Description
The U.S. Government is conducting a market survey of industry to identify sources that are interested in the manufacture of the complete M67 Fragmentation Grenade, including all components and Load, Assemble and Pack (LAP), NSN 1330-00-133-8244, PN 9235492. The M67 Fragmentation Grenade is used to supplement small arms fire in close combat situations. The Grenade produces casualties by high velocity projection of fragments in a uniform pattern when detonated. This device is composed of three major components 1) the metal body, 2) a pyrotechnic delay fuze, 3)and the Load Assemble and Pack (LAP) of the explosive composition and components into the final assembly. The explosive composition would be provided a Government Furnished Material. The survey is being performed to identify sources capable of supplying the entire quantity of Hand Grenades, as a systems acquisition, over a five-year period. The estimated quantities over the five-year period are 51,000 to 61,000 hand grenades per month. Attached is a questionnaire requesting specific information regarding your company's capability. Top drawings for the M67 Hand Grenade contain restricted distribution statements. In order to obtain access to this "limited distribution" material, interested parties are to follow the following procedure: A completed AMSTA-AR Form 1350, Technical Data Questionnaire and the signed and approved DD Form 2345, "Export Controlled Technical Data Agreement: submitted to U.S. Army TACOM-ARDEC, Attn: AMSTA-AQ-APA, Shannon Wagner, Bldg. 10, Picatinny Arsenal, NJ 07806-5000, Telephone (973) 724-4152. The DD Form 2345 can be obtained at; www.dlis.dla.mil/jcp. At the top of the website page, open "documents" on the blue bar. If an offeror does not have a DoD Certification/Registration Number, the above website provides instructions for submittal of the required information to obtain one. Offerors should note the requirement included in the DD Form 2345 approval process for the submittal of the Company's State License, Incorporation Certificate, Sales Tax Identification Form or other Documentation, which verifies the legitimacy of the company. Documents containing restricted markings will not be released until a signed and approved DD Form 2345 is received along with the AMSTA-AR-Form 1350. All information submitted will be retained in the solicitation file as market research but will not be released to the public. MARKET SURVEY QUESTIONNAIRE FOR INTERESTED PRODUCERS FOR THE M67 GRENADE: Instructions: This survey questionnaire is not a procurement action, but simply an inquiry for interest. There will be no compensation by the U.S. Government for your response to this questionnaire. Please reference they survey question number with each response. 1. Identify your current Business Structure: a. Name of Company - include a contact person's name, company address, telephone number, email address, CAGE code or Duns Number. b. Company Size (please verify as either Large, Foreign, Small, Small Disadvantaged, 8(a), Woman-Owned Small Business, Historically Underutilized Business Zone concern, Veteran-Owned Small Business, or Service-Disabled Small Business., c. Identify whether you are interested in the acquisition as a prime contractor or subcontractor., d. If a Small Business Manufacturer, provide information as to how you or your joint venture intend to be in compliance with FAR Clause 52.219-14, "Limitations on Subcontracting", e. Describe your parent company and subsidiaries (if applicable), corporate evolution, fiscal stability and other relevant factors. If your organization does not have adequate resources (technical, manufacturing, personnel, etc) available, identify what type of sub-tiervendors or subcontractors your organization would partner with and the time frame for creating any business relationship. 2. Identify your organization's Facilities, Equipment, Success stories, unique capabilities, Environmental Concerns, Customer Service/Satisfaction, Storage, Shipping, Receiving and other Support Functions. 3. Identify your organization's current and previsous experiences with similar system buys. Your answer should address the following: a. What Government and commercial contracts/items/production lines and period of production, b. Whether the contracts have been satisfied, c. Who are the Customers and what are the quantities, d. What was the maximum monthly production, both concurrent and stand-alone? 4. Identify current major manufacturing processes, critical skills and equipment, 5. Identify current production capabilities of the integrator and component suppliers to meet the desired quantities. Include in your answer the following: a. What are your current production rate capabilities and what capabilities must be developed? b. What capabilities would be subcontracted? c. Whether your organizations historical experience with process or component outsourcing? e. What are your organizations projected schedule for long lead material and time to begin deliveries. 6. Identify your organization's quality system and your organization's contractor's quality philosophy. Your answer should include any information regarding six sigma, lean manufacturing, and Greenbelt or Blackbelt personnel and your organization's experience in critical defect inspection, implementation or the latest Government requirement on critical defects clause and Statistical Process Control (SPC) requirements, 7. Identify your organizations rate flexibility. Your answer should include your maximum production rate attainable, minimum sustaining rate, and minimum order quantity, 8. Identify your organization's points of contact that wish to be notified if or when the Army solicits for the production of M67 grenades.
