15 chest x-rays, 5 extremity x-rays, 1 to 2 skull or facial series and 3 to 5 lumbar series. On a weekly basis approximately 3 to 4 barium enemas, IVP?s or Upper GI series are to be scheduled and performed. Procedures not performed on inmates due to logistics or unavailability of the inmate (court, lockdown, etc.) will be rescheduled for the next working day. Any change is to be coordinated with the MDC-LA Health Services Administrator or designee. Each session will be documented in the contractors log book located in Medical Records by recording the arrival and departure time. INMATE MANAGEMENT: Contractor will be in contact with convicted felons while providing services in the facility. Contractor will conduct him/herself in an appropriate manner at all times and report any infraction of the rules and regulations by any inmate to the Bureau of Prisons Staff immediately. The contractor will attend an orientation program prior to assuming their duties within the institution. Reimbursement is authorized for the time spent in mandatory institution training, prorated hourly, upon completion and clearance of the background checks. The background checks are outlined in the section entitled Security Requirements. The list is subject to change at any time. The resulting contract is not a personnel appointment; therefore, payment is based on the provision of an end product or the accomplishment of a specific service. The resulting contract does not constitute an employee/employer relationship; therefore, the contractor will not be subject to government supervision, except for security related matters. The contractor performance will be closely monitored by the Contracting Officer Technical Representative (COTR). Contractor agrees to adhere to all rules and regulations prescribed by the institution and set out for contract professionals. The contractor shall attend an orientation training program provided by the institution for all contract and volunteer personnel. Performance will occur on a delivery order basis. Schedule of services will be established by COTR, Monday to Friday, except Federal Holidays. Quantity is an estimate only. SECURITY REQUIREMENTS: Contractors will need to complete an NCIC background investigation and adhere to all security requirements of the Federal Facility. The following will be completed prior to performance and entrance into the institution. FD-258 Fingerprint check, Law Enforcement Agency Checks, Vouchering of employers over the past five years, OF-306, Declaration for Federal Employment, and Appropriate Resume or Optional Application, Complete Contractor Pre-employment Form, Release of Information, Urinalysis - with disqualification if positive. Payment will be made net 15 days ARO by electronic funds transfer or Government Credit Card. Place of Performance: Metropolitan Detention Center, 535 North Alameda Street, Los Angeles, CA 90012. Period of Performance: The contract Base Period shall be from 10/27/03 or Date of Award, whichever is later, through 09/30/04, plus four (4) option years, 10/01/04 to 09/30/05, 10/01/05 to 09/30/06, 10/01/06 to 09/30/07 and 10/01/07 to 09/30/08. Offeror must provide prices per session and yearly total separately for: the Base period and Four Option Years. Pursuant to FAR 17.203(d), offerors may quote varying prices for option periods. The RFQ incorporates the following provisions and clauses that are in effect through Federal Acquisition Circular 2001-15(b). FAR Provision 52.212-1 Instructions to Offerors-Commercial Items (JULY 2003), subsection (a), (b), (f), (g), and (j) applies. Provision 52.212-3 Offeror Representations and Certifications-Commercial Items (JUN 2003), Provision 52.212-2 Evaluation -Commercial Items, Clause 52.212-4 Contract Terms and Conditions-Commercial Items (FEB 2002), Clause 52.212-5 Contract Terms and Conditions Required To Implement Statutes or Executive Orders-Commercial Items (JUN 2003), subsection (b)1,3,7,10,13-19 and 30 applies. Clause 52.237-2 Protection Of Government Buildings, Equipment, and Vegetation (APR 1984). The following FAR clauses are hereby incorporated into the contract by reference: 52.219-6 Notice of Total Small Business Set-aside; 52.223-5 Pollution Prevention and Right-to-Know Information; 52.224-1 Privacy Act Notification; 52.224-2 Privacy Act; FAR 52.237-07 Indemnification And Medical Liability Insurance, applies to this contract. The Contractor shall maintain during the term of this contract liability insurance issued by a responsible insurance carrier of not less than the following amounts(s) per specialty per occurrence: $1,000,000.00. 52.237-3 Continuity of Services; 52.232-18 Availability of Funds; FAR clauses incorporated in full text: 52.217-8, ?Option to Extend Services?: The Government may require continued performance of any services within the limits and at the rates specified in the contract. These rates may be adjusted only as a result of revisions to prevailing labor rates provided by the Secretary of Labor. The option provision may be exercised more than once, but the total extension of performance hereunder shall not exceed six months. The Contracting Officer may exercise the option by written notice to the Contractor within 15 days of the expiration of the contract year. 52.217-9 ?Option to Extend the Term of the Contract?: (a) The Government may extend the term of this contract by written notice to the Contractor within 10 days provided that the Government shall give the Contractor a preliminary written notice of its intent to extend at least 60 days before the contract expires. The preliminary notice does not commit the Government to an extension.(b) If the Government exercises this option, the extended contract shall be considered to include this option provision.(c) The total duration of this contract, including the exercise of any options under this contract clause shall not exceed 60 months (5 years); 52.216-21 Requirements: This is a requirements contract for the supplies or services specified, and effective for the period stated, in the Schedule. The quantities of supplies or services specified in the Schedule are estimates only and are not purchased by this contract. Except as this contract may otherwise provide, if the Government's requirements do not result in orders in the quantities described as "estimated'' or "maximum" in the Schedule, that fact shall not constitute the basis for an equitable price adjustment. The estimated quantities are not the total requirements of the Government activity specified in the Schedule, but are estimates of requirements in excess of the quantities that the activity may itself furnish within its own capabilities. Except as this contract otherwise provides, the Government shall order from the Contractor all of that activity's requirements for supplies and services specified in the Schedule that exceed the quantities that the activity may furnish within its own capabilities. This will be a firm fixed price contract. The award will be to the responsible, responsive bidder offering the best value to the government. Award will be based on past performance, technical factors and best price to the Government. Past Performance and Technical factors will consist of seventy percent of the proposal. PAST PERFORMANCE: a. Identify existing or past contracts with terms and conditions the same as or similar to those in this solicitation. Include contract number, point of contact, and telephone number. b. Provide any information encountered during performance of those contracts that would have an adverse rating, and steps taken to correct the adverse rating. PRICE: Provide written pricing as per schedule. EVALUATION NOTE: Technical and past performance, when combined, are substantially more important than price. OPTIONS: The Government will evaluate offers for award purposes by adding the total price for all options to the total price for the basic requirement. The Government may determine that an offer is unacceptable if the option prices are significantly unbalanced. Evaluation of options shall not obligate the Government to exercise the option(s). A written notice of award or acceptance of an offer, mailed or otherwise furnished to the successful offeror within the time for acceptance specified in the offer, shall result in a binding contract without further action by either party. Before the offer's specified expiration time, the Government may accept an offer (or part of an offer), whether or not there are negotiations after its receipt, unless a written notice of withdrawal is received before award.
